Special Services a) RFP 1487 Automated Vehicle Locator and Monitoring System (AVLM) Accept proposal received from LbTelematics of Memphis, Tennessee, for Automated Vehicle Locator and Monitoring System during the five-year period beginning October 1, 2016, with an option to renew annually, in the amount of $55,650. b) RFP 1494 Temporary Personnel Employees for the Water Testing Laboratory Accept proposal received from Lab Support of Glen Allen, Virginia, to provide temporary personnel, as needed, for the Authority s Water Testing Laboratory for a one-year period beginning October 1, 2016, with option to extend for an additional year.

4 Agenda 8/29/16 Page 4 c) RFP 1496 Performance of Water Tank Inspection Services Accept proposal received from H2M Water of Melville, New York, to provide water tank inspection services during the one-year period to begin upon the execution of this Agreement. 6. Members a) Review and Approval of Time Worked for Authority P. Halpin Review and approval of the schedule of time worked from March 30, 2016 through June 30, 2016 for Patrick G. Halpin, for the purpose of retirement reporting; and approve and post the reporting resolution on the Authority s website for 30 days, and thereafter file the resolution with the NYS retirement System. 7. DEC Applications a) Carleton Avenue Well No. 2, Central Islip 8. General a) Proposal for Reactivated Coconut Granular Activated Carbon Authorize Calgon Carbon to reactivate a 40,000 lb. load of coconut Granular Activated Carbon that had been previously used at the Authority s Flower Hill pump station in the amount of $4,900, which includes delivery to the Flower Hill vessels. 9. Budget a) Budget Amendment #2 - May 31, 2017 Authorize revision to the budget for year ending May 31, 2017, as follows: Decrease to Item 026 (Hydrants-Crew Jobs) in the amount of $175,000; Increase to Item 020 (Blow-Offs and Vault Conversions) in the amount of $175,000. These transfers will not increase the current capital budget of $71,610, Finance a) Year End Review May 31, 2016 Approve the following: i) Audited Financial Statements for the year ending May 31, 2016 and that the Statements be forwarded to the Governor, Chairman of the Senate Finance Committee, Chairman of the Assembly Ways and Means Committee, the State Comptroller, the New York State Authority Budget Office, and other applicable state and local agencies; ii) Annual Investment Report in draft including Investment Policy and Audited Investment Statements for the year ending May 31, 2016, and that the Statements be forwarded to the Governor, Chairman of the Senate Finance Committee, Chairman of the Assembly Ways and Means Committee, the State Comptroller, the New York State

Equipment a) Automatic Flushing Devices Authorize the purchase of three (3) permanent automatic flushing devices with de-chlorination systems (Hydro-guard HG-3) from Mueller of Chattanooga, Tennessee, in the amount of $24, b) Laboratory Purchases for Chemical Reference Standards Authorize the purchase of chemical reference standards for the Laboratory from AccuStandard Incorporated of New Haven, Connecticut, for a oneyear period beginning September 1, 2016, in an amount not to exceed $35, Meetings, Seminars, Training, Conferences a) American Metropolitan Water Agencies (AMWA) Annual Meeting Authorize the attendance of Jeffrey W. Szabo, Chief Executive Officer, at the American Metropolitan Water Agencies (AMWA) annual meeting, in Scottsdale, Arizona, from October 16 through October 19, 2016, at an approximate cost of $2,632. b) LabWare Training Authorize on-site training by LabWare for LIMS Basic and Crystal reports courses for 8 staff members, on dates to be determined, in the total amount of $10,710. c) New England Water Works Association Annual Conference Authorize the attendance of Joseph Roccaro, Water Quality Engineer, at the New England Water Works Association Annual Conference, in Providence, Rhode Island, from September 19 through September 21, 2016, at an approximate cost $1,600. d) Annual Northeast Arc Users Group Conference (NEARC) Authorize Tyrand Fuller, Director, Office of Strategic Initiatives, James Touchet, Vincent Lautato and Brad Feldman from the GIS Department to attend the Annual Northeast Arc Users Group Conference in North Falmouth, Massachusetts, from October 16, 2016 through October 19, 2016, at a total cost of approximately $4,490 covering all attendees. e) IDEC PLC Training Authorize Andrew Bloecker, Assistant Electrical Engineer, to attend the IDEC PLC training course in Detroit, Michigan, from September 13, 2016 through September 15, 2016, at a total cost of approximately $2,
