1 MEMBER POSITION/TITLE HEALTH EDUCATOR AMERICORPS PROGRAM Program: National Health Corps Location: Chicago HOST SITE NAME & LOCATION Erie Family Health Center Erie Lake View Health Center Lake View High School 4015 N. Ashland Ave. Chicago, IL ORGANIZATION DESCRIPTION & MISSION At Erie Family Health Center we believe health care is a right, not a privilege. Our mission is to provide accessible, affordable and high quality health care for those in need. Erie Family Health Center delivers high-quality, culturally-sensitive, bilingual health care to more than 70,000 medical patients and 12,000 dental patients annually. Our community-based health care centers are portals to high-quality care and a higher quality of life regardless of a patient s ability to pay. Today, Erie serves patients from 13 sites, including seven large primary care facilities (four with integrated, on-site dental care), five school-based health centers and the only freestanding comprehensive teen health provider in Chicagoland. Erie Lake View Health Center is a school-based health center that serves students attending Lake View High School. MEMBER POSITION PURPOSE The Health Educator at Lake View High school will provide students with vital information regarding their own health. Furthermore, through the education and outreach efforts of the member, students will also learn about the health care facilities that Erie operates and their ability to access the centers without experiencing barriers. Often, the NHC member is a student s original point of contact with Erie. MEMBER TERM OF SERVICE This is a full-time AmeriCorps national direct service position. To fulfill this position, the member will: complete a minimum of 1,700 hours of service during this period. A maximum of 20% of these hours may be in training and a maximum 10% of these hours may be for pre-approved fundraising activities.

SITE CONSIDERATIONS Is the site accessible via public transportation (if yes, what line/route)?: Yes; Irving Park Brown Line station and Irving Park Rd or Ashland Ave buses Does this position require a personal vehicle? No How will your organization reimburse the member for transportation costs? Erie will reimburse mileage or cost of CTA transportation. Erie reimburses NHC Chicago members for their CTA passes. Members purchase passes and submit their receipt to the Department Director for reimbursement. Organization dress code: Business Casual Jeans may be work on Fridays EXPECTED SERVICE TIME REQUIREMENTS/SCHEDULE Monday Friday, 8:00 am 4:30 pm Erie Family Health Center will respect the National Health Corps set calendar. During the holidays, NHC members are able to take time off as long as they have their work finished or get coverage for their work, if needed, are in good standing with their hours and/or have a plan to make up those hours. There are opportunities for service on the weekends, assisting with insurance enrollment or health fairs. Host site supervisor and NHC member will work to figure out a plan to make up the hours. MEMBER ROLE/DESCRIPTION OF DUTIES: Erie Family Health Center s school-based sites rely heavily on NHC members to provide health education for students. Through these outreach efforts, students not only receive vital information regarding their own health, but they also learn about the health care facilities that Erie operates and their ability to access the centers without experiencing boundaries. The member s position as a health educator will allow him/her to work closely with the students to assist with one on one nutrition counseling, coordinate dental and vision vans, teach comprehensive sexual health education in the classrooms, coordinate lunchroom health promotion events monthly, as well as represent Erie at health fairs around the city of Chicago. 3 The activities of the Erie NHC members align closely with many National Health Corps performance measures. Specifically, the health education activities (Teen Pregnancy Prevention, 1:1 Nutrition Counseling, and Adolescent Health Campaigns) fall within the following parameters of the NHC s performance measures: Health Education, Nutrition and Physical Education activities. Through these classes, students are introduced to Erie Clinics where they can find medical providers that provide health screenings and testing to meet any health care needs the student may have. In addition, students learn from the NHC health educators that they are able to access Navigators (for health insurance questions and enrollment) at any Erie site. Each student s original point of contact with Erie is often the NHC member. The member position activities follows the requirements related to supplementation, duplication or displacement of staff as outlined in 45 CFR (e) (f). No other employee, volunteer or intern at the site has the same tasks as the member. The members in this position will not be conducting AmeriCorps prohibited activities as outlined in 45 CFR To ensure this, the host-site supervisor will monitor the member s activities. The members in this position will receive no more training than 20 percent of the aggregate of the total member service hours as outlined in requirements of 45 CFR The member position description activities do not put members at risk for exceeding the limitations on allowable fundraising activity as outlined in requirements of 45 CFR PROGRAM OR PROJECT NAME Nutrition/ Physical Activity Education (10%) MEMBER ACTIVITIES MEMBER OUTPUTS NHC PERFORMANCE MEASURE(S) THIS ACTIVITY FALLS UNDER (if any). Coordinate Get Real 1:1 nutrition counseling meeting with Lake View students: Complete child and adolescent nutrition education training conducted by Erie staff and CLOCC. Attend and complete Cooking Matters training. Complete Motivational Interviewing training (if available). Coordinate, schedule, and facilitate Get REAL: 1:1 nutrition counseling meetings with Lake View students referred or identified by providers, as well as those interested from flyers placed in the high school. During meetings, members will develop and set selfmanagement goals with patients who have been identified as overweight/obese. Member will initiate 1:1 nutrition counseling for 20 students annually. 4 Identify additional nutrition education opportunities at Lake View and implement: Research possible nutrition education programs to implement at Lake View, including, but not limited to Cooking Matters. Build relationships with school staff to implement additional nutrition education programs. Member will identify and implement 1 new nutrition education program at Lake View. This may include cooking demonstrations, exercise classes, and/or group workshops. Health Education Teen Pregnancy Prevention Program- (25%) Vision Care (15%) Implement Chicago Public School curriculum, along with additional evidenced based education with other AmeriCorps members to students in schools and community organizations: Participate in TPP Program training through CPS. Complete teach backs to ensure understanding of curriculum. Schedule TPP classes with teachers and school administration. Develop content and prepare for TPP classes. Partner with other NHC Chicago Erie members to teach TPP classes in classrooms at Ward, Johnson, Amundsen, Clemente, Lake View High Schools and other schools and community organizations. Implement pre/posttest evaluation of curriculum. Document teaching in centralized database. Partner with gender/sexuality after school club to discuss sexual health at meetings. Arrange for Vision Specialists to see students at school: Work with the staff of CPS Vision Care Specialists to arrange for students at school to be seen. When a bi-annual schedule is agreed upon with CPS Vision Care Specialists, arrange for the facilities at In conjunction with other NHC members, member will teach approximately 20 classes of (30) students annually across all sites. The Vision Specialists can accommodate 25 vision visits per Optometrist. The member will arrange for the Vision Specialists to visit the school twice during the year. 5 Dental Care (15%) school to accommodate these services. Advertise service offerings through school intercom announcements and flyers. Engage patients at the clinic to determine if they are in need of dental services. Schedule patients for the service offering. On the day the service is offered, manage the operations of the day to ensure patients are seen and providers have what they need. Distribute glasses to students following the visit and after prescriptions for glasses have been filled. Arrange for the Advocate Dental Van to see students at Lake View: Work with the staff of the Advocate Dental Van to arrange for students at Lake View to be seen. Once a monthly schedule is determined with Advocate arrange for the facilities at Lake View to accommodate these services. Advertise service offerings through school intercom announcements and flyers. Engage patients that come to the clinic to determine if they are in need of dental services. Schedule patients for the service offering. On the day the service is offered, manage the operations of the day to ensure patients are seen and providers have what they need. A minimum of 12 students and a maximum of 15 students are scheduled monthly for the dental van. Health Care Service Enrollment and Scheduling Primary Health Care Service Use Page 5 of 12

6 Adolescent Health Campaigns- (10%) Develop monthly adolescent health topics to address on health center bulletin boards: Select topic for bulletin board in collaboration with Health Center team. Design and create bulletin boards in health center. Bulletin board should be planned and changed monthly. While a valuable activity that achieves the NHC objective of providing health education, the member will not report any performance measures for this activity because it does not involve direct interaction with the students. Host every other month promotions in the lunchroom: Plan activities and prepare materials. Host lunchroom promotional activities at a table in the lunch room every other month at assigned schools Lunchroom promotions should be scheduled every other month or more frequently if approved by administrators. Health Education Deliver Information about Health Insurance, Health Care Access and Health Benefits Programs Provision of Health Center services and activities associated with Affordable Care Act- (10%) Promote the services of Erie Family Health Center city wide at Health Fair and other community-oriented events and serve as a navigator for health insurance enrollment for the agency and their site(s): Participate and complete ACA State and Federal trainings. Screen and process patient information and perform navigation duties with patients to obtain health insurance. Participate in health fair/outreach events coordinated by the Patient and Support Services Coordinator. Members will participate in at least 6 agency-wide health fairs and events during their service term. The official open enrollment period will be determined. 7 Patient Portal Enrollment - (10%) Teen Mental Health (5%) NHC Member Position Description Enroll patients in Erie s Patient Portal, an online system designed to enable patients to communicate with their care team: Attend training on Erie s Patient Portal training. Work with patients in health center waiting room to enroll patients in portal system. Teach mental health topics to Lake View students: Collaborate with school psychologist to teach classes on mindfulness. Member will enroll 30 patients per year for the patient portal. Member will teach 1-3 mental health topics a week. Deliver Information about Health Insurance, Health Care Access and Health Benefits Programs Health Care Service Enrollment and Scheduling Health Education Teach about relevant topics at teen mental health after school club. SITE ORIENTATION AND TRAINING Erie has over 10 years of experience hosting AmeriCorps members. During this time we have created formal and informal processes for onsite supervision and ongoing mentorship. The member will attend Erie s orientation prior to or 1-2 weeks after the scheduled start date. Erie orientation is coordinated through Erie s Human Resources department and gives new employees, volunteers, and AmeriCorps members an overview of the health center and its policies and procedures. The member will participate in Erie s Bus Tour Day. AmeriCorps Members visit all Erie sites and visit with program staff to learn more about Erie resources. The member will meet with their direct supervisor (Lauren Fifarek, Project Coordinator) on their start date to receive a tour of their clinic, be introduced to staff, be oriented to member s space, review work schedule, and have any questions answered. Member will be provided Erie s Standard of Conduct and Dress Policy prior to start date. Member will be provided with an Outreach Volunteer Binder (created by previous member) on first day. This will orient member to position and key roles. The member and their direct supervisor will meet on a weekly basis for a one-hour formal meeting to review member work plan, goals, and next steps. Frequency and length of meeting will be determined by member and supervisor on a weekly basis. The member will be scheduled for Electronic Medical Record training within the first 2 weeks of service, including learning about how to sign up patients in the Patient Portal. The member will meet with key personnel within the first 3 weeks of service to learn more about Erie s various resources and services. Page 7 of 12

8 The member will attend a CPS Sexual Health Education training within the first 2 weeks of service, in preparation for their TPP Sexual Health Education responsibility. The member will attend a CPS 101 and Classroom Management training within the first 2 weeks of service given by Erie staff. The member will attend a Nutrition Education training within the first 2 weeks of service, given by Erie staff, in preparation for their Get REAL 1:1 nutrition counseling sessions and nutrition classes they will teach. The member will attend a CareMessage training within the first month of service to learn how to use the texting website application we use to send text reminders to patients for appointments and classes. The member will attend a Cooking Matters training within the first month of service. The member will complete an online ACA State and Federal training in order to become a certified application counselor for open enrollment. This will be completed during the first month of service. The member will talk to behavioral health specialist in the clinic for more information about teen mental health and assistance with coordinating and creating presentations for students. The member will attend 2 professional development trainings planned by host-site supervisor, one in the winter and one in the spring (potentially one more in the fall, TBD). MEMBER BENEFITS The member in this position will receive from the NHC program the following benefits: 1. Living Allowance in the amount of $12,630.00: a. The living allowance is t taxable, and taxes will be deducted directly from the living allowance. b. The living allowance is not a wage. It is intended to provide for expenses a member incurs while actively serving and is not linked to the number of hours a member serves. A member who completes his/her term of service early or will receive the portion of the living allowance that would be provided for that period of participation under the program s living allowance distribution policy (a member who leaves in the first week (or first ½) of a pay period receives ½ a stipend; a member who leaves early in the second week (or second ½) of a pay period receives a full stipend). Members who end their service early will not be eligible for the remaining amount of their living allowance, either in lump sum or incremental payments. A member may not receive a living allowance if they are suspended by the program. 2. Health Insurance. If a full-time member is not currently covered by a health insurance program or loses coverage due to participation in the Program, they are eligible to receive limited health insurance through the program where they serve. Insurance coverage for fulltime members begins after mandatory documentation is received and processed. Member insurance coverage does not cover family members and dependents. Page 8 of 12

9 3. Child Care: Child care subsidy, paid directly to the child care provider by a CNCS benefits administrator, GAP Solutions, is available to members who qualify. GAP Solutions distributes this allowance evenly over the term of service on a monthly basis. Members are responsible for locating their own child care providers. The amount of the child care subsidy that the member may be eligible for varies by state and may not cover the full cost of child care. 4. Education Award. Upon successful completion of the member s term of service, the member may be eligible to receive an education award from the National Service Trust. For successful completion of a full-time term (minimum of 1,700 hours), the member will receive a $5,815 Education Award. The member understands that he/she may not receive more than the amount equal to the total value of two education awards for full-time service from the National Service Trust, regardless of the stream of service in which the member serves. 5. Loan Forbearance Interest: If the member has received forbearance on a qualified student loan during the term of service, and the member successfully completes the term of service, the National Service Trust will repay a portion or all of the interest that accrued on the loan during the term of service. After a member is enrolled in e-grants by the Program Director, they may use the CNCS web-based system to apply for loan forbearance. The NHC is not responsible for following through with private lenders. MINIMUM NHC MEMBER QUALIFICATIONS In order to be eligible to serve in this position and in the NHC Program, a person must meet the following requirements: 1. Must be at least 18 years of age by the time training begins; 2. Must be a United States citizen or National or have a permanent resident visa; 3. Must have a high school diploma or an equivalency certificate (or agree to obtain a high school diploma or its equivalent before using an education award) and must not have dropped out of elementary or secondary school in order to enroll as an AmeriCorps member (unless enrolled in an institution of higher education on an ability to benefit basis and is considered eligible for funds under section 484 of the Higher Education Act of 1965, 20 U.S.C. 1091), or who has been determined through an independent assessment conducted by the Program to be incapable of obtaining a high school diploma or its equivalent; 4. Must have complied with all CNCS required criminal history checks including 1) a State Criminal Registry Check of the CNCS designated repository in both the state in which the program operates and the state in which the member resided at the time they applied to the program; 2) a nationwide fingerprint based FBI background check; and 3) a Department of Justice National Sex Offender Registry Check. The member understands that if the results of the required criminal history checks reveal that they are subject to a state sex offender registration requirement and/or have been convicted of homicide (1 st, 2 nd or 3 rd degree) they will be ineligible to serve in the Program. The member also understands they will have the opportunity to review and dispute the findings from the criminal history check. Page 9 of 12

10 5. Must submit valid forms of documentation to prove date of birth and citizenship/naturalization/resident alien status and must have a valid government issued photo identification; 6. Must disclose any history of having been released from another AmeriCorps program; failure to do so will render one ineligible to receive the education award; 7. Must submit evidence that they successfully completed any previous AmeriCorps terms, if applicable; 8. Must furnish all other documentation deemed appropriate by the program and host service site. 11 Ability to engage teachers and administrative staff to organize health education lessons and health care services Must have a desire to work with children and adolescents Organizational skills to keep track of attendance, pre and post test data information Ability to work effectively with Erie/NHC member team and colleagues EVALUTION AND REPORTING All NHC member will be given written performance review by both their host site supervisor and NHC Program Director at the mid-term and end of their term of serve. These performance reviews will be, based on the member s performance at their host site and their participation in National Health Corps responsibilities such as member trainings, committees and group service projects. If a member disagrees with any aspect of their performance review, they can indicate that on the review and they may appeal to the Program Director in writing, according to the grievance procedure. NHC members will be evaluated according to the following criteria: a. Whether the member has satisfactorily completed service assignments, tasks or projects; b. Whether the participant has met any other performance criteria which has been clearly communicated both orally and in writing at the beginning of the term of service; c. The member s ability to establish and maintain positive interpersonal relationships and whether they participated in NHC activities; d. Whether the participant has completed or is on track to complete the required number of hours outlined in their member contract to complete their term of service.
