3 Implementation Strategy Introduction Established in 1971, Eisenhower Medical Center is a dynamic, progressive health care complex comprised of a 463-bed hospital, the Annenberg Center for Health Sciences at Eisenhower, and the Barbara Sinatra Children s Center at Eisenhower, in addition to its outpatient facilities in Yucca Valley, Palm Springs, Cathedral City, Palm Desert, Rancho Mirage and La Quinta. The Betty Ford Center is also located on the Eisenhower campus. Eisenhower Medical Center is a not-for-profit organization that exists to serve the changing health care needs of our region by providing excellence in patient care with supportive education and research. Eisenhower is an accredited teaching hospital, with a School of Graduate Medical Education training new physicians in family medicine and internal medicine. Eisenhower has established health centers in Palm Springs, Cathedral City, Rancho Mirage, Palm Desert and La Quinta (its primary service area) to serve the health needs of a diverse population. The health centers provide a variety of services, including primary care physicians, specialists, breast centers, imaging centers, laboratories and urgent care centers. Eisenhower also provides off campus occupational health centers in Cathedral City and La Quinta, an adult day care center in Palm Desert, and the Eisenhower Wellness Institute in La Quinta. Community Health Needs Assessment In 2016, Eisenhower Medical Center conducted a Community Health Needs Assessment (CHNA) to comply with state and federal regulations guiding tax exempt hospitals. The CHNA is posted on the hospital website and can be accessed at Public comment on the CHNA is encouraged and comments are used to inform this work. The Community Health Needs Assessment incorporated demographic and health data for the communities served by the hospital. Significant health needs were identified from issues supported by primary and secondary data sources gathered for the Community Health Needs Assessment. The needs were indicated by secondary data sources and community surveys. Health indicators were considered significant health needs when they exceeded benchmark data, specifically county or state rates or Healthy People 2020 objectives. The following significant health needs were identified: Access to health care Asthma/lung disease 3

4 Cancer Cardiovascular disease Dental health Diabetes Environmental health Mental health Overweight and obesity Safety/violence STD/HIV/AIDS Substance abuse (alcohol, drugs, tobacco) Teen births A complete description of these health needs can be found in the 2016 CHNA report. Priority Health Needs The identified significant health needs were prioritized with input from the community. Surveys with community stakeholders were used to gather input on the identified health needs. Participants were asked to use quantitative rating scales to prioritize each of the health issues. The scales measured perceptions of the importance of addressing the issue, the severity of the consequences of each issue, the adequacy of the existing resources to address each issue, and the overall trend of the issue over time. The prioritized health needs can be found in the 2016 CHNA. Significant Health Needs the Hospital Will Address This Implementation Strategy describes how Eisenhower Medical Center plans to address the selected significant health needs identified in the 2016 Community Health Needs Assessment. Eisenhower examined the significant health needs and applied the following criteria to identify the health needs it will address: organizational capacity, established relationships, ongoing investment, and acknowledged competencies and expertise. As a result, Eisenhower will address the following health needs through a commitment of community benefit programs and charitable resources: 1. Access to health care 2. Chronic diseases (asthma, cancer, cardiovascular disease, diabetes, HIV/AIDS) 3. Preventive practices For each health need the hospital plans to address, the Implementation Strategy describes: actions the hospital intends to take, including programs and resources it plans to commit; anticipated impacts of these actions; and planned collaboration between the hospital and other organizations. 4

5 Access to Care Goal Increase access to health care for the medically underserved. Strategies Eisenhower will address access to health care by taking the following actions: 1. Provide financial assistance through both free and discounted care for health care services, consistent with the hospital s financial assistance policy. 2. Provide transportation support to increase access to health care services. Impact The anticipated impact of these actions will be to: Increase availability and access to health care Provide financial assistance to qualified patients Aid access to health care services by providing transportation assistance Collaboration To address access to care, Eisenhower plans to collaborate with: Coachella Valley Volunteers in Medicine and other nonprofit clinics and health organizations Eisenhower health centers Chronic Diseases Goal Reduce the impact of chronic diseases on health and increase the focus on chronic disease prevention and treatment education. Strategies Eisenhower will address chronic diseases by taking the following actions: Provide chronic disease education, screening and treatment Provide support groups to assist those with chronic diseases and their families Provide funding support to community organizations to support chronic disease prevention and treatment Provide nutrition consultations with Registered Dieticians Impact The anticipated impact of these actions will be to: Increase the identification and treatment of chronic disease Increase public awareness of chronic disease prevention Increase individuals compliance with chronic disease prevention and 5

6 management recommendations Collaboration To address chronic diseases, Eisenhower plans to collaborate with: AIDS Assistance Program Alzheimer s Association American Cancer Society American Diabetes Association American Heart Association Desert AIDS Project Mizell Senior Center Neurovitality Center Preventive Practices Goal Improve community health through preventive practices. Strategies Eisenhower will address preventive practices by taking the following actions: Provide free health screenings. Provide education and resources focused on healthy living and disease prevention. Reduce injuries and falls among seniors through exercise and balance classes. Provide funding support to community organizations to support preventive health services. Impact The anticipated impact of these actions will be to: Increase availability and access to preventive care services. Increase individuals compliance with preventive care recommendations. Collaboration To support preventive practices, Eisenhower plans to collaborate with: American Cancer Society American Diabetes Association American Heart Association Barbara Sinatra Children s Center Cathedral City City of Palm Springs 6

7 Indio Senior Center Jewish Family Service of the Desert Joslyn Center LGBT Community Center of the Desert Evaluation of Impact Eisenhower Medical Center will monitor and evaluate the programs and activities outlined above. The hospital has a system that tracks the implementation of the strategies and documents the anticipated impact. The reporting process includes the collection and documentation of tracking measures, such as the number of people reached/served, increases in knowledge or changes in behavior as a result of planned strategies, and collaborative efforts to address health needs. An evaluation of the impact of the Medical Center s actions to address these significant health needs will be reported in the next scheduled Community Health Needs Assessment. Needs the Hospital Will Not Address Taking existing hospital and community resources into consideration, Eisenhower will not directly address the remaining health needs identified in the CHNA including: dental health, environmental health, mental health, overweight and obesity, safety/violence, substance abuse, and teen births. Eisenhower cannot address all the health needs present in the community; therefore, it will concentrate on those health needs that can most effectively be addressed given the organization s areas of focus and expertise. 7
