3 How we deliver your care It is expected that all the people involved in your care, whatever the setting and whether or not your care is delivered using the Care Programme Approach (CPA), do so within set values and principles that: Ensure that we always help you to feel valued, show respect for you as a person and demonstrate equality of opportunities for everyone, regardless of ethnic origin, gender, gender identity, sexual orientation, age, disability, cultural or spiritual beliefs, recognising your personal strengths and qualities Work to promote your recovery and to support your ideas, ambitions and life plans Promote and support your independence and offer you reasonable choices regarding your care and support Recognise and where possible work with your family and friends (with your permission) who support you towards your recovery, making sure that their own needs are recognised and supported Recognise that we may need to work with you for some time and that your care needs may be complex Always involve you in decisions about your care and treatment. 3

4 The Care Programme Approach What is CPA? CPA is simply a term used for describing the more structured process of how mental health services deliver care. Your care may be managed using CPA if: You have more complex needs which could require more support and guidance You need a range of services and/or agencies to support you. Under CPA you will have a named Care Co-ordinator, who will keep in touch with you and all the other people involved in your care. They will be responsible for ensuring that your care plan is delivered and for making sure that it is reviewed when appropriate, or at least every six months, to make sure it is working. If you have lower levels of need and are more able to manage your own care with support and guidance, the same principles will still apply. In these situations your care will not be known as CPA but as Standard Care, and the person overseeing your care will be known as the Lead Practitioner. Whether or not your care is delivered using CPA, we will fully assess your needs, plan ways to meet them and check that they are being met. We will help you have as much control of the process as possible. 4 Your care and how it is delivered to you, with you

5 This process aims to: Fully assess your needs with you, in relation to any given situation Help you develop your skills and strengths to support you in your day to day activities and work towards your recovery Help identify with you, any risks to the safety of you or others Develop a plan with you, in response to the needs identified, recognising the strengths and skills you already have and the formats you may require future documents in Share responsibility with you (and others as needed), to put the plan into action Review the plan with you at least every six months, to see that it is meeting your needs and to agree any changes that might need to be made. What happens in an assessment? In an assessment, one or more mental health service workers will talk to you about your needs. This means talking to you about thoughts and feelings that might be troubling you and also other parts of your life where you may be experiencing problems, such as housing, relationships or employment. This information will be provided into a document, so that any needs, strengths and risks are clearly identified. We will then look at how your needs can be supported. At the end of the assessment process we will decide whether your care needs would be better met using the Care Programme Approach (CPA). 5

6 The Care Programme Approach Who will be the Care Co-ordinator? Usually a nurse, social worker or occupational therapist, although it may be someone else, with your agreement Ideally, they should be the person who knows you best and with whom you feel most comfortable to talk with You should always be informed of the name and contact details of your Care Co-ordinator They will work closely with you, listening to your opinions and wishes, and answer questions you might have. They will stay in touch with you and make sure you get the support you want. What is a Care Plan? A Care Plan is a way of recording agreements about the help and support you need. It explains what you are hoping to achieve, how this will be done and who is responsible. It may be set within a formal Care Plan document if you are on CPA, or in the appropriate format you require if you are receiving Standard Care. 6 Your care and how it is delivered to you, with you

7 Whatever the format, your Care Plan should: Be produced with your involvement, wherever possible, and recognise what you are able to and also what you want to do Aim to improve your mental health and social situation Provide you with the contact details of your named person, together with details of anyone else involved in your care Provide information for people who support you Detail any areas you disagree with. For those on CPA it will also include: A crisis or coping plan to help support you when things aren t going so well A contingency plan to detail what we will do, if for some reason, part of the care plan cannot be delivered The date of the next planned review. Once your Care Plan has been completed both you and your carer (if applicable) will be given an opportunity to sign it, alongside your Care Co-ordinator or Lead Practitioner. Everyone involved in the care plan will be offered a copy including you, your carer (if applicable) and your GP. If you have developed an advance statement i.e. a WRAP (Wellness Recovery Action Plan) or similar, you may wish to include some of this in your Care Plan. Your Care Co-ordinator or Lead Practitioner will be able to help you do this. 7

8 The Care Programme Approach What is a Review? No matter how good your Care Plan is, it will need to be reviewed periodically to make sure the help and support you are receiving remains right for you. When a Care Plan is reviewed will depend on your needs and current situation. A review may be called before the previously agreed date, if your needs or circumstances have changed. A Review: Should be convenient for you and flexible about where and when it happens, and who attends Can just be you and your Care Co-ordinator having a discussion, if this is what you prefer Can be called when required, by anyone involved in your Care Plan, including you and your carer Should recognise any progress that has been made or any difficulties in implementing the Care Plan Should take place at least every six months.
