1 1 Dear Senior, Welcome to your senior year. Many of you are making plans to attend some form of post secondary schooling after May 2018 Graduation. Most of you will also want to apply for financial assistance in the form of scholarships, loans, and/or grants. We are very fortunate in Jefferson County to have numerous businesses and community organizations sponsor scholarships for students. The Guidance Department of Washington High School is publishing this directory, which includes most of the local scholarship listings. There are several optional forms in the front of the book. The first is the Scholarship Information Form which will be helpful for students in summarizing information needed in their scholarship applications. Many of the local community organizations, as well as the scholarship selection committee, are asking students to include this information in their letters of application. There are two worksheets that allow you to compare the cost of attending specific schools as well as the Financial Aid Packages offered from a specific school. Many scholarships ask students to demonstrate financial need. The last form enclosed is an optional form that a student may select to use if he/she wishes to demonstrate financial need. This Financial Need Form is OPTIONAL. Some scholarship committees will ask students to include the financial information that is in the FAFSA report. (SAR- Student Aid Report) Seniors and their parent(s)/guardian(s) are also required to complete the federal financial aid form known as the FAFSA (Free Application for Federal Student Aid) in order to receive financial aid. This form became available on line in October, Students and their parent(s)/guardian(s) must register for a FSA ID and password in order to enter your private information required on the FAFSA form. The FAFSA is the Free Application for Federal Student Aid that most post secondary schools utilize. Seniors should check with the financial aid office at the post secondary school which they are planning to attend as some schools require their own additional forms. You must have your 2016 tax information and social security numbers in order to complete the process. The PROMISE SCHOLARSHIP is a merit-based scholarship program that is designed to make college affordable to all qualified West Virginia students. Students are required to take the ACT or SAT. The Promise Scholarship Application must be submitted by March 1, 2018 online at A student must file a FAFSA by March 1, 2018 for Promise consideration. ( For eligibility requirements and further information, visit the website at ) The West Virginia Higher Education Grant is also designed to make college affordable to West Virginia students. Awards range depending on need and are applied towards tuition and fees for a full time student at an approved West Virginia or Pennsylvania college/university. The student must demonstrate financial need, have a 2.0 GPA, and submit FAFSA no later than March 1, Once the FAFSA application has been sent, the student is automatically considered for the West Virginia Higher Education Grant.

2 2 How do I get an FSA ID? The FSA ID process consists of three main steps: 1. See website, click on the Padlock Icon at the top of the page with FSA ID listed. Click on Create a FSA ID now Provide your address, a unique username, and password, and verify that you are at least 13 years old. 2. Enter your personal information. Provide your Social Security number, name, and date of birth. Include your mailing address, address, telephone number, and language preference. (Do not use your school address) For security purposes, provide answers to five challenge questions. 3. Submit your FSA ID information. Agree to the terms and conditions. Verify your address. (This is optional, but helpful. By verifying your address, you can use your address as your username when logging into certain ED websites. This verification also allows you to retrieve your username or reset your password without answering challenge questions.) Parents of a dependent student will need your own FSA ID in order to sign your child s FAFSA electronically. If you already have a PIN from previous years, you can link your information to your new FSA ID by entering your PIN while registering for your FSA ID. (This will save you time when registering for your FSA ID.) If you ve forgotten your PIN, don t worry; you can either enter the answer to your PIN challenge question during the FSA ID registration process to link your PIN, or you can create your FSA ID from scratch. A PIN is not required to create a FSA ID. Your information must be correct with the SSA before your FAFSA or FSA ID can be processed. You must be sure the information you are entering will match the Social Security Administration verification. If you believe they may have the wrong name or date of birth, you need to go to to find out how to correct any errors.

3 Once your information is verified with the Social Security Administration (one three days from the date you apply), you will be able to use your FSA ID to access your personal information on any of these ED websites, depending on what you need to do: or the National Student Loan Data System, to view a history of any federal student aid you have received. 3

7 7 N A S F A A National Association of Student Financial Aid Administrators Award Package Comparison Worksheet Different schools have different resources, so financial aid offers can vary greatly from school to school. Gift aid such as grants and scholarships are ideal, but often more limited. Use this worksheet to compare aid offers. When doing so, consider the amount of your federally-calculated expected family contribution (EFC) and the amount offered in "self-help" aid such as work-study and student loans. There may also be unmet need you need for aid that the school may be unable to fund--meaning that your parents may have to borrow additional funds through the student parent loan programs. You will also need to know and consider each school's packaging philosophy to find out if a similar package will be offered in future years. If an institutional grant is offered, is it restricted to the first year or can it be renewed? If the school's web site or publications don't provide this information, call or visit the school's financial aid office. 8 8 N.A.S.F.A.A. Cost of Attendance Comparison Worksheet The actual cost of attendance (COA) is unique for each student at each school. Some expenses are fixed (tuition and fees) and others are influenced and controlled by the student's lifestyle (housing and personal expenses). When considering the award package from a particular school, or in comparing packages from multiple schools, you can use this worksheet to compare your cost expectations to each school's estimated total COA. The goal of this comparison is to better decide if the difference is manageable given the resources available to your family and the aid being offered by each school. For more than 35 years, SREB s Academic Common Market has enabled students to pursue out-of-state college degrees at in-state tuition rates, through agreements among the states and colleges and universities. Search from more than 1,900 undergraduate and graduate degree programs in 15 states. Through the Academic Common Market, a program coordinated by the Southern Regional Education Board (SREB), students in 15 southern states can enroll in selected programs at public institutions in other states and pay in-state tuition. The Academic Common Market is not a distance learning program; students must attend classes at the college or university. The states participating in the Academic Common Market are Alabama, Arkansas, Delaware, Florida, Georgia, Kentucky, Louisiana, Maryland, Mississippi, Oklahoma, South Carolina, Tennessee, Texas, Virginia and West Virginia. 12 12 Scholarship Title: American Public University System (APUS) Academic Scholarship Established By: Criteria Amount: Deadline: Process: American Public University System, Inc. 1) You must be a Jefferson County High School senior. 2) You must be accepted by a two or four year accredited college, university, or technical/vocational school. 3) Your GPA will be assessed, but it is not the deciding factor in the final selection. 4) Financial need is considered, but it is not a requirement for eligibility. APUS will award two scholarships at each Jefferson County High School for the 2018 graduation year. Applications will be accepted starting February 1, 2018 through March 15, ) Compose a letter (minimum one page) to the selection committee explaining why you are requesting the scholarship, including what degree/program you intend to pursue. 2) Include your name, address, and telephone number. 3) Include a copy of your complete high school transcript. 4) Include documentation validating your acceptance to an accredited institution. 5) Provide information on extracurricular activities in your school and community. 6) Include a minimum of three letters of recommendation. 7) Include any work experience. 8) Send your completed application packet to: American Public University System 393 N. It is crucial that you submit a complete application package. If your application is incomplete, or if required documents are excluded, your application may not be considered. Shepherd University admission standard (each year) 2. Maintain a "C" or better average in business education 3. Two scholarships will be awarded, one to each high school. To be eligible to apply students must be on track to graduate in May 2017 and be residents of Jefferson County. 4. Demonstrate high moral and personal qualities The students selected must enroll and attend Shepherd University on a full-time basis. The scholarship will be paid directly to the students accounts in the amount of $ at the beginning of the each semester, not exceeding $ per year. The scholarship is renewable for up to four years. April 11 th Applicants will submit a letter of application to their school counselor stating his/her career and college intentions and qualifications for this award. Financial need may be considered, but is not the only criteria in applying for this scholarship. Final selection will be made by the School Scholarship Committee. 15 15 The Barber Dechaine French Studies Scholarship For Washington High School Student: Washington High School 300 Washington Patriots Drive Charles Town, WV Criteria for Eligibility: Must have completed or be enrolled in French 3 at the time of application. Must have maintained a B or higher average in French in current and all previous semesters as well as overall GPA of 3.0 Must be admitted to a 2 or 4-year college, in or out of state Must be continuing with French studies at the college level as a major, minor or area of interest. Amount of Award: Minimum of $ with a maximum of $1,000 to any one qualified candidate Multiple recipients may be selected within any given award period Funds will be dispersed directly to the college or university upon receiving notification on recipient s enrollment. Must be a United States Citizen and a Jefferson County resident to apply. Open to both high schools. 2. Accepted by an accredited college/university or technical/vocational school in West Virginia or within a 100-mile radius of Jefferson County 3. Grade point average of a B (no lower than a 2.5), which must also be maintained in college 4. Financial need is considered but is not criteria for applying 5. Student must reapply each semester by showing transcript of grades. New applicant reviewed when previous student is finished with current degree or fails to reapply. Scholarship will be paid direct to the school of choice. If not used, it will become available to another student. 6. Demonstrate high morals and personal qualities 7. Keep in contact with scholarship committee during time of receiving scholarship $1, per year; $500 per semester for 2 or 4 years (depending on the course of study) Scholarship reviewed after each semester. Graduate from a high school located in Jefferson County, West Virginia 2. Be accepted by an accredited vocational/technical school, college or university in the U.S.A. 3. Have an overall GPA of 2.5 or higher 4. Demonstrate good citizenship and financial need 5. Any applicant receiving a full paid scholarship from another source will become ineligible for this scholarship 1. More than one scholarship will be offered 2. Preference will be given to students whose ancestors attended Charles Town High School 3. Financial need and good citizenship will be considered 4. 20 20 Charles Town Lions Club Scholarship Application The Charles Town Lions Club is steep in tradition and dedicated to serving those in our community in need. We Serve is not only our motto, but a definition and a philosophy. As such the Lions Club is seeking applicants who are also community and civic minded. While understanding the importance to achieve academic excellence, the Lions Club of Charles Town equally feels that is also important to give back to their community. That being said, this scholarship has been established by the local club to assist students who graduate from a high school in the Jefferson County District. Award Criteria: To qualify, candidates must be a Jefferson or Washington High School Senior and have been accepted to a two or four year accredited college. Candidates must have a minimum 2.25 GPA. All applicants must fill out an application for the Charles Town Lions Club Scholarship. This involves submitting an essay, letters of recommendation, participating in community service activities. The Lions Scholarship Review Team will ensure that a qualified candidate is a recipient of the prestigious award. The recipients of this award will be selected without regard to sex, race, religion, or creed. Deadline: All Applications must be submitted to your Guidance Department by April 1, Amount of Award: One-time award $1, Number of Awards Per Year: The Charles Town Lions Club will award two scholarships as determined by the Scholarship Committee. Scholarships are $1, for one year and are non-renewable. Scholarships will be paid in two installments of $500: one for the Fall semester and one for the Spring semester. Checks will be made payable to the school and in the recipient s name and given to the recipient after proof of college registration has been received. Selection criteria: Complete the application available in the Guidance Office for scholarship consideration. One-page TYPED essay describing why he or she is deserving of this scholarship.. Two letters of recommendation one of, which is written by a school official or organization/civic leader who are aware of the applicant s potential for college success. The other recommendation can be a character or a community-involvement reference. List all community and extracurricular activities with which you were involved over the past four years. [Please provide a point of contact and a phone number for verification of your involvement in all listed activities.] Student must complete a minimum 20 hours of community service. 8 hours must be served volunteering/participating in Lions Club activities. (Contact Tanya (304) to inquire on Volunteering.) Financial need is a strong consideration when awarding the Charles Town Lions Club Scholarship. The applicant must demonstrate a financial need as a qualifying factor for this scholarship.

21 21 Chesapeake District Civitan Scholarship The Chesapeake District Civitan Scholarship program awards scholarship to students considering majoring in a variety of fields including, but not limited to, Special Education, Medical, Theology, Education or Vocational Training. The Civitan Club of Martinsburg has been a sponsor of the scholarship for 31 years. They have awarded over $86, to students in the Eastern Panhandle. Amount: Scholarship awards range from $1000 to $1500 based on the application. Deadline: Applications must be received NO LATER than February 15 th. Please allow adequate time for mail delivery. 22 Scholarship Title: Class of 1981 Memorial Scholarship 22 Established by: JHS Class of 1981 Criteria: 1. Graduating Senior from Jefferson High School and graduating Senior from Washington High School planning to attend any post-secondary school 2. Applicant must be accepted to a post-secondary school such as a 4-year college, 2-year college, or vocational school and provide documentation of acceptance. Amount: Varies Deadline: April 3 Application Process: Address a letter to your counselor containing the following information: 1. Information about your school/community services 2. Career plans 3. Interests 4. Post-secondary school choice Also include a copy of your transcript, your attendance and discipline records and any other facts that you feel would identify you as a worthy recipient of this award. Include at least two letters of recommendation. These may be from school staff members or community leaders. Letters may not come from relatives. Selection Process: Recipient selected by the high school scholarship committee. Please submit thee copies or your application.

23 23 Dewey McDonald Foundation Scholarship The Dewey McDonald Foundation is honored to give back to our local youth in many ways. Our scholarship is one of those opportunities. Scholarship approval is based on certain criteria, and each of these must be met completely to be considered eligible. One scholarship will be awarded to a male athlete and on female athlete per academic year; per county. Be a resident of Jefferson County, WV Be a current enrolled student in the Jefferson County, WV school system. (Must provide proof of current school registration, dated and signed by a WHS School Counselor.) Must maintain a GPA of 3.0, with no major disciplinary actions taken during enrollment of current school year. Must be active in sports, either High School or Team Leagues. You must have a referral letter from your coach, if only on League Teams. Must have at least 4 hours of Community Service. (Also required by the Board of Education, and hours must be in the system by November 1 st of each school year. This may be included on a transcript.) Must provide the Dewey McDonald Foundation a short essay stating your objective after high school and why you believe you should be eligible to receive this scholarship. 25 Eastern Panhandle Board of REALTORS Annual Scholarship Program 25 Rules & Requirements Scholarship recipient must be a graduating senior from Jefferson, Berkeley or Morgan County High School. The amount of each scholarship to be awarded shall be $ This scholarship may be used for tuition/educational expenses in any College, University or Tech School. A scholarship recipient may accept other scholarships or other financial aid or loans. The scholarship check will be made payable to the recipient and institution. 31 Scholarship Guidelines The Eastern West Virginia Community Foundation administers scholarship funds for Jefferson, Berkeley and Morgan county residents pursuing education in a post-secondary setting. Each scholarship fund has its own selection criteria, which is used to evaluate and determine scholarship awards. Please refer to the 2018 Scholarship Criteria (available on our website: for a list of scholarship funds and their specific criteria. The Community Foundation uses this application for all scholarship funds, unless otherwise specified in the 2018 Scholarship Criteria. 49 49 Jefferson County Schools Service Personnel Association Mary Diane Franks Memorial Scholarship The Jefferson County Schools Service Personnel Association has created a scholarship to honor the memory of fellow associate and past President of the Association, Mary Diane Franks. Diane died on September 25, 2004, was President of the JCSSPA for numerous years, and took pride in her job. She was a shining example to all. For her great contribution and dedication to service personnel and our Association, we created this scholarship to honor her memory. GUIDELINES 1. The recipient of this award shall be one graduating senior of Jefferson High School and one graduating senior of Washington High School. Also, recipient must be a child or grandchild of an active Jefferson County Schools Service Personnel Association member. 2. The recipient of the award shall be nominated annually from applications submitted to the Guidance Office at Jefferson or Washington High School, who will in turn submit them to the Jefferson County Schools Service Personnel Education Committee. 3. The recipient must plan to become a full time student at an accredited West Virginia college or university, and attach a Letter of Acceptance as part of their application. 4. The Scholarship Committee will interview the final candidates. RENEWAL: The scholarship is renewable annually for up to three additional consecutive years. Full-time enrollment and maintenance of a minimum 2.5 GPA for each semester are required to renew the scholarship. The student is required to fill out a new FASFA every year and send an official transcript to the Program Director at their expense. 76 76 University Healthcare Foundation Judge Gray Silver, Jr. University Healthcare Foundation Health Professions Scholarship Program PURPOSE The University Healthcare Foundation has established a health career scholarship program in order to interest area students (graduating seniors from high schools in Berkeley, Jefferson and Morgan Counties, and other area private schools) in pursuing preparation and advancement into the health professions. It is hoped, additionally, that scholarship recipients will consider initiating their health careers with one of the University Healthcare affiliate organizations. The scholarship guideline is as follows: 1. The $ per year will be given to assist qualified and selected high school seniors attending college in a health related curriculum (curriculum priorities for 2016 include Physician, Registered Nurse, Radiologic Technologist, Physical Therapist, Pharmacist, Respiratory Therapist, and Medical Technologist) 2. Priority for scholarship aid will be given to the family members of University Healthcare affiliate employees. Area students not associated with University Healthcare may also apply. 3. A selection committee will establish the process for the annual solicitation of candidates, who will be selected based on field of study, academic achievements, extracurricular activities, references and financial need. Applications, available in the Guidance Office, must be received by the committee no later than March 31, The committee will be made up of representatives of the University Healthcare and the University Healthcare Foundation Board of Directors. 4.
