2 General Information FIU admission requires the student s transcripts to be sent to a National Association of Credential Evaluation Services (NACES) organization ( The institution picked by is Josef Silny & Associates ( for credential review. The transcripts will go directly to Josef Silny & Associates for their review, and then Josef Silny & Associates will send the transcripts directly to Florida International University (FIU). For them to process your evaluations, they would need to receive the following from each student: Foreign Credential Evaluations Application (Sign and Dated) Original Transcripts with English Translation Payment for the Services Requested

12 Printing your Application Please print 3 copies of this application. Sign and date all three of them. Recommendations: Please review the application once is printed out. Make sure that all address details are clear and visible. -Many times, given that the application is two page-long, it will not show FIU address complete. Please try to avoid this situation. Complete details are of extreme importance to ensure that your evaluations are sent out to the correct FIU department. Make sure to SIGN your application If Josef Silny and Associates receives your application without your signature, they will stop your credential evaluation process. They will contact you to request an application with your signature, so please avoid delays in this process.

13 Josef Silny Payment How do I pay for my evaluation and translation? You may pay in U.S. dollars by cashier's check, money order, or a check issued by a U.S. bank. Josef Silny also accepts payment by travelers' check and by credit card (Visa, MasterCard and Discover only). If you wish to pay by credit card, go to Downloads in the Services section of their website, download the Credit Card Information Form and follow the instructions.
