2 The majority of data demands are driven by the requirements of Category 7 (Results) Category 7 requires 3 types of data; Health Care and Process Outcomes Data Survey and Government Outcomes Data Other Outcomes Data Work Force Outcomes Data Leadership and Governance Outcomes Data Financial and Market Outcomes Data LTC Trend Tracker is a powerful tool to address the Health Care and Process Outcomes Requirements Good organizational performance levels for some areas of importance to the item requirements should be evident. Some trend data should be reported, and a majority of the trends presented should be positive. Early stages of obtaining comparative information should be evident. Provide at least three (3) key clinical outcome results over appropriate time frames. At least one of the outcomes should clearly show improvement over time across at least three data points.

3 If available, show your outcomes in comparison to competitors or to state or national averages, whichever seems most appropriate. Assisted Living Facilities (ALFs) and Developmental Disability Residential Services providers (DD) may choose to substitute nonclinical process outcome results.
