Advocate Charitable Foundation AIDS Foundation of Chicago Toward the Mobile Dental Program at Advocate Illinois Masonic Medical Center which provides direct care, outreach, and preventive education to reduce inequities in oral health and improve the quality of life for underserved individuals across Chicago. For the Chief Clinical Officer position which addresses two major goals: improving the existing local HIV care system and helping the system prepare for implementation of the Affordable Care Act. $75, American Cancer Society, Inc. For Patient Navigation Services at John H. Stroger, Jr. Hospital of Cook County which reaches the County's most vulnerable and underserved cancer patients with comprehensive resources and support to help reduce barriers to care and improve their health. $35, American Indian Health Service of Chicago, Inc. To support a community nurse who will provide comprehensive diabetes prevention, treatment and health promotion services for medically underserved men, women and children in lowincome communities. $35,000.00

Chicago Children's Advocacy Center To improve access to specializedmmental health services throughout Chicago for children who have been sexually abused and their families. Chicago Jesuit Academy To provide medically under-served students and their family's continued access to a school nurse. $10, Chicago Women in Philanthropy In support of the Women s Leadership Mentoring Program. $3, Chinese American Service League For In-Home Service-Intensive Care Coordination which provides holistic care for at-risk seniors.

Communities in Schools of Chicago To connect Chicago Public School Students to a range of essential healthcare services and strengthen the role of school nurses. CommunityHealth Funding will support salaries and benefits for key nursing staff. $65, Council on Foundations For philanthropy resources for foundations; program support. $4, Corporation for Supportive Housing To help local supportive housing organizations capitalize on opportunities to partner with the health sector to better serve their clients and to achieve greater self-sufficiency. $20, Donors Forum For philanthropy resources for Illinois foundations; program support. $4, DuPage Health Coalition dba Access DuPage; DuPage Community Clinic To secure wide scale pro-bono specialty care consultation and treatment for low income and uninsured residents of DuPage County. 4 EverThrive Illinois To pilot a program which will allow school health center staff to utilize texting to send appointment reminders and requests, and which will engage youth to develop health messages to text to peers. $20, Facing Forward to End Homelessness To support nursing case management services within permanent, supportive housing for homeless women and families. $25, Family Focus To give children a healthy start in life by providing home visiting, prenatal classes, developmental screenings, parent education and support groups. $20, Family Health Partnership Clinic To utilize the new Family Health Partnership Clinic in Crystal Lake as a training ground for nurse practitioner students. Fox Valley Volunteer Hospice In support of hospice care for Spanish-speaking people. $5, Goldie's Place For comprehensive dental services for homeless adults provided by volunteer dentists and dental students. Health & Disability Advocates To provide outreach and education regarding the Affordable Care Act to community-based providers through training and technical assistance, and to build capacity to maximize enrollment and eligibility in new and continuing health coverage programs. $20,000.00

To create a full time position for a Nurse Clinical Manager. Heartland Health Centers To maintain the Prenatal Case Management Program which assists low-income, pregnant women by providing care coordination, individualized counseling with a HIHC nurse and necessary referrals. Heartland Health Outreach To add a dental hygienist/manager who will provide preventative oral health services to individuals who are homeless or have HIV/AIDS, and absorb some administrative duties from the dental director, allowing the dental director to focus on treating more serious dental conditions. Howard Brown Health Center To improve the health of homeless LGBT and other medically underserved youth through the delivery of acute medical and sexual health services at the Broadway Youth Center. Illinois Foundation of Dentistry for the Handicapped dba Dental Lifeline Network Illinois To bring dental care directly to Chicagoans with disabilities or who are elderly and cannot travel to dental offices for necessary treatment. $25,000.00

6 Illinois Association of Free and Charitable Clinics To contract with a grant writer so the Consulting Executive Director can focus on a range of timecritical activities to establish the association and support free clinics. $5, Illinois Association of Free and Charitable Clinics To implement initiatives that will advance free and charitable clinics as a recognized and integrated sector of the new health safety net and to continue infrastructure building. $75, Infant Welfare Society of Chicago To support the bilingual nursing leadership position which is vital to clinical operations. Inner-City Muslim Action Network To provide free primary care to diverse uninsured residents in Chicago Lawn and surrounding communities. Instituto Health Sciences Career Academy To support a full-time Nurse position which is a crucial part of the school support team. Interfaith House For a respite care center that provides medical care and housing to homeless men and women after they have been discharged from the hospital. JourneyCare (formerly Hospice of Northeastern Illinois) To provide pain and symptom management for people with chronic illnesses. Korean American Community Services To support a nurse practitioner who provides outreach and education services, and free and lowcost preventive and acute care services. $48,660.00

Metropolitan Chicago Breast Cancer Task Force To support the Manager of Community Outreach who will disseminate information on best practices for outreach/education, host a breast cancer health series on CAN TV, and promote mammogram screening. Old Irving Park Community Clinic To support the salary of the Clinic Manager. PCC Community Wellness Center To extend maternal child health services beyond clinic walls and into hospital rooms and homes of new mothers most in need of breastfeeding education and support. Pilsen Homeless Services For general operating support of this agency providing mobile and clinic-based health services to homeless individuals and families. $1, Planned Parenthood of Illinois To support the salary and benefits of a part-time Advanced Practice Nurse at the Roseland and Englewood health centers. Porchlight Counseling Services To improve the physical and mental health of college sexual assault survivors by providing counseling to reduce trauma symptoms, teach coping skills and help survivors stay in school. $35, Public Health Institute of Metropolitan Chicago For the Suburban Cook County Intimate Partner Violence (IPV) Healthcare Coalition working to improve the healthcare response to IPV and increase awareness.

Sarah's Circle To provide formerly homeless women who have recently been placed in permanent housing with physical and mental health services to improve their health and housing retention. $25, Suburban Primary Health Care Council To enable low-income, uninsured adults to access primary care through contractual services with local providers. $35, Swedish Covenant Hospital To support home telemonitoring and care coordination to low-income patients with chronic diseases following hospital discharge. Teen Living Programs To provide homeless youth with primary and mental health care to assist them on the path to selfsufficiency. Teen Parent Connection To support doulas who provide young mothers with home-based prenatal education and support services. $25,000.00
