1 2017 BENEFIT ENROLLMENT
2 2017 Medical Plans. All medical plans will be on the Wichita Preferred Quality Point of service (QPOS) plans in the Wesley preferred narrow network. Employees will be required to pick a Primary Care Physician (PCP) during online open enrollment for themselves and each family member enrolled in the plan. Members will be required to obtain a referral from their PCP to seek care from an in-network specialist. Members can seek care from out-of-network providers but will be required to pay more out of pocket for those services. Wesley remains the preferred hospital for all medical plans
3 2017 Urgent / Immediate Care Centers Via Christi Immediate Care (Former Wichita Clinic on Murdock) will NOT be in network
4 Primary Care Physician (PCP) Must be one of the following physician types: Cannot be one of the following: Family Practice Pediatrician Internal Medicine General Practitioner Specialist Physician Assistant (PA) Advanced Registered Nurse Practitioner (ARNP) These provider types are not excluded from the plan, they just cannot be selected as a PCP.
5 Designating a PCP Families do not have to all have the same PCP. Employees will be asked to designate a PCP for each dependent they enroll into the plan. During Open Enrollment a list of in-network PCPs will be available within the online enrollment system and employees will be required to designate a PCP for themselves and each of their dependents. If an in-network PCP is not specified by the employee during open enrollment, Aetna will assign a PCP to them. Employees who are auto-assigned a provider, will receive a letter from Aetna after open enrollment notifying them of their designated PCP. 6 USD 259 Nurse Navigator Our Nurse Navigator (Mary Singleton) will be available to offer our employees one on one assistance in navigating the new Aetna Wichita Preferred Quality Point of Service (QPOS) plans. Employees can receive assistance finding a new in-network PCP or with navigating the transition of care process. DO NOT HIT CONTINUE. You must select the plan first. Choose 2017 Wichita Preferred QPOS.
7 Changing PCP Members can change their PCP anytime and as many times as they would like. PCP changes will take affect the 1st day of the following month. Members can speak to an Aetna representative to change their PCP by calling customer service line for Wichita Public Schools at Members can also change their PCP by logging into the Aetna Navigator (online personal portal). Tour Aetna Navigator Employees will receive a new ID card each time they (or their dependent) selects a new PCP. Employees will receive separate cards from Maxor Pharmacy and Delta Dental (if enrolled in Dental Plan)
8 Referrals To Specialists Effective January 1, 2017, employees (or dependents) needing care from a specialist, will need to obtain a referral to an in-network specialist, from the designated in-network primary care physician (PCP) that is listed on the employee s new Aetna card. If an employee (or dependent) is currently receiving treatment with an in-network specialist, they will need to request a referral from their designated in-network PCP to the specialist so they can continue treatment with that provider. Contact Nurse Navigator by phone at (316) or doctorfindhelp@usd259.net If an employee (or dependent) is currently treating with a specialist, who will no longer be in the network in 2017, the employee will need to complete a Transition of Care form to request continuation of this care for a temporary period of time. Our Nurse Navigator is available to assist employees with the Transition of Care process
9 Referrals Employees (and dependents) will need to obtain referrals from their designated Primary Care Physician (PCP) to receive all services except those listed. Behavioral Health Durable Medical Equipment Gynecology care (routine, non routine, pregnancy) Home Health Care Hospital Ambulatory Surgery Hospital Outpatient Services Midlevel practitioners (Physician Assistant, Nurse Practitioner at the PCP s office, or at the specialist offices (when a referral is on file for you to see that specialist) Preventative Immunizations when given at the PCP s office Radiology, Pathology, Lab Routine Vision Care Physical, Occupational, Speech Therapy Urgent Care Facility Walk-In Clinic (Minute Clinic, etc.) Nutritional Counseling Termination of Pregnancy (Involuntary) You may still need to have a prescription or precertification for services whether or not they require a referral.

11 Out of Network Care Higher deductible Employees do have an option to seek care outside the network. Higher coinsurance percentage and higher out of pocket maximum. Employee can be billed for the difference between the billed charges and the allowed charges in addition to their deductible and coinsurance. Even if employee reaches the maximum out of pocket limit and begins to receive 100% benefit, they can still be responsible for the difference between billed charges and the allowed amount. Employees may still be required to obtain precertification or review for certain services. The employee will have higher out of pocket expenses. Employees may be required to pay the bill and submit the claim to be considered under your plan of benefits.
12 Transition of Care Employees can request in-network coverage for a limited period of a time under certain unique circumstances. Employees can complete a Transition of Care Coverage request form and submit to Aetna for the following: To receive care from an out of network doctor To continue care temporarily with a doctor whose network status has changed (moved from in-network to out of network) To continue care temporarily with other health care providers who have treated you When unable to obtain a referral from in-network PCP to see a specialist Contact Nurse Navigator by phone at (316) or doctorfindhelp@usd259.net Our Nurse Navigator is available to assist employees with the Transition of Care process
