3 Contact details The Woodlands reception number The Ferns Telephone number Where possible we ask that you telephone the ward after 11am, to allow staff time to help patients with washing / dressing and breakfast. Introduction to The Ferns The Ferns is a 12 bed unit, specifically for patients with organic illness, who have complex needs. We provide a person centred approach to the assessment and delivery of care and treatment. The emphasis of this is on understanding and supporting an individual s qualities, as determined by their life history, experiences, likes and dislikes, and their needs and abilities. 3
4 Our aim is to work in partnership to promote the mental, physical and spiritual wellbeing of people who access the help and support of the Older Peoples Mental Health Services at The Woodlands. All staff will care for and respect the beliefs and values of all users of the service and their carers by Engaging and listening to all our service users Respecting people s privacy and dignity Providing opportunities to enable individuals to achieve their potential. We provide a holistic service that aims to support individual strengths and abilities in order to maximise and promote independence and wellbeing, and to enable individual to feel safe and valued. Patients, their families or carers are central to this approach, and will be involved in all decision about care and treatment. Multi-disciplinary Team You will be under the care of a consultant psychiatrist and an associate doctor while you are an in-patient on The Ferns. You will also be allocated a named nurse and a nursing team who will co-ordinate an individual plan of care with you. Your consultant is:... Your associate doctor is:... 5 Other professionals will also be involved in assessing and planning your care during your stay. These may include occupational therapist, physiotherapist, dietician, speech and language therapist and pharmacist. All members of staff wear a name badge and some members of the team wear uniform. There is a telephone in the central area for making calls. However telephone calls can be received on all wards There is a TV / sitting room, a dining room, and separate female and male only quiet lounges General ward facilities All rooms are en-suite and have a private sink, toilet and shower. There is a separate bathroom on the ward if you prefer to use a bath There is a kitchen on the ward which can be accessed by staff on your behalf, 24 hours a day. You can store food and drinks of your choice if you prefer. (Food can not be heated by staff) 5
6 There are therapeutic activities which you will be encouraged and supported to take part in e.g. baking, reminiscence, games and discussions. Treatment and Care While you are on the ward a multi-disciplinary assessment of your physical, mental health and spiritual needs will be completed. You will be allocated a named nurse, who will develop a person centred care plan. You will be asked if you or your carers would like to have a copy of this. Multi-disciplinary team review meeting Within the first few weeks of your admission the multi-disciplinary team will meet to review the outcome of your assessments and subsequently plan your care and treatment. You will be included in this and you are welcome to invite a relative, a carer, friend or advocate to attend with you. This meeting takes place each week and monitors your progress. Please let members of the nursing team know if you would like your relative, friend, carer or advocate to attend the meeting with you and they will arrange this. Medication When you are admitted to hospital you will be asked about any medication you are currently taking. If you are prescribed any medication during your stay you will be given the choice to discuss the different treatment options. You will also be given written information on any prescribed medication and the opportunity to discuss with a relevant member of the multi-disciplinary team, your carer or advocate. A pharmacist is available to discuss medication with if you prefer. 6 Welcome to The Ferns
7 Discharge planning Our aim is to support you in your recovery so that you feel well enough to return home or to supportive accommodation. At all times your needs and views are central to the process of planning your discharge. A date for discharge will be discussed with you and your carers when you attend the multi-disciplinary meeting. We will discuss with you any help and support you require following discharge, to meet your health and social care needs. You may need a referral to a social worker to assess and provide support in the community. If this is required this will be discussed with you and your carers. There is a national system for delivery of care in mental health services called the Care Programme Approach (CPA). If this applies to you, you will have a CPA care coordinator who will be responsible for overseeing your care. If you would like to know more about CPA, we can give you a leaflet. Training staff An important part of our work is training the doctors, nurses and other professionals for the future. Students in training regularly spend time working on the ward. All students are supervised by fully qualified staff members. On occasions, we may ask if you are happy for a student to provide your treatment. You have the right to say no, and this will be respected. It will not affect the care you receive. Research and a range of improvement projects will help to develop the care and treatment that patients will receive in the future. We are always looking for ways to involve patients in this please let us know if you would like to take part. Sometimes the ward is involved in research projects, and it is possible that we may ask you to help. 7
8 This will only happen with your permission and you have the right to refuse. If you refuse it will not affect the care you receive. Practical issues Meal, snacks and refreshments Meals are served in the dining area on the ward. Meals are served at approximately: Breakfast 8am Lunch 12pm Evening meal 5pm Supper 9.30pm onwards. Snacks and drinks are offered throughout the day. The Ferns operate protected meal times on the ward, which means that at these times no clinical activity takes place, other than in an emergency. This means that all staff are available to assist with meal provision. We do not encourage visiting at meal times, but there may be times that staff asks a relative or carer to attend at meal times to assist with feeding their relative/friend. We aim to provide varied and nutritious meals that take into account dietary and cultural needs. We will offer: A choice of dishes to suit individual dietary needs A choice of portion size. Please ask staff if you have any special requirements, such as halal or vegetarian, or if you have a strong dislike or allergy to foods. There may be occasions when a referral to a dietician is requested as part of your plan of care. Nursing Staff will discuss this with you or your carer. Clothing While in hospital you will need day and night clothing, as well as indoor and outdoor shoes. Staff will encourage and help you to get dressed each day in day clothing. 8 Welcome to The Ferns
9 You will need to bring your own supplies of toiletries, makeup, perfume / aftershave. Where possible we will ask you to send your personal clothing home to be washed. Please speak to the nurse on The Ferns if this cannot happen, as we may be able to help. Money and valuables The finance department is situated at the entrance to The Woodlands. Any money or valuables you bring onto The Ferns will be kept for safekeeping in this department and a receipt will be given to you. A small amount of money can be kept in the ward safe, to allow newspapers, sweets etc to be purchased. We will try to take care of your clothing and personal belongings, but we can not take responsibility for missing clothing or valuables on the ward. We strongly recommend that all items of value should be send home for safe keeping or placed in the finance department. Photographs In order to deliver person centred care, you may be asked to bring in photographs of you and your family. These can be photocopied, or if they are recent digital photographs they may be printed from a computer. Staff will then laminate them to use in discussion with you. We will try to take care of your photographs, but they may be lost or damaged on the wards, so please do not bring photographs that are valuable to you. Hairdressing The Woodlands has a hairdressing room that can be used by staff and visitors to wash and dry hair. We also have a hairdresser who visits each week who can cut, perm and colour hair. If you would like to use the hairdresser, please let a member of staff know. There will be a charge for using this service. 9
10 Car parking Visiting times There is limited disabled parking only in front of The Woodlands unit. All other visitors will be required to use the parking arrangements on the Rotherham Hospital site. Information on local transport arrangements can be found in the entrance to the unit. Visiting times on the ward are: 10 Welcome to The Ferns 2-4pm and 6-8pm every day. If you are unable to visit at these times, arrangements can be made for you to visit at other times. If this is required, please speak with the nurse in charge on the ward to arrange this.
11 Visitors will be asked to visit in the central area of the unit, where a café run by the local Alzheimer s Society is available for refreshments. Occasionally, this may not be possible and arrangements will be made for your visitors to stay on the ward with you. If children are likely to visit please inform the nurse in charge, as we have a family friendly visiting area in the unit. If there are certain people you wish not to visit you while you are on the ward, please inform one of the nurses. Visitors are asked to respect the privacy of other service users / patients on the ward and comply with ward safety regulations. use before entering. Hand washing facilities are available in the toilets in the central area. Telephone and mobile phone use At the entrance to The Woodlands is a pay telephone. If you ask a member of staff they will help you to use this. Telephone calls can be received on the ward if this is more convenient. Where possible we ask that phone calls are received after 11am. Mobile phones can be used within the ward area, but we ask that you consider the views of the other patients on the ward and only use them in the designated areas. The taking of photographs of staff, patients or facilities at The Woodlands is strictly forbidden. To prevent the spread of infection we ask that visitors do not attend the ward if they have signs of a cold, cough or stomach upset. Alcohol hand gel is available at the entrance to the ward for visitors to 11
12 Safety of patients, visitors and staff We are aware that being in hospital can be upsetting, frightening and confusing, and staff will be keen to talk to you about your feelings and how they can help you to feel safe on the ward. Please tell staff if you feel worried or unsafe at any time, or if you feel concerned about the safety of anyone else on the ward. If you would prefer to talk to someone outside of the ward, you can ask to speak to the modern matron or any of the advocacy services. If you wish to make a complaint about safety on the ward, this will be carefully investigated and we will do all we can to maintain the safety of everyone while this happens. We aim to provide high standards of care and service and, in return, we all expect to be treated with respect. The ward staff and the Trust are committed to providing a safe and pleasant environment for people who use the service and our staff. Violence and abuse will not to be tolerated by anyone on the ward, and this includes staff, patients and visitors. Please do not bring dangerous objects, such as knives, into our premises. Items such as razors and nail scissors should be handed to staff for safe keeping until they are needed. Failure to respect this code of behaviour can cause distress to others staying and working on the ward. Environment We aim to make sure you are cared for in a clean, safe and pleasant environment and we ask that as a patient you respect your environment. As an NHS service, we are concerned about the risk of smoking to general health, both for those who smoke and for those who have to be near smokers. For this reason, and to comply with the law, we do NOT allow smoking on 12 Welcome to The Ferns
13 the premises. RDaSH is a smoke free trust and no smoking is allowed on the grounds at all. All patients who smoke will be offered nicotine replacement therapy to help manage their nicotine levels. If you would like to stop smoking, our staff will be very pleased to support you with this. Delivering same sex accommodation We are proud to confirm that mixed sex accommodation has been eliminated in all our inpatient wards. Every patient has the right to receive high quality care that is safe, effective and respects their privacy and dignity. RDaSH is committed to providing every patient with same sex accommodation because it helps safeguard their privacy and dignity when they are often at their most vulnerable. 13
14 Chaplaincy service During your admission, if you wish to speak with a member of the chaplaincy team, please inform a member of the ward staff who will then contact the chaplaincy department on your behalf. If you would prefer, your community chaplain can visit you while you are in hospital. Chaplains for Rotherham and Doncaster can be contacted through the chaplaincy office which is based at the Spiritual Care Centre, Tickhill Road Hospital Doncaster. Comments, compliments and complaints We aim to provide the highest possible standards of health care for people using our services. If you have a comment, compliment or complaint to make about our service, we would be pleased to hear from you.
