2 SECTION A PROJECT SPECIFIC INFORMATION INCLUDE ADDITIONAL SHEETS AS NECESSARY TO COMPLETE THE QUESTIONS. 1. PROJECT TYPE (All applicants must complete this section) The proposed project will involve the following activities regardless of the funding source. Check all that describe the project, including activities that will be funded through other revenue/grant sources. PROJECT PURPOSE AND DESCRIPTION (All applicants must complete this section) Describe the project and explain why the project is needed. If necessary, attach additional sheets or submit architectural, site, or construction plans. This description should be the entire project, not limited to the portion for which funding is requested (ex. renovate a transitional housing facility with 25 units, etc.). Priority #1- Total Building Condition Assessment $35, A Building Condition Assessment (BCA) provides a comprehensive building deficiency information and forecasts possible future maintenance or repair requirements for a single or multiple properties. BCA s will help WestCare develop a long-term capital planning of expenditures for major repairs/replacements of building systems and equipment. Additionally, BCAs can be used to address building code issues, fire code requirements, and accessibility legislation requirements. This request is for a single building (Davis-Bradley). The BCA is expected to be carried out in two steps. The first being the site inspection, which is a systematic recording of the building systems with the use of photographs, note taking, drawings and information provided to WestCare by the consulting team lead by an Architect who would bring in other appropriate disciplines (consultants) as needed. The second step is analyzing the data collected by this assessment team and turning it into a report that includes a summary of the building, its systems condition, tables that reflect the immediate and projected long-term costs of maintaining the building. The detailed BCA will provide estimated costs connected with renewal, repair, and code compliance issues. Building Condition Assessments can also be used to determine the need for the following: Facilities operational costs including lifecycle and building systems renewals Maintenance programs including current and deferred, such as structural, window replacement, HVAC systems, etc. Page 1 of 20

3 Functional deficiencies will be identified, photographed, and recorded, using industry standards (Building Codes, Accessibility, BOMA, IFMA, & LEEDS), and applying replacement costs using traditional cost databases like BCIS, RS Means, and historical benchmark data. This information allows WestCare s key facility team members to address current operational requirements, while also improving the portfolio renewal forecasting and addressing capital funding for capital projects. From the data gathered from the BCA the facility management team will be able to develop a long-term maintenance program that can address specific needs and conditions of Davis- Bradley, which in turn helps our Senior Vice President of Facilities (Mike Lavin) maintain the building portfolio in an operational condition while reducing maintenance costs. A BCA should ideally be conducted on a regular basis; the standard is a three to five- year basis. The BCA frequency also depends on the buildings usage, structural integrity, age, and size. Davis-Bradley is 64,000 sq/ft and was built in The BCA scope will include: Type of building (commercial, institutional, high and low-rise residences, offices, Government, etc.) Site components (parking lots, walkways, curbs, lighting, and site drainage) Structural & Foundation systems (exposed systems only) Roofing systems (type, age, and condition) Exterior enclosure (masonry, curtain wall, EFIS, etc.) Interior systems (walls, partitions, doors, finishes, etc.) Parking garages (multi-level, above and below ground) Plumbing systems (existing) Electrical systems (existing) Mechanical systems (boilers, HVAC, etc.) Vertical transportation systems (elevators, moving walkways) Building and fire code issues (accessibility compliance updates) The BCA report will include: Photograph documentation each system and requirement Assessment of the property's interior and exterior conditions, systems, components and deficiencies Recommendations to rectify physical deficiencies based on Consultant s experience Budget cost to remedy physical deficiencies based on the costing tools used by the software if system is unique the suggested cost should be applied Why do we need a BCA? In today s world where facilities operational budgets are always shrinking, we must find new ways to do more with less. There is a strong need to develop a strategy to be cost efficient when managing building maintenance and future replacement costs: today and for tomorrow. Building Condition Assessments are the most intelligent way to increase the building portfolio s life and yet at the same time reduce repair and maintenance costs. The main purpose of a BCA is so that our SVP of Facilities (Mr. Lavin) can evaluate the physical condition of the building portfolio, list any deficiencies existing in the Davis-Bradley s Page 2 of 20

4 building systems, assess the useful life of major components of the facility, and plan for longterm renewal and maintenance. Priority #2 Parking Lot including Architectural and Engineering $91, PARKING LOT AND GUARD SHACK +/- 42,100 SF - SAFETY REPAIRS We will pull all necessary permits and will have a licensed and insured company do the following for an asphalt overlay: 1. Power broom to remove all loose dirt and debris in the working area and haul away 2. Mill for transition along adjacent concrete curbs, gutter pans and metal structures (manholes etc.) Milling is done to tie into the elevations of the existing concrete and metal structures. Also to preserve the appropriate drainage patterns on the asphalt surface 3. Apply ss-1 tack coating system on entire area that is to be overlayed. 4. Apply 1 ¼ to 1 ½ inches of type iii asphalt and compact. Roll surface course to achieve a true and firm finish. 5. Lightly sand for set up 6. Repaint crosswalk, handicap, fireline, arrows and parking stalls. ASPHALT ON GRAVEL AREA 1. Power broom to remove all loose dirt and debris in the working area and haul away. 2. Base area at 4-5 inches (area that is needed) and re-grade 3. Apply 1 ¼ to 1 ½ inches of type III asphalt and compact. Roll surface course to achieve a true and firm finish. 4. Llightly sand for set up. 5. Re-stripe curb, blocks and arrows using the best paint approved by D.O.T. GUARD SHACK AREA +/- 535 SF PATCHED 1. Digout area 535 sf and base at 4 inches with crushed concrete (4 locations) 2. Apply ss-1 tack coating system on entire area that is to be overlayed 3. Apply 1 ¼ to 1 ½ inches of type iii asphalt and compact. Roll surface course to achieve a true and firm finish 4. Lightly sand for set up 3. Describe the use that will be made of requested CDBG funds from the City of Largo (ex. renovate five transitional housing units, Add a 150 square foot room; acquire a building; reconstruct 600 linear feet of road; provide the following services). 1. Building Condition Assessment of the Davis-Bradley building located at 1735 Dr. MLK S., St. Petersburg, FL. The Davis-Bradley Community Involvement Center (DBCIC) is a residential program for low/mod income homeless men and women who are generally court ordered to participate in substance treatment and provides critical social needs for these clients. 2. The repair of the parking lot surfaces and guard shack area in order to reduce trip, correct runoff, and other hazards. 4. Describe how the project meets one or more of the City of Largo CDBG Consolidated Plan Objectives (listed on pages 1 and 2 of the application instructions) and the priorities listed on Page 2 of this application (if applicable). 5 The Davis-Bradley Community Involvement Center (DBCIC) is a critical care facility and provides residential programs for low/mod income homeless men and women who are generally court ordered to participate in substance treatment and provides critical social needs for these clients. All clients at DBCIC are considered special needs population and one hundred percent (100%) report a substance abuse or mental health disorder. Provide information to substantiate the project as an Eligible Activity (described in the application instructions). Describe how one of the following qualifying criteria are addressed by the project: The project, or that part of the project funded by CDBG, principally benefits low- and moderateincome Largo residents or low- and moderate-income areas in Largo, or The Davis-Bradley Community Involvement Center (DBCIC) is a residential program for low/mod income homeless men and women who are generally court ordered to participate in substance treatment and provides critical social needs for these clients. All clients at DBCIC are considered special needs population and one hundred percent (100%) report a substance abuse or mental health disorder. Support capital improvements for homeless facilities. The project presents or eliminates slum and blight in Largo. 1. Support capital improvements to facilities that address critical social service needs, and services for senior, children and people with special needs. The Davis-Bradley Community Involvement Center (DBCIC) is a residential program for low/mod income homeless men and women who are generally court ordered to participate in substance treatment and provides critical social needs for these clients. All clients at DBCIC are considered special needs population and one hundred percent (100%) report a substance abuse or mental health disorder. Support capital improvements for homeless facilities. 5. PROJECT AREA (All applicants must complete this section): Submitted projects must provide services and/or benefit City of Largo residents a) Describe the project area and client base that will benefit from this project (e.g. Citywide, countywide, Community Redevelopment District, homeless population, persons with HIV/AIDS, etc.) The Davis Bradley Community Involvement Center is located in St. Petersburg and serves clients from all over Pinellas County including the City of Largo which is only about 20 miles away. The Largo clients served through this proposal are considered homeless but report their last permanent residence as the City of Largo. All individuals residing in these programs have a low to moderate income and have serious issues with alcohol and/or drug abuse. Additionally, many have experienced mental health issues which have contributed to their homelessness. Page 4 of 20

PROJECT/PROGRAM OUTCOMES (All applicants must complete this section): Describe the expected performance outcomes from the proposed project and any relevant performance measures or outcome data related to prior experience by the agency for the same or similar project/program (e.g. 50% of clients moved from shelter to permanent housing and it is anticipated that the proposed project will enable the agency to increase this outcome to 60% moved into permanent housing). Priority 1 Outcome: The outcome of the BCA will allow the SVP of Facilities (Mr. Lavin) the information and data needed to fully evaluate the physical condition of the building s portfolio, list any deficiencies existing in the Davis-Bradley s building systems, assess the useful life of major components of the facility, and plan for long-term renewal and maintenance. This will allow us to be smart and strategic with any future funding requests or investments. Priority 2 Outcome: To provide a safer (by reducing trip hazards) and more aesthetically appealing (the parking lot is visible from the public roadway) parking area for clients, staff and visitors. 7. RELOCATION (All applicants must complete this section): a) Will any residents be permanently displaced as part of this project? No more than 36% of gross income, up to $25 per day. The program that clients participate in is federally and state funded. If clients stay for the complete treatment, during the reentry phase of the program, they are required to obtain full-time employment. Once they ve obtained that employment, clients pay 36% of their salary; and the rest is placed in a savings account, which they receive after the completion of the program. Page 5 of 20

7 iii. Will residents receive rental subsidy/housing assistance? If rental or housing assistance is available through the local jurisdiction help will be available to fill out application forms and if needed credit counseling will be offered. Veterans will be connected to the VA for VASH vouchers. iv. If residents will not pay rent, state the reason. N/A v. These projects include federal, state, private, and local funding such as CDBG and other HUD programs. Jeramy has managed Florida projects which required compliance with Davis-Bacon and preparing bid documents. He also holds a General Commercial Contractors license for the state of Florida (#CGC ).
