2 Introduction The team on Silver Birch Assessment Ward would like to welcome you to the ward. We hope this booklet will answer some of your questions whilst you/ your relative is in hospital. The team on Silver Birch believe that everyone has the right to be treated as an individual with dignity and respect. Our ward vision is to ensure everyone is involved with their care and treatment throughout their period of assessment on the ward. We understand that this may be a difficult time and we want to help however we can. Please speak to a member of the ward team if you have any questions. What is Silver Birch Assessment Ward? Silver Birch Assessment Ward is a twenty bedded assessment ward. The Ward is dedicated to the care of patients needing assessment, which provides Multidisciplinary assessment and interventions for people who present with different types of cognitive impairment. There are three lounges, one dining room, an activities / Snoozelen room and garden. How can I find Silver Birch Assessment Ward? We are located at Highbury Hospital which is on Highbury Vale in Bulwell. The Basford Registry Office is next door. The Silver Birch Telephone number is ext Your Ward Manager is.. Ward Manager s telephone number is ext Your Clinical Lead is. Telephone: ext Access to the hospital Car parking at the hospital is free but space might be limited at busy times. Regular buses stop outside the main entrance from Bulwell and Nottingham City. Access to the hospital is also possible via train to Bulwell and tram to Highbury Vale. Health and security For the safety of our patients we operate a secured door system. Access for visitors is made possible using an Intercom system at the main entrance. Please be patient if nobody responds to the bell immediately, sometimes staff may be busy attending to patients and they will let you in as soon as they are free. Please use the alcohol hand gel that is available on entry to the Ward to help us keep a clean environment. 2 3
3 What happens when I am admitted onto Silver Birch Assessment Ward? A member of nursing staff will meet with you to explain the admissions process. A named nurse will be responsible for organising all initial assessments during the admission. This nurse will discuss any care needs and care plans with you/your relative. This will include skin state, mobility, nutrition, continence, self-care, communication skills and social interaction. On admission to Silver Birch ward you/your relative will be seen by a doctor. This may include an examination/ review to check on your physical health with the Doctor. Once you/your relative has been assessed, you may wish to speak to the ward consultant. You can make an appointment through the nurse in charge. The ward team will review progress through weekly Multi Disciplinary Team (MDT) meetings. These ensure that you/your relative are placed in the least restrictive environment that safely meets all care needs. When assessment and intervention has completed on Silver Birch Ward you may go home, be transferred to another Ward, or discharged directly to a nursing home depending on your needs. Your Named Nurse is Your Key Worker is... Your Responsible Clinician is Our ward team includes nurses, healthcare assistants and an activities co-ordinator. We also have specialist input from Psychology, Physiotherapy, Speech & Language Therapy, Pharmacy and Dietetics. We seek the advice of specialist continence, tissue viability and diabetes nurses where required. Information for relatives/carers When a person with cognitive impairments is admitted to hospital it can be helpful for the ward to ask for information from relatives as, based on the severity of their cognitive impairment, the sufferer may not be able to contribute to their own assessment. We aim to involve relatives throughout assessment and treatment. We ask relatives to complete the THIS IS ME booklet about the person with memory difficulties so we can learn more about them and their life. Sometimes a person with cognitive impairment or a diagnosis of dementia will need a relative to act as their advocate. We are happy to speak to you on the telephone if needed. Please speak to the nurse in charge if you have any concerns about care on the ward and please complete the questionnaires to provide feedback about the ward. What happens when I am discharged? When assessment and treatment is finished and transfer/ discharge is appropriate the ward team will discuss future care needs and plans with you/your relative. They will also plan transfer/discharge arrangements with you, and keep you and your family member up to date with the process. 4 5
4 Mobile Phones To minimise noise on the ward we ask that mobile phones are either turned off or switched to vibrate. What do I need to bring with me into hospital? Please bring an adequate supply of the following items: 1. Clothing 2. Nightwear 3. Slippers and shoes 4. Toiletries/hairbrush 5. Walking stick/frame (if used) 6. Hearing aid/glasses (if worn) We do not supply any toiletries. Please avoid bringing: 1. Valuables 2. Expensive jewellery 3. Large amounts of money The hospital has a laundry service but cannot wash delicate items such as woollen jumpers. These can be bagged and placed in your wardrobe for your relative to wash. Department of Spiritual and Pastoral Care We are part of the hospital team and here to help care for you, whether you are a patient, their relative or a staff member, during your time spent at the hospital. Spiritual does not always mean the same thing as religious. You don t have to feel you are a religious person or a worshipper to make use of our services. Patient Advice and Liaison Service (PALS)/ Making a Complaint. If you have any issues or concerns, please contact the Ward Manager in the first instance. If the Nursing staff are unable to resolve your concerns then please feel free to contact the Patient Advice and Liaison Service (PALS). PALS works with staff to ensure that you and your relatives have information and a way of resolving issues and concerns you may have. Chaplains are always ready and willing to listen. 6 7
