1 Community Health Implementation Strategy FY Continuing Care Hospital

2 Introduction The Continuing Care Hospital (CCH) is licensed as a general acute care hospital, and addresses the unique and complicated physical, emotional, spiritual, nursing, and rehabilitation needs of the long-term acute care patient. There is no limitation regarding diagnosis type, patient age, length-of-stay and scope of services provided. Continuing Care Hospital is part of KentuckyOne Health, the largest health system in Kentucky with more than 200 locations including hospitals, outpatient facilities and physician offices and more than 3,100 licensed beds. An 18-member volunteer board of directors governs KentuckyOne Health, its facilities and operations, including Continuing Care Hospital, with this mission: Our Purpose To bring wellness, healing and hope to all, including the underserved. 3 Continuing Care Hospital is located in Lexington. Lexington (officially Lexington-Fayette Urban County) is a major city the second largest in the state of Kentucky. As of 2011, the Lexington-Fayette metro area had a population of 301,569, ranking it 63rd in size nationally. The Lexington-Fayette metro area is home to six counties: Clark, Fayette, Jessamine, Bourbon, Woodford and Scott. Lexington- Fayette metro is located in the center of the inner Bluegrass Region. This document provides a summary of Continuing Care Hospital s plan to develop new and enhance established community benefit programs and services from FY This plan is focused on addressing the top community health priorities identified in the community health needs assessment conducted by Continuing Care Hospital in FY2013. Identifying Health Needs Continuing Care Hospital identified community health needs by undergoing an assessment process in collaboration with the Lexington-Fayette County Health Department and the Lexington-Fayette County Mobilizing for Action through Planning and Partnership (MAPP) Coalition. Over 1,200 residents provided input through a community health survey. Another 77 adults participated in three focus groups intended to identify needs in minority and low-income households and five individuals with specialized knowledge participated in key informant interviews.in addition, secondary data was compiled from demographic and socioeconomic sources as well as national, state and local sources of information on disease prevalence, health indicators, health equity and mortality. This was analyzed and reviewed to identify health issues of uninsured persons, low-income persons and minority groups, and the community as a whole. Health needs were prioritized utilizing a method that weighs: 1) the impact on vulnerable populations; 2) the importance to the community; 3) the size of the problem; 4) the seriousness of the problem; 5) prevalence of common themes; 6) how closely the need aligns with the strategies and strengths of the hospital and KentuckyOne Health; and 7) an evaluation of existing hospital programs responding to the identified need. The hospital engaged BKD, LLP to assist with compiling secondary data and prioritizing identified health needs. Subsequently, Continuing Care Hospital s leadership entered into a dialogue to discuss the results of the evaluation and select health priorities. Participants were given the opportunity to revise rankings and debate issues until a consensus was reached on a composite ranking of health issues. The process identified the following issues with scores of 14 or more (on a scale of 28): Obesity Heart Disease Access to Care/Cost of Care Uninsured Lack of Health Knowledge/Health Promotion Stroke Cancer Drug Abuse Chronic Lower Respiratory Disease Excessive Drinking Continuing Care Hospital Page 2

4 As a result of the analysis, the following areas were identified as priority areas on which we will focus over the next three years. Continuing Care Hospital Priorities Obesity Correlated Community Health Need Obesity Lack of Health Knowledge/Health Promotion Heart Disease Lack of Health Knowledge/Health Promotion Stroke Cancer Chronic Lower Respiratory Disease Access to Care/Cost of Care Uninsured Continuing Care Hospital Page 3

5 PRIORITY: Obesity Goal 1: Promote employee-based and community wellness programs. Strategies: A. Expand employee and community education about the benefits of healthy eating and active living. B. Promote existing hospital programs that benefit employees and the community (bariatrics, diabetes, wellness, etc). C. Maintain professional memberships to assist with programs offered through lunch & learn, churches, wellness programs, school programs, and the YMCA. Goal 2: Promote healthy food choices in the counties served. Strategies: A. Explore the feasibility of facilitating patient referrals to community-based resources to include local farmer s markets. B. Offer free community education programs on healthy eating and living. C. Promote and participate in the Partnership for a Healthier America program, an initiative supported by Catholic Health Initiatives and other large health systems to improve the environment by eliminating waste and fostering more sustainable practices. D. Post nutrition information on foods served in the hospital cafeteria and promote healthier menu options for both children and adults. E. Collaborate with approved vendors to increase purchasing of local food products and actively promote items branded as Kentucky Proud. PRIORITY: Prevention and Management of Goal 1: Promote awareness of prevention, control, and self-management of diabetes. Strategies: A. Continue to offer diabetes education classes and individual counseling. B. Provide pre-diabetes and diabetes refresher education offerings to enhance disease prevention. C. Partner with Saint Joseph Hospital Bariatric Center to provide educational classes. D. Explore providing diabetes education to the Hispanic population in culturally appropriate venues. Goal 2: Enhance community education about the benefits of healthy living. Strategies: A. Expand educational and health promotion through screening, seminars, presentations and event promotion at nontraditional sites such as community centers, churches, and schools. B. Explore partnering with Visiting Nurse Association to link patients with appropriate resources and information to prevent and manage chronic diseases in order to decrease hospitalizations. C. Collaborate with Saint Joseph Hospital and Saint Joseph East on developing an educational brochure to include a listing of available resources within the community regarding: Physical Activity Facilities and Programs, Nutrition and Healthy Cooking Resources, Chronic Disease Management Programs. 6 Goal 3: Increase awareness of cardiovascular health through wellness initiatives. Strategies: A. Encourage companies and corporations to include cardiovascular screening services and healthy lifestyle education. B. Partner with community venues such as churches, colleges, and community health fairs. Needs Not Addressed Some issues identified through the community health needs assessment have not been addressed in this plan. In initial discussion and subsequent prioritization, Continuing Care Hospital s Community Needs Assessment Team considered the levels to which some needs were already being addressed in the service area. Additionally, some community needs fall out of the scope of expertise and resources of Continuing Care Hospital. The following chart outlines how some of the needs identified in the assessment are addressed by others or in different ways: Community Need Excessive Drinking How Need is Addressed Substance abuse treatment services are offered by numerous organizations within the Lexington, Kentucky area.
