2 VISION/MISSION Our vision is to be a One-Stop Service Centre to Better Serve the Community. Our mission is to promote social services to needy families and elderly residents and jobless living in Yuhua and nearby areas and also to encourage community bonding to all residents and Singaporeans. POLICIES The Centre is partially funded by the Government grants (Yuhua Senior Activity Centre) and from public donations. MEMBERSHIPS The Centre is open to the public for participation in its charitable activities in helping the social needy. Classes, talks and enhancement lessons are also conducted for the public in pursuing of the objects of the Centre. RESERVE The Centre treats the balance of the unrestricted fund as reserve to ensure the sustainability of the programmes. The Committee will review the amount annually to decide the need to raise funds. CONFLICT OF INTEREST The policy on conflict of interest was set up for all committee members and staff of the Centre to abide. REVIEW OF FINANCIAL YEAR APRIL 2015 TO MARCH 2016 Staff Report 1. Coordinator $27, Staff Movement 1 staff movement.in FYE March Ms Siti Zubaidah bte M.Noor left on 3 rd Apr PROGRAMS / EVENTS / ACTIVITES REPORT Please refer to Appendix FINANCIAL REPORT REVIEW OF FINANCIAL STATE AND EXPLANATION OF MAJOR FINANCIAL TRANSACTIONS. 1. The Loving Heart Multi Service Centre recorded a surplus $1,428, in the year ended 31 March We wish to thank our donors, friends and supporters whom have made significant contributions to Loving Heart Multi Service Centre for specific projects. (Details are in the financial statements ending 31 st Mar 2016). 2. Loving Heart Multi Service Centre is also the custodian of funds raised for the setup of the new NKF Dialysis Centre in Year A grand total of $ after expenses, was raised for this project. A Memorandum of Understanding was signed between Loving Heart Multi Service Centre and NKF on 2 Feb

Loving Heart Multi Service Centre carried out a major revamp of the centre in December A total sum of $ was paid to Kingsville Pacific Pte Ltd and Ultimate Solutions Pte Ltd, being payment for renovation and designing works respectively, which carried out on the premises. 4. Care & Share is a national fund-raising and volunteerism movement led by Community Chest for the social service sector, in celebration of SG50. It aims to show care and concern for the needy and recognise the contributions made by voluntary welfare organisations (VWOs). Eligible donations raised by participating VWOs were matched dollar-for-dollar by the government. This matched amount will go towards building the capabilities and capacities of the social service sector and supporting social services to meet rising needs. Loving Heart Multi Service Centre raised $1, from Dec 2013 to Mar It would be used for the dialysis centre setup, training, renovations, assistive programs, medical escort services, technology refresh and other operational needs. EXPLANATION OF THE PURPOSES FOR WHICH THE CHARITY S ASSETS ARE HELD Loving Heart MSC : Property rented in Y2000 for the purpose of 210, Jurong East St 21, running a range of programmes and services #01-389, Singapore for the needy, elderly and jobless Yuhua Senior Activity Centre : Property rented in Y2001 for the purpose of 318A, Jurong East St 31, running a range of programmes and services #02-308, Singapore for the elderly in 318A and the rental blocks. FUTURE PLANS AND COMMITMENTS 1. Provide assistive services and ad-hoc activities relating to community bonding for the residents. 2. Enhanced skillsets of staffs at both centres. 4
5 STATEMENT OF ACCOUNTS Statement of accounts for the financial year ended 31 March 2016 attached. Services provided to the community are as follows; Our Services Meals Delivery (referral) Food Ration Job Match Befriender Home Help Information & Referral Community Mediation Medical Escort Service Silver Heart Wellness Program Family Life Education Programmes Yellow Ribbon Community Program Other Recreational Activities SOCIAL ASSISTANCE 1. Meals Delivery This service was referred to TOUCH Community Service on 1 st Nov Yuhua recipients were mostly referred to TOUCH, via through Yuhua MPS. 2. Food Ration LHMSC provided rations of groceries to our 37 needy families. These residents were mostly referred through Meet Your People sessions. Rations were collected through SWCDC and other donors. 3. Job Match The centre provides job opportunities for Yuhua residents. These jobs are mainly food-packer, cleaners, store assistants, admin, drivers, security guards etc. 5
6 LHMSC is the central hub for all job opportunities in Yuhua, Information on available job opportunities will be broadcast to various RCs, NCs and other partners in Yuhua. 4. Yuhua Befriender The role of Silver Heart (SH) Befriender and Yuhua Peer Support Group are to aid in the social, psycho-emotional and physical well-being of lonely seniors through community participation whilst upholding our belief that every older person is a valued member of society and deserves to age with grace and dignity.volunteers are ambulant and active seniors that provide us with a channel to outreach to more lonely seniors living within our community. 5. Home Help The above relates to services like housekeeping through assistance from students from nearby schools, polytechnics, universities and working adults. Installation of ramps for the needy families are also available. 6. Medical Escort Service (Loving Heart Wheels) Our medical escort service, otherwise known as Loving Heart Wheels, provide transportation for the frail and vulnerable person to their medical appointments in the hospital and clinic. Most of the referrals are through their medical social workers, whom are then managed by Agency of Integrated Care (AIC), a government health agency. Our area of coverage is within SWCDC boundaries e.g Jurong East, Jurong West, Taman Jurong, Bt Batok and Clementi. SOCIAL ENGAGEMENT 7. Information and Referral LHMSC has an intention to increase the awareness of Government health policies and guidelines. This is to ensure that the residents understand the various available health schemes. It is also our aim to network with nearby VWOs to understand their work and ensure that our residents are well informed of their activities. 8. Community Mediation LHMSC will assist to refer clients and residents to nearby Family Service Centres, on the following: family disputes (excluding family violence) disagreements with friends stallholder/owner squabbles and arguments between strangers 6

Family Life Education Programme LHMSC planned to work with various agencies and partners on organizing family life talks, caregiver workshops and health talks that cater to the community We are looking to tap on pool of resources from HPB, AWWA,SAMH, SATA, SIT, DTS and others. 10. Yellow Ribbon Community Programme Provide social support to incarcerated resident s family, through phone calls and house visits. Understand the social needs of the families and seek various partners in Yuhua to assist, accordingly. Follow up visits were to ensure the family continued lived harmoniously and self sufficient. WELLNESS 11 Silver Heart Wellness Program The above program provides an array of health services within LHMSC. Ceregem, Qi Gong, 312 Meridian exercise, 5 Style Exercise, mini gymnasium and rehab area. Programs for the elderly eg Zumba Gold, yoga, ukelele, karaoke, mind boosting games eg Rummy-O, were well received by the elders residents. LHMSC introduced La La Workout to Yuhua wheel-bound residents. 12 Other Recreational Activities LHMSC has also planned for outdoor activities to various places of interest eg Garden By The Bay, Sentosa Underwater World, for the residents. One day tours, community events were organized or participated together with various GROs, to bond our residents and our GRLs. Occasional gatherings eg weekly singing, were staged, to encourage a friendly engagement among the residents. END OF REPORT 7
