If your PCP is on this list, you may stay with him/her. PCPs with an by their name are open to new patients. Clinics work to help patients better their health with innovative care. They are known for better office access, and organization of care. Clinics with PCPCH next to their name are PCPCH certified. 5 Selecting a doctor You must choose a doctor or nurse practitioner as your Primary Care Provider (PCP) before you may receive services from AllCare CCO. Each person in the family must have a PCP. Each person in the family may have a different PCP. There are three kinds of doctors and nurse practitioners who may serve as your PCP: Family Medicine treats children and adults. Internal Medicine treats only adults. Pediatrician treats only children. Your PCP (doctor or nurse practitioner) will: Get to know you and your medical history; Provide all your medical care; Keep your medical records up-to-date and in one place; Send you to a specialist, if needed; and Admit you to a hospital, if needed. If you already have a PCP (doctor or nurse practitioner), look in this directory to see if he/she is listed. If your PCP is not in our directory, you must choose one who is. Choose a PCP (doctor or nurse practitioner) who is easy to get to one who is close to where you live or work. After you have made your choice, please: Fill out the pink Primary Care Provider Selection Form in the new member packet. Fold and mail the pre-stamped form. A new Medical Identification card, with your doctor or nurse practitioner s name and phone number printed on it, will be sent to you. OHP requires you to select a PCP within 30 days. If you have not returned your selection form within two weeks, we will assign a PCP for you. Meeting your new doctor After you get your AllCare CCO Identification Card, please have a physical exam with your new doctor or nurse practitioner within 90 days. (800) ReadyRide provides rides to and from AllCare CCO-covered appointments when no other ride is available. AllCare CCO members can get rides to covered medical, dental, therapy, counseling, and behavioral health appointments. Please call at least 48 hours before you need your ride. Rides for the same day are only scheduled for urgent needs and hospital discharges. Call between 8 a.m. and 6 p.m., Monday through Friday. Follow the prompts to schedule an urgent ride after-hours. When You Are Sick Please call your Primary Care Provider (PCP) when you become sick. Tell the receptionist or nurse what is wrong so they can determine whether or not you need a same-day appointment. Take your AllCare CCO Identification Card and your DMAP Medical Care card to your appointment. If you become sick at night, on weekends or holidays, call your PCP. Your PCP provides medical care 24 hours a day, seven days a week. If your PCP is not working that day, the answering service will refer you to another doctor on-call to answer your PCP s calls. If Your Illness Is Urgent An urgent medical problem is one that needs attention soon to prevent harm to your health, but is not an emergency. If you are sick, call your PCP. Your PCP, or the doctor on-call, will either: schedule an appointment with you, give you medical advice over the phone, or send you to an Urgent Care facility or a hospital. 13 provided by your PCP. Do not go to an Urgent Care facility for routine care that could be provided by your PCP, unless your PCP sends you. What To Do In An Emergency If you believe you have an emergency medical condition, call or go directly to the emergency room. An emergency medical condition means you have symptoms that are severe. (Examples of emergencies include, but are not limited to: unusual or excessive bleeding, acute abdominal or chest pain, suspected heart attacks, serious injuries or burns, poisoning, unconsciousness, convulsions, difficulty in breathing, or similarly acute conditions.) You must believe your health will be in serious danger if you do not get help right away. This includes your unborn child s health if you are pregnant. If you are not sure your condition is an emergency, call your Primary Care Provider. Your PCP, or the on-call doctor, will instruct you what to do. Do not go to a hospital emergency room for care that should take place in your PCP s office.
