2 Director of Nursing for Long Term Care Facility Charge Nurse, RN Long Term Care Physical Therapist Occupational Therapist MSW for The DON must have experience in managing organization culture change, managing diverse and talented associates, directing an overall Nursing operation, and the ability to inspire and influence teams. The DON will lead our 57 bed LTC. RN license in MD required, 5-7 years mgmt. exp. in LTC & Nursing, great communicator, possessing computer and people skills. BSN req. Provides direct nursing care to the residents. Assists w/ the day-to-day nursing activities performed by the Certified Medicine Aides and Nursing Assistants. Completes admissions, discharges, meets and consults with families and POA. MUST have RN license in the state of MD. & minimum yr. exp. Long Term Care exp. is preferred. Experience working with geriatric population is preferred. The therapists will see clients in their homes in Montgomery County, as well as on our 30 acre campus in Gaithersburg. Therapists will provide coordination of the plan of care and maintain continuity of client care with other health professionals assigned to the case. Therapists will document patient visits accurately and timely, utilizing outcome measures and other therapy related documentation tools. Candidates must have current Physical Therapy License in the state of MD. Min. year exp. Assess client s home safety, ADL independence and safety, balance, strength, AROM, PROM, vision, sensation, muscle tone, fine motor skills. Through the assessment organize and perform and clearly document clients physician ordered program for self-care training, home management, medication management, cognitive training, balance training and strength training needed to be safe and independent. The MSW provides identification of social and emotional needs of clients who are utilizing Home Care Services. The Social Worker will visit clients in their homes and work collaboratively with other disciplines (PT, OT, Nursing) within the Dept. The clients are located in Montgomery County. Pulls out and disposes of all carpeting, wall covering, cabinets, and curtain rods during renovations. Removes and replaces ceiling tiles, dry wall, light fixtures, switch plates, and receptacle covers. Works with various paints, thinners, brushes, rollers, sprayers, and other miscellaneous tools commonly used by painters as well as all necessary tools and equipment required to perform the jobs assigned. Must have 3 years of painting experience. Must have active driver s license. Performs day to day housekeeping functions as assigned, as well as specific tasks in accordance with daily work assignments. Monitors leftovers including properly stores/utilizes them and consistently documents the amount of leftovers. Provides Storekeeper with an accurate and up to date pull list of frozen items; ensures items are pulled from freezer at least 48 hours prior to serving time. One () to two () years exp. as a Cook s Helper, Grill Cook or Line Cook, preferably in a Health Care operation. Maryland Food Handler s Certificate preferred. Prepares alcoholic and non-alcoholic beverages with appropriate garnishes at catered events. Arranges bar supplies and glasses into attractive displays. May prepare and/or sell food items.
