President s Message For the financial year ended 31st March 2010, the Home was able to achieve a surplus of S$1,107, In spite of falling donations and nursing fees collected during the year, the Home was able to turn around after more than 2 consecutive years of running into the red. The excess of income expenditure was due mainly to the Contingency Funds received, the write back of unutilised Integrated Framework fund and the effective control measures implemented in reducing wastage and discriminate spending. We will continue to be prudent in the cost management of expenses and resources. The in-house ITE Training Centre will be opened in September 2010 to cater mainly to our nurses, nursing aides and nursing assistants. The Management Committee is committed to ensuring that staff received appropriate training to upgrade their skill and knowledge in discharging their duties. During the year, the Home held a half-day workshop on "Attitude and Building Relationships" conducted by an experienced trainer who had volunteered her kind services to 22 senior staff including Heads of Department. We believe and sincerely hope that with proper training in attitudes and behaviours staff can become better. When they become better, they can serve the elderly residents and clients better. By becoming a "better" staff would mean more responsible, more loving, more sensible and more compassionate. Another exciting development for the Home is the leasing of a piece of vacant land adjacent to the Home from HDB. In line with the nursing care services; we intend to provide garden therapy for our elderly residents. Many of them are suffering from dementia and garden therapy has been proven to be beneficial to their physical and mental well-being. The proposed site will be used for planting fruits, vegetables and flowering plants. The construction and design of the garden is earmarked in mid- September Besides garden therapy there is another tangible advantage derived from planting. Similarly when we give a man a fruit, he relishes for a day. Teach a man to plant, he gets a bountiful supply for a lifetime. I would like to thank all our donors and volunteers for their continuous support and contributions. I would also like to express my gratitude to the Management Committee members and dedicated staff for their valuable contributions and encouragement towards the growth and development of the Home. 8 Operations Review SWAMI Home provides 7 integrated services catering to more than 600 elderly in a month. The next few pages will take you through the operations of various departments which put aim in achieving the mission of providing holistic care to the sick and needy elderly. 8

Residential Care Geriatric and Dementia Residences SWAMI Home s Residential Care Services comprise of 190-bedded Geriatric and 52-bedded Dementia Residences. Our residents consist of mainly the elderly requiring long term nursing care as well as those with physical and mental disabilities. Our team of dedicated nursing staff and social workers aspires to meet the nursing and medical needs, as well as the psychosocial needs of the residents. It is our top priority to ensure that all our residents basic needs are adequately met and that they are comfortable during their stay in SWAMI. The team works in unity to provide comprehensive and holistic care for our residents. To further augment the level of care for our residents, we have adopted an integrated framework approach, which is supported by a team of resident medical officer, geriatrician, psycho-geriatrician, rehab physician, pharmacist, podiatrist, speech therapist, and dietician. The residents benefit substantially from these additional professional care without having to pay an extra fee. In addition to nursing and medical care, we are also concerned for the psychosocial needs of the residents whereby we involve them in a variety of activities. We aim to create a socio-friendly living environment for the residents; this is made possible with the joint efforts from our team of occupational therapists, together with the support of nursing staff as well as our social workers. Pet assisted therapy held once a month puts a smile on the faces of the residents as they play with the friendly dogs. Residents also get their daily dose of Vitamin D when nurses bring them to the garden every morning for their sunlight therapy. Apart from that, birthday celebrations for residents and staff are also organized every month. It is a fun-filled event with competitive games and attractive prizes to be won. On some occasions, there are performances put up by volunteer groups. Since the opening of SWAMI Mama Store Sunshine Shop in 2007, it has been very well received by the residents as they get a chance to purchase tidbits, cakes, etc. from the stall. It has now become part of their routine to eagerly look forward to its opening. Cooking therapy together with a volunteer and nursing staff is also one of the activities that the residents enjoy once a week. They helped in preparing the ingredients and afterwards enjoy the food they cooked all. Excursions for residents about thrice a month coordinated by the physiotherapists and the nursing staff let the residents connect with the outside world. 9

Residential Care Geriatric and Dementia Residences (Continued) Reminiscence Therapy is another useful intervention with residents having dementia condition. A room specially decorated with items used in the olden days and pictures of the past stimulate their memories. Every week, residents would gather in a Reminiscence Room to share with nursing staff, their stories of their past. Dementia Residences is furnished in such a manner to simulate a cosy family-like environment with sofa sets, sewing machine and personal cupboards storing cosmetic items for the residents. SWAMI emphasizes the importance of continuous education and training of our staff in order to upgrade their knowledge and skills. Monthly lectures on medical topics are conducted by our visiting geriatrician for all grades of staff. They are also sent for external courses related to Geriatrics and Dementia Care. On-the-job training forms an integral part of the nurses work life and are conducted by registered nurses. A multidisciplinary discussion of residents medical conditions is being held twice a month with the doctor, nurses, social workers, and therapists. Interactive learning is also being carried out in Dementia Residences whereby each staff takes turn to share medical topics issues of interest once a month. In order to improve basic communication skills for our foreign staff, weekly Hokkien and English lessons are being conducted internally by our local staff. Training on soft skills was introduced in June 2010 with staff doing role-play twice a month to have knowledge on excellent service. SWAMI Home has also been taking in nursing students for clinical attachments from Tsao Foundation and Ngee Ann Polytechnic. This has offered our senior nurses the opportunities to educate and share experiences with the students on Geriatrics and Dementia Care courses. This in turn enhances the students understanding and learning in the area of Geriatrics Care. In our continuing bid to establish SWAMI Home as a conducive and interactive living environment for the residents as well as our staff, the Residential Care Department is on track to create more social activities, continued effort to motivate our staff and to come up with new care management directives for our residents and staff. 10

Dementia Day Care Centre Dementia Day Care Centre (DDCC) is part of the community-based healthcare services available in Singapore for clients requiring intermediate and long-term healthcare services. And SWAMI DDCC is one of the 5 Dementia Day Care program under the purview of NCSS. Our centre provides structured day care program for seniors diagnosed with dementia. This centre-based service would provide the care person or family member a respite from their care-giving commitments during the day. The healthcare personnel are equipped with the skills and knowledge on how to cope with the demands of caring for those with dementia. The service areas which the centre mainly serves include Yishun, Ang Mo Kio, Woodlands, Sembawang and Marsiling, and later with boundaries redrawn, Yew Tee and Kranji will come under SWAMI S purview. There are different transport schedules in place to cater towards the diverse needs of caregivers. We have a team of staff, ranging from the senior manager, occupational therapist, nursing aides and health attendants. Although most are non-singaporeans, they have over time mastered dialects and are able to communicate with their clients effectively. They engage our clients in a wide variety of therapeutic activities (games and exercises) that stimulate the mind, body and heart of our clients. These activities are geared towards preserving if not improving the cognitive function, memory recall, physical strength and performance in Activities of Daily Living (ADL), thereby sustaining the functional capabilities and emotional well-being of our clients for as long as possible. Other therapeutic modalities include Reminiscence Therapy, Reality Orientation, cognitive games and simple arts and craft activities. These aid in maintaining the cognitive function, memory recall, work skills and behaviour of our clients. We also conduct diversionary activities like bingo games, karaoke, majong and birthday celebrations and arrange outings to encourage socialization among our clients. This will ultimately benefit the emotional well-being of our clients. Our Philosophy of Care is therefore one which is holistic and dynamic and which takes into account the total bio-psycho-social, cognitive and mental aspects of our clients. In conjunction with World Alzheimer s Day (21 st September), there would be a public forum on the 25/9/10. All these sessions will be conducted concurrently in English and Mandarin from 9:00am to 1:00pm. The items on display are works of handicraft done by our clients in Day Dementia Care. This is a testimony to the daily activity program that has immensely benefited our clients. 11

Day Rehabilitation Centre The Day Rehabilitation Centre aims to provide quality rehabilitative therapy for clients suffering from the aftermath of a stroke, de-conditioning due to genetic diseases such as Parkinson s disease and those who require rehabilitation for regaining their functional abilities. The centre also incorporates social and recreational activities such as outings, karaoke sessions and art therapy for clients to enhance their social well-being and also to encourage them to interact with one another in the centre. For some clients who are unable to cope with their physical conditions, we provide additional counselling and empowerment to help them deal better for a better quality of life. For this financial year, the number of referrals has increased slightly as compared to last financial year. The main source of referrals comes from Agency of Integrated Care (AIC), which is currently the mandatory route of referral from hospitals. The centre sees younger stroke clients now as compared to last financial year and with this trend, we also have more discharges as most of these younger clients take shorter time to regain their functional abilities and are able to function in the community. 12

Home Medical & Home Nursing Services In FY , SWAMI Home Medical & Home Nursing continue to provide service to the client who are homebound. Currently, we service clients that reside at Sembawang, Admiralty, Marsling, Woodlands, Chong Pang, Yishun, Ang Mo Kio, Khatib and Yio Chu Kang. This October 2010, we will cover two new areas, which are Kranji and Yew Tee. Home Medical Services As at 31 st March 2010, we have 41 Home Medical clients, 27 new admissions, 13 discharged cases and 18 deceased cases. For the 13 discharged clients, some of them were admitted to nursing home and some of them were readmitted to hospitals. In total, the Home Medical team had made 314 visits for FY The Home Medical team visits our clients every Wednesday. The team comprises of Doctor Lim Shee Lai and one nurse. During the Home Medical visit, the doctor reviews clients general conditions, medications and prescribed the medications that they need. Doctor Lim also advises the caregiver who looks after the client and changes the Percutaneous Endoscopic Gastrostomy and Tracheostomy tubes. The Home Medical and Home Nursing service fees are based on means testing. For those clients with difficulty in paying the minimal sum, we will refer the case to our Executive Director for SWAMI assistance. Home Nursing Services As at 31 st March 2010, we have 79 Home Nursing clients, 109 new admissions, 47 discharged and 25 deceased. In total, our team had made 4,626 visits to our clients. Most of the discharged clients are either their wounds have recovered or the doctor had removed their nasogastric tube or urine catheter in the hospital after their review. When the client s general conditions deteriorated and require higher nursing care they will be admitted to nursing homes. Our Home Nursing team continues to provide services to the homebound clients who require nursing care. Our services comprise of: Wound management Change of nasogastric tube & urine catheter Blood pressure and glucose monitoring Caregiver teaching Injection Supervision of medications There is no place like HOME Our team of nurses has been attending courses to upgrade their knowledge and skills. With the support of SWAMI Home management, our team will continue to serve our community clients to enable them to reside as long as possible with their family. 13

Home Help Service Home Help service was launched in August 2000 and we are now entering into the tenth year of operation. SWAMI Home Help Service Department has managed to uphold good service quality in providing the five services to our homebound needy elderly persons. We even went further to aid our services to needy homebound patients who do not meet the admission criteria. We also address these prevailing service gaps within the community service care sector to Ministry of Community Development, Youth and Sports (MCYS) and the National Council of Social Services (NCSS). With 10 health care assistants, the Home Help Service operates daily including Sundays and public holidays. We served 190 (as at 31 st March 2010) needy home bounds elderly clients. This has provided respite to more than 80 caregivers who are mainly low-income earners unable to employ domestic helpers for the patients. The profiles of our Home Help users include more than 85% elderly persons mainly of multiple service users. Most Home Help users avail the meals-onwheels and medical escorts services. Only 6% of the users receive personal hygiene and 20% receive housekeeping assistance. Home Help users are provided with these services at a minimum charge after the means testing was done. With this assistance, homebound patients basic needs of daily living are met. This enables them to continue to live in the community and not resort to institutional care. Based on evidence taken from service utilisation statistics over the years there has been a consistent need for meals-on-wheels and medical escort assistance. For the past one year there has been an increase in the need for housekeeping. With greater community outreach efforts to instil service awareness and a growing need for these services, it is anticipated that user numbers will increase significantly, putting strain on the current available resources. Corporate companies have also volunteered their services to the elderly in the community, for example Sembcorp has offered its services to volunteer to spring clean the elderly clients houses for three consecutive years with effect from March 2010, and host a dinner for the elderly. Avargo Technologies offered a one-day befriending and have lunch with for elderly clients. Sembcorp Volunteers Spring Cleaning Avago Technologies Befriending Lunch with our Elderly Meals-On-Wheels Assistance Meals-On-Wheels assistance is provided to homebound elderly clients who do not have the ability to cook for themselves. As of 31st March 2010, 120 elderly clients received this service with most of them mentioning that they were satisfied with the meals provided. 14

Home Help Service (Continued) The menu is prepared by our cook and approved by the dietician. A high quality is maintained for all meals in order to provide patients with nutritious food. The food is also packed hygienically using Styrofoam box and is kept warm in airtight insulators before being delivered to the elderly clients. At present, we deliver meals to the elderly clients twice a day, as it is our utmost concern to provide hot meals to our elderly clients. Medical Escort Assistance Most of our service users are referred by the hospitals during discharge planning; hence they are required to attend regular medical follow-ups to ensure the effective maintenance of their health. As at 31 st March 2010, 123 elderly clients received medical escorts assistance. Despite our best efforts to cater to every patient s needs, there have been times, mainly due to a very busy escort schedule where the department has been unable to provide the service to all users using our own vehicle. In order to meet the needs of our users whose medical appointment are important, the department try to change the medical appointments to the next earliest dates. We also liase with hospitals to ensure not to have too many medical appointments on the same day. The donation of vehicle from Singapore Power also eased the problems we faced in the shortage of vehicles. House Keeping Assistance House Keeping assistance is usually rendered to homebound elderly persons who are staying alone and who are not able to carry out daily housekeeping tasks to maintain hygienic conditions for healthy living. Currently there are 30 users requiring this assistance and it is our goal to reach out to more homebound or semi homebound elderly persons staying alone, usually in a one room flat, so that their home conditions can be improved. Personal Grooming Assistance This service is mostly provided to wheelchair-bound patients. As at the end of FY09/10, we have 9 patients requiring this service. Trained home-helpers are deployed to carry out the personal grooming so that careful handling of the patients can be achieved without causing mishap resulting hurt and injury. Moreover, privacy of the patients is strictly observed to ensure their dignity is maintained at all times. Future Goals We hope to reach out to more needy elderly persons who are suffering in the community but are unable to get any help. Outreach activities will encompass a major portion of our work for the next financial year, so as to increase community awareness of our home help service and its role. We hope our continuous process of intervention for frail elders with multiple service needs has enabled them to remain in their own homes for as long as possible, thereby enabling the seniors to age-in-place. 15

Rehabilitative Therapy Physiotherapy Physiotherapy and Occupational Therapy Departments are the two sections of Rehabilitative Therapy services provided in SWAMI home. Physiotherapy is concerned with maximizing our clients movement and function by the use of different exercise programs and techniques to promote, maintain and restore their physical, psychological and social well-being. In FY , the physiotherapy team implemented highly effective exercise programs to cater the needs of 267 residents and 178 Day Care clients representing 20% increase in our clients served compared to that of FY In conjunction with our roadmap the following goals were achieved: 28 ambulant residents achieved their maximum ability to walk with or without assistive devices thus promoting their independence to move around our premises. 68 wheelchair bound residents who are able or unable to propel their wheelchairs are able to walk in the parallel bars with minimal to moderate assistance promoting weight bearing and increase in standing balance and tolerance necessary in every aspect of their activities of daily living. Only 14 residents are in the refuse cases decreasing about 10% of the ones in the list. Still in the process of working out with them so that they will join in the exercise programs. 20 wheelchairs bound and bed bound residents benefited to our Chest Physiotherapy programs. Out of the 109 clients served in the Day Rehabilitation Centre, 39 of them significantly improved in their muscle strength, joint range of motion, balance and ambulation. Sixty-four (64) of them maintained their current function and 19 of them declined in their functional status due to their age and other illness like heart disease. Out of the 59 Dementia Day Care clients served, 13 of them significantly improved in their function, 38 maintained their current function and 8 of them have functionally declined due to the progressive process of Dementia. In verifying how well our clients sustain these positive gains in the area of their functional status, progress reports are being done quarterly and reassessment is done every 6 months from the day our clients were admitted to our program. In order to maintain these standards, our team includes one physiotherapist in-charge of evaluation and formulation of treatment plans according to the client s case and three therapy assistants to help in the implementation of the treatment plans. One health care assistant is also a part of the team to give support especially in logistics. All team members are trained in the area of physiotherapy and are committed to serve the residents and clients with sincere, tender love and dedication. 16

Rehabilitative Therapy Physiotherapy (Continued) In addition, our Home Physiotherapy program is still on going and has helped 16 clients from Sembawang, Yishun, Marsiling and Ang Mo Kio areas. This program is especially catered for those cases that need a higher level of care in their home. They are usually bed bound elderly people who are very weak, unable to tolerate long sitting or need exercises. Caregivers are given two hour of training on the proper transfer of patients from bed to wheelchair or vice versa, correct bed positioning. They encourage the patient to do exercise at home. Below are the Physiotherapy programs and interventions that we are currently implementing: 1. To increase and maintain the elderly residents and clients muscle strength and endurance which is facilitated by cycling exercises, pulley with weights, dumb bell exercises, NK table, gait training in the parallel bars, staircase or with the use of quad cane or walking frame. 2. To restore or increase their joint range of motion by shoulder pulley exercises, spring exercise, shoulder wheel exercises, passive range of motion exercise and manual stretching exercises. 3. To decrease pain using ultrasonic therapy, paraffin wax, electrotherapy, massage, hot and cold packs. 4. To prevent contracture and deformity of limbs by consistent exercise and increase their motivation in their exercise regime. To continue meeting a higher standard of care, our partnership with Tan Tock Seng Hospital Rehabilitation were strengthened by sending us consultants to help us integrate intensive rehabilitation for our potential clients to have higher chance of recovery. A new consultant who visit once a month is Dr. Samantha Yap who joined us in January Their inputs helped the Rehabilitative Therapy team go for the common goal of living their lives to the full. We are grateful to Tan Chin Tuan Foundation s Project Solid Gold for making us one of their beneficiaries. Their consultant who is a physiotherapist helped to revamp the Physiotherapy Centre in order to maximize space. They donated mirrors, cycling machines, grab bars and provided training for all the RT and DRC staff to fully utilize these equipment. In this project our clients benefited from various exercise techniques depending on their abilities. 17

Rehabilitative Therapy Occupational Therapy (Continued) The main goal of the Occupational Therapy team is to continuously facilitate therapeutic activities to enhance the residents and clients well-being utilizing the following treatment components: 1. 31 Calendar of Activities SWAMI Home in 2009 Unlike last year, which was marked as a busy year, this financial year (FY ) is notable because of a 3-month hiatus of activities at SWAMI Home brought about by the threat of H1N1 Influenza virus. Previously confirmed visits were cancelled and eventually activities by external groups were temporarily discontinued. This hiatus, which ran from May-July 2009, also included a temporary cessation in residents excursions. On those months, elderly residents and clients contented themselves with the usual daily activities organized by our physiotherapists and occupational therapists. It was only in August 2009 that the activities by external groups had resumed putting an end to the boredom of the residents who felt that activities facilitated by our therapists were too monotonous. Apart from the hiatus, this year was also distinct for the different roles that SWAMI Home has played apart from being an institution that gives care to the elderly. As a Venue for Corporate Social Responsibility Most often, SWAMI Home would witness several companies holding their corporate social responsibility programme at its premises. Otis Elevator Pte Ltd staff play for the residents Staff together with some of the bigwigs of their company would spend mostly half days to entertain the residents, celebrate birthdays together with the residents, or do painting works for the Home. After the activity, some of these staff ended up being a volunteer of SWAMI Home. As a Partner of Schools Community Involvement Programme Just like last year, SWAMI Home plays an important role to younger generation especially the students, in instilling social responsibility to the less fortunate elderly citizens by becoming a partner to their Community Involvement Programme (CIP). Several times in a month, throngs of students from different schools (primary to universities) flock to SWAMI Home to facilitate games for the elderly residents and clients, help out in their exercises, entertain them with performances and even befriend them for the day. A Nanyang Poly student declared his reflections after his CIP at SWAMI Home We should not take our grandparents for granted. 31

Calendar of Activities (Continued) SWAMI Home welcomes these students who can help in making its elderly residents happy and will continue to support schools as a partner for their CIP programme. As a Partner of MCYS Rehabilitation Programme CSO youths making jewelleries to sell at the charity funfair. SWAMI Home has also been an active partner of the MCYS on its rehabilitation programme of juvenile delinquents. For FY , SWAMI Home accepted close to 40 youths to serve their probation order at the Home. These youths spent their community service order (CSO) doing cleaning services, assisting elderly residents and clients during their exercises and sometimes helping out in simple administrative work. CSO youths had been very helpful to SWAMI Home providing extra hands where it is most needed. These youths proved to be very useful during the last fundraising event - the SWAMI Home s Charity Food and Fun Fair held on December CSO helpers sent by MCYS were most helpful during the preparations such as promptly providing needed supplies to stallholders during the event. In the course of the preparation, the youths felt sense of belongingness and took pride in the tasks given to them. Likewise, SWAMI Home is happy to be an instrument in the rehabilitation of these youths. Please trust us this time. We will really distribute these flyers, said one of the CSO youths whom I entrusted with the task of handing out our charity funfair flyers to shoppers who were queuing up in a nearby shopping center early in the morning. I just told them beforehand Without me going with you, you can just throw all the flyers in a bin, walk around and come back after an hour and tell me you did distribute them. But I trust you, that you wouldn t do it because you know why SWAMI Home is doing this fair. I believe the students did a good job because after an hour I noticed that there hordes of people going to our fair. Indeed, we had a high turn out of visitors on that fateful day. Thanks to those youths! --Ann, PR Executive As a Learning Hub Apart from being an organization, which has been consistent in its mission of providing quality eldercare to needy elderly, SWAMI Home has also been a vital channel when it comes to getting practical knowledge. SWAMI Home hosted visits of young professionals, interns and social workers who are keen in learning the eldercare system of SWAMI Home. Several batches of students who are taking eldercare courses from TSAO Foundation had completed on the job training at SWAMI Home to get practical knowledge in caring elderly. Meanwhile, young professionals working in hospitals or government Social workers from Korea at SWAMI Home 32

Calendar of Activities (Continued) agencies holding front line positions in eldercare also visited SWAMI Home to get more information on this sector. SWAMI Home not only shares experiences with local professionals in the social service but also with social workers and government officials from Korea, Macau and Japan. During their visits last year, they were very eager to learn SWAMI Home s eldercare system to replicate in their own countries. Snapshots of Events and Activities from April 2009 to March

The centre was closed for five weeks during Christmas holiday season. The program is being run once a week on Thursdays from 10 am to 2 pm. Client attendance ranged from 15 to 28, with an average around 22. Client list is made up of regular clients of around 30 and 10 more who attend occasionally. The program is supported with regular attendance of 8-10 volunteers from a pool of around 15 volunteers, while few more provide occasional support. The volunteer s support is adequate for the present level of activity. The Day Activity program was stream lined and improved with frequent interactive sessions of sharing of jokes, quiz, games and activities to stimulate the mind, crafts, and physical indoor games. Taichi, yoga, chi kong and line dancing sessions were conducted regularly. Table tennis board is being used by interested members. Usually up to about 8 clients are engaged in mah-jong game. SWAMI committee likes to thank both the members for their service effort in raising funds for the association. Client feedback had been positive for the Day activity program, as indicated by the regular and enthusiastic attendance of the clients. The client contribution was retained at A$8.00, despite similar program run by Melville council charging A$ Occasionally some of the clients offer voluntary additional contributions. For details of activities refer annexure. Finance During the year, the association maintained Bank accounts with Westpac.
