1 UMB GLOBAL HEALTH INTERPROFESSIONAL COUNCIL AND CENTER FOR GLOBAL EDUCATION NITIATIVES Interprofessional Global Health Grant Award Facilitated Student Placement Opportunities 2014 The following international opportunities are available to all UMB students interested in interprofessional global health. These opportunities are eligible for grant funding in amounts meant to help defray the cost of travel and lodging. Note some opportunities are in the form of an approved course and others are shorter experiences outside of the curriculum. Student application is at the end of this document. Note that the deadline to apply for these opportunities is March Course: Philippines: May 25 June 6, 2014 Course Overview This elective course helps students understand interprofessional practice in an international context by focusing on the experiences of citizens and immigrants from the Philippines. The course provides direct experience with a variety of professionals in practice settings and community development agencies in the Philippines. This interprofessional course is designed for graduate students interested in learning about other cultures and how different societies view and address social problems (i.e., health, education, economics, social justice and equity). The international field visit will include daily trips to meet with health and human service workers addressing issues related to children and families, and people organizing for economic improvement. Teaching Method The teaching model used for this class is observational/educational. The primary focus of this course is the intensive field visit to the Philippines. The course is combined with four classes before the trip (these are taped and available for viewing for those outside the school of social work who may not be able to make the class) and two written assignments and the journal recording of observations, impressions and reactions to daily field visits and interactions in the host country. This information will be processed during travels through daily journaling and group discussion. The class is heavily experiential and requires a high level of participation and involvement. Research: Malawi Malaria Prevention: June 2014 Malaria prevention in school children in rural Malawi. This project will focus on collecting baseline information about the health of school age children in rural Malawi (ages 6-16, but focusing mostly on year olds). We will use the information gathered to help to design an intervention designed to decrease malaria among school aged children and also hopefully target young women who might be pregnant. A medical student will work with a student either from the School of Social Work or Nursing to examine the following issues: 1. Who does or does not go to school and why? 2. What health problems do school aged children have and where do they seek care? 3. Do adolescents who are pregnant attend school? 4. How could an intervention to either treat or prevent malaria in school aged children be designed to reach the maximum number of children in this community? In Malawi, the students will do the following activities: 1. Conduct basic fact-finding interviews with community members and leaders 2. Conduct a survey among women delivering their first babies at the local health center to determine the last time they attended school 3. Participate in focus group discussions about the above issues. To prepare for this project, students will become familiar with adolescent health issues in developing countries, the burden of malaria in adolescents and also in pregnancy and qualitative research methods. They are expected to meet together (usually with me) every week in April and May. The project will occur in June 2014 with an expected duration of 4-6 weeks. Research: Malawi Rehabilitation Program: July 2014 This project involves follow-up home visits to clients from the Kachere Rehabilitation Program (Blantyre) and local community to record status days/months/years post client discharge, observe the home environment, interview the client, and determine pressing activity and participation/life needs. Instructions and recommendations would be shared with the health service assistants (HSAs) and rehab staff. A summary report enhanced with photos would document the visit, including recommendations for next steps. Mr. Chimatiro, the Physiotherapy Supervisor and his staff and students will not only debrief with the team but have pledged to move this project forward after the IPE team s initial investigation. The number of in-country visits and other details are in the formation stage, but would need to consider the location of the discharged clients, ground transportation, interpreters, and distance/time needed to access them. The local staff will develop the schedule, arrange transportation, help to develop interview and training materials, and brief the team prior to the home visit project, as well as debrief with them following the visits. We anticipate about home visits over about a two-week period in

3 Malawi. Total time in Malawi is approximately 4 weeks. The rehab program at Kachere is the only one in Malawi and there is no follow-up post-client discharge to determine implementation of instructions and status so this work will be ground-breaking. Project Development: Malawi HIV/AIDS Legal Clinic: Dates TBD HIV/AIDS Legal Clinic: Creating a Malawi/Maryland Collaboration In this two week project, a School of Law and School of Social Work faculty members will take three students to Malawi to meet with law faculty and local health care providers in Zomba, Malawi to facilitate an ongoing collaboration between the fledgling HIV/AIDS Legal Clinic at the Chancellor College Law Faculty and the HIV/AIDS Legal Clinic at UMB Law School. Last year, UMB faculty traveled to Malawi to initiate this process and this summer s trip is designed to make concrete plans for an ongoing collaboration. The collaboration may include students in both clinics working on the same projects and communicating electronically; providing advice and technical assistance to each other; and possibly creating the opportunity for an exchange in which students from both clinics travel to work and study in the other clinic. Part of this summer s project will include reaching out to health care providers in the vicinity of the law school to encourage them to send clients to the legal clinic if indicated. Research: Gambia: Flexible dates between May 21 July 8, 2014 For the first time, in summer 2014, UMB students have the opportunity to conduct research in the Gambia through a partnership with St. Mary s College of Maryland (SMCM), utilizing the infrastructure and staff support St. Mary s already has in place in the Gambia. Options for students include Interprofessional research projects identified by UMB and Gambian organizations open to hosting students. Details to be finalized based on students interests and the alignment with Gambian needs. Individual research or service projects. These are arranged independently with the UMB Student Center for Global Education and the St. Mary s staff in the Gambia. Interprofessional projects are eligible for the new UMB International Interprofessional grant, which provides some financial support for projects that involve students from more than one UMB school. Individual projects are self-funded. Interprofessional Projects Three possible projects are broadly identified for this year, and the goals and details of the research may be adjusted to suit the needs of the Gambians and the interests of the students involved.

4 1) Foundation for Research on Women s Health, Productivity, and the Environment (BAFROW) BAFROW is an organization dedicated to serving the needs of women in the Gambia. They operate a clinic, conduct research, and are deeply involved in public information and advocacy campaigns. Students interested in women s health issues in a developing country will find opportunities to get involved in one of several potential areas of research or service, including research on fistulas, female genital mutilation (FGM), and general medical public information campaigns related to nutrition. Details of the interprofessional projects will be developed once the students are selected. 2) Gambian Department of Social Welfare and Child Protection Alliance Gambia s Department of Social Welfare offers opportunities for students to work and research. The department provides a one-week introduction to policies and procedures, and a tour of the different units of the department (childcare, child protection, custody, adoption, abuse; adult care, mental health, family support services; elderly care; disability care). The department also runs a center for children as a temporary residence, for children working or living on the streets and a one-stop public-private center to address violence against women. The Child Protection Alliance is a Gambian coalition of 36 member NGOs, established in 2001, to educate and advocate for children s rights. Their work is primarily coordination of advocacy and the members work with religious leaders, parents, medical personnel (teaching how to recognize signs of abuse) and others. In 2011, their work resulted in the first Gambian law to protect children s rights. Research by member organizations includes studies on child abuse, linkages between HIV/AIDS and commercial exploitation of children; sexual harassment; and corporal punishment in schools. The Alliance offers a program Voice of the Young in which children meet twice a week to do capacity building, learning about their rights. A program or project involving the social welfare department and the child protection alliance will be customized based on the interests of the students involved. 3) University of the Gambia (UTG) Medical School, UTG School of Nursing, the Royal Victoria Teaching Hospital, and the Medical Research Center (MRC). The University of the Gambia Medical School is located at the Royal Victoria Teaching Hospital, and offers programs in Allied Health Science, nursing, master s in public health, physical therapy, radiography, among others). The global health program is strong, and students will witness firsthand, the hospital operations that rely heavily on the services of Cuban and Syrian doctors, overseen by a Gambia Chief Medical Director. All heads of departments at the hospital are teaching staff so the integration of teaching and service delivery is excellent. The Medical Research Center is the only such entity in the Gambia, and students are welcome to participate in one of several ongoing research projects. In addition, the following projects may be arranged if students are interested: 1. Mapping projects: a. Map emergency medicine (or ob/gyn, or pharmacological, or..) resources and assess strengths and opportunities, with recommendations. b. Map a specific disease prevalence and analyze reasons for the findings.

5 c. Map medical/pharmacy/dental education in the country and analyze strengths and opportunities. 2. Social and cultural determinants of health a. Explore disease prevalence with a specific focus on social and cultural influencing factors b. Legal underpinnings that affect population health 3. Health Care coverage a. Does Gambia have universal healthcare? b. What works; what doesn t? How could the system be improved? 4. Maternal mortality: just a medical problem? 5. HIV/AIDS policy: national policy, local interpretations, and prevention programs. Students interested in an interprofessional opportunity should propose a topic area and Gambian professionals will be contacted to assess the potential for developing such a project for the May to July timeframe. Program Evaluation: Human Resources for Health (HRH) Rwanda: Summer 2014 (TBD) Background: The HRH Program in Rwanda aims to build a sustainable, high-quality health workforce, capable of providing the citizens of Rwanda with world-class healthcare (CHAI, 2012). The program is a novel multimillion-dollar approach, expected to last 8 years, and by far one of the largest endeavors undertaken by a single country. Given the enormous investment, there is great need to strategically examine the program s implementation. The University of Maryland School of Nursing currently has 11 faculty and School of Dentistry and School of Medicine (OB/GYN) have one faculty each on the ground with the HRH Rwanda program. U.S. faculty members are twinned with Rwandan counterparts to improve education and clinical practice toward raising the quality of dentists, doctors, nurses, health managers. The overall goal of this global health project is to tell the story and share lessons from Rwandan HRH program for current quality improvement and for future programs in other countries to learn from successes and challenges with the HRH Program. Methods: A quality improvement program evaluation will be conducted over 4 weeks in country. The evaluation will employ interviews, focus groups, and quantitative surveys. Primary participants will be UMB faculty in Rwanda, Rwandan counterparts, and key Rwandan stakeholders, and potentially other faculty from other U.S. academic institutions. U.S. students and faculty may partner with Rwandan students and faculty from respective institutions to gather, transcribe, and analyze data. Collaborative activities will begin in Rwanda and may require some post-trip involvement and follow-up meetings. UMB students will be mentored and supported by UMB faculty and will be required to participate in several pre-trip meetings. While this is an unpaid field opportunity, any academic credit for this global health field experience will be dependent on the student and his/her individual school. Outcomes: Students and faculty will gain insight into cross-cultural context of implementing health education programs as well as gain exposure to realities of health and health disciplines in Rwanda. Information from this program evaluation will help to improve the quality of UMB s

6 efforts in HRH Program and contribute knowledge toward quality improvement for the overall HRH program. An inter-professional team from various specialties is ideal for this evaluation. Nursing, dentistry and OB/GYN are the most obvious partners. However, students from other disciplines interested in investigating global education initiatives and furthering implementation science as an approach to global health program execution are strongly encouraged to apply. Student Qualifications: Must be part- or full-time students in good standing at UMB. Proficient health background (clinical, public health, education, advocacy) experience. Potential to exercise mature interpersonal skills, flexibility, patience, discretion, judgment/discipline, manage own work, and commitment to working within a teamoriented, multi-cultural environment. Potential to sustain continuous and productive work in a resource limited setting, i.e. given intermittent access to electricity and long days, must interact and communicate appropriately with internal and external parties in a professional and efficient manner. Excellent writing skills. French or Kinyarwanda competency is an asset. Pilot Study: Examining Palliative Care in China: Summer 2014 (TBD) Overview The Office of Global Health (OGH) at the University of Maryland School of Nursing is deeply committed to serving local and global communities through the lens of social justice. The vision is to build nursing and healthcare worker capacity worldwide, strengthen health systems, and improve global health. As part of an emerging partnership with the University of Hong Kong School of Nursing, OGH has an exciting inter-professional opportunity for 2-3 UMB students to provide targeted field assistance and implement a pilot research project in China focusing on palliative care. Project Summary: Students will be accompanied by UMB faculty from the School of Nursing to collaborate with Chinese faculty to conduct a survey of university-based faculty and healthcare practitioners examining existing perceptions of palliative care and current practices related to end-of-life care. This pilot study will include an orientation to the healthcare system in Hong Kong (and potentially a nearby province) and will involve survey data collection and focus group discussions. Observational information may also be gathered during clinical and community health site visits. The survey team will utilize global health knowledge and inter-professional collaboration skills to plan and coordinate the execution of this pilot survey. The students will be mentored and supported by UMB faculty from the School of Nursing and will be required to participate in several pre-trip meetings. While this is an unpaid field opportunity, any academic credit for this global health field experience will be dependent on the student and his/her individual school.
