Military Medic 4. How much does it cost to test? a. $65.00 dollars for one section of the test b. $75.00 dollars for both sections of the test 5. What form of payment do you accept? a. 2 7. I am a Student Nurse: What are the requirements for me to test? IMPORTANT: YOU MUST BE CURRENTLY ENROLLED AS A NURSING STUDENT AND TAKING CLASSES IN THE CURRENT TERM IN ORDER TO TEST AS REQUIRED BY THE INDIANA STATE DEPARTMENT OF HEALTH. A. You need to fill out Section 1, 4. 5 only. a. Section 4 is: Applicant s Signature, Phone Number and Signature Date b. You need to be enrolled in the nursing program in the current term. c. We need an official, sealed, unopened transcript that shows current semester s classes. Ivy Tech students must also send an official, sealed transcript. B. What if I can not get a transcript with current semester s classes on it? a. We will accept an official letter from the school showing your current enrollment. C. What if I have just taken the Fundamentals of Nursing or its equivalent? a. You can NOT test to be a CNA unless you are in the nursing program currently and taking classes during the current term. We must be able to view the grade for Fundamentals of Nursing. 8. How long do I have to test if I have an out-of-state license and I am working already? a. You have 120 days to test. 9. I am a transfer from Out-Of State: Am I required to take a test? a. You must take a written exam only. The test is 100 questions and is all multiple choice. We have practice questions at You must answer 80 out of the 100 questions correctly to Pass. 10. If I am an Out of State transfer or Nursing Student, how do I get information if I do not have access to a computer? a. Once we have received all your paperwork and payment, it will be 2 weeks before you are contacted. We attempt to schedule you within 30 days of receipt of all completed paperwork and payment. 3 12. What if there is a problem in processing my application? a. We will contact you by mail, phone or . b. Once we have received the payment and corrected paperwork, it will be 2 weeks from that date before you are contacted to be scheduled. 13. What happens if I decide not to test? a. We can refund your money to you. The refund process takes 3 to 4 weeks. Please call and leave your name and current address to where you would like the check sent. You have two years from your clinical completion date to test as long as you are not working as a CNA. 16. What happens if my 2 years are up to test? a. You must retake the 105 Hour training course over again 16. How many times can I test? b. You have 3 chances to test c. If you fail your 3 rd attempt, you must retake the training course. 17. Do I have to repay the testing fee? a. You must submit a new payment for each time you retest. 18. What happens when I pass the test? a. No, you must first apply for testing by sending in your application with payment. We do not offer preset test dates.

4 20. Do you have information on classes and cost? a. Our office only does the testing for CNAs and QMAs. You will have to contact the closest Ivy Tech campus to you if you are interested in taking classes at Ivy Tech. In addition, the Department of Health s website lists all approved programs for the state by county. 21. When do I find out if I pass or fail? a. You will be told your test results the day of testing. We will give you a letter, indicating your results. That letter will give you all the instructions on how to retest if you fail the test. 22. How do I retest? A. Please read and follow all directions below which you will also find on your fail form. B. You must mail a copy of your fail form to the address listed below. C. You must mail in a money order or cashier's check for either $65.00 or $ for retesting fee to the address listed below. a. $75.00 for both skills and written sections of the test. b. $65.00 for skills only or written only section of the test. D. You will not be scheduled until your results are received from the testing site into our office to be recorded. Please note it does take some test sites time to get the results to our office. No, they are two different tests. 25. What should I bring on Test Day for both skills and written? a. A State Issued Photo ID

We must receive notice of cancellation 48 hours prior to the test date or you will be required to repay the testing fee. 28. Where will I be scheduled? a. You will be scheduled at the closest testing location to the address on your application, unless, you have noted that you would like to be scheduled elsewhere.
