3 Medication Management and Use standards (MMU) Organization and Management 1. Medication use in the hospital is organized to meet patient needs, complies with applicable laws and regulations, and is under the direction and supervision of a licensed pharmacist or other qualified professional To ensure efficient and effective medication management and use, the hospital conducts a systems review at least once a year. - How well the system is working related to o the selection and procurement of medications; o storage; o ordering and transcribing; o preparing and dispensing; and o administration and monitoring - Monitoring resulting from any changes in the formulary, such as addition of drugs o Monitoring of medication errors and near misses o Any education needs identified o Consideration of new evidence-based practices - The review allows hospitals to understand the need and priority of continued system improvements in quality and safety of medication use.

4 MMU standards Selection and Procurement 2. Medications for prescribing or ordering are stocked, and there is a process for medications not stocked or normally available to the hospital or for times when the pharmacy is closed 2.1. There is a method for overseeing the hospital s medication list and medication use.

Emergency medications are available, monitored, and safe when stored out of the pharmacy 3.3. The hospital has a medication recall system

13 MMU standards 4. Ordering and Transcribing 4.1. The hospital defines the elements of a complete order or prescription The hospital identifies those qualified individuals permitted to prescribe or to order medications 4.3. Medications prescribed and administered are written in the patient s record

14 MMU standards 5. Preparing and Dispensing Medications are prepared and dispensed in a safe and clean environment 5.1. Medication prescriptions or orders are reviewed for appropriateness a) the appropriateness of the drug, dose, frequency, and route of administration; b) therapeutic duplication; c) real or potential allergies or sensitivities; d) real or potential interactions between the medication and other medications or food; e) variation from hospital criteria for use; f) patient s weight and other physiological information; and g) other contraindications A system is used to dispense medications in the right dose to the right patient at the right time.

Administration The hospital identifies those qualified individuals permitted to administer medications Medication administration includes a process to verify the medication is correct based on the medication prescription or order. a) medication with the prescription or order; b) time and frequency of administration with the prescription or order; c) dosage amount with the prescription or order; d) route of administration with the prescription or order; and e) identity of the patient (scored at IPSG.1, ME 3) Policies and procedures govern medications brought into the hospital for patient self administration or assamples. 7. Monitoring Medication effects on patients are monitored 7.1. The hospital establishes and implements a process for reporting and acting on medication errors and near misses
