2 About the service Craig En Goyne is situated at the bottom of Tak Ma Doon Road, a quiet residential area in Kilsyth and is registered to provide care and support for up to 48 older people. The service was registered with the Care Inspectorate in 2011 and is provided by Craig En Goyne Care Company Limited. The property is a well maintained converted and extended Victorian house consisting of three floors two of which are used by residents with a passenger lift providing access to all floors. Each floor used by residents has a lounge, dining area and pantry. All rooms have en suite facilities and residents are encouraged to bring in their own furnishings to personalise their rooms. Residents and visitors have access to well maintained gardens with ample seating areas to enjoy in the better weather. The residents guide states: "Craig En Goyne is committed to providing quality services for residents. Our aim is to ensure residents are treated with dignity and respect at all times by staff, visitors and fellow residents alike. All our staff share our philosophy of person centred care". What people told us We spoke with residents as part of the inspection. They told us: "I love it here. Always plenty of food choices and plenty to keep busy" "There is plenty to do if I want to" "There are outings in the summer" "I can go to bed and get up when I want" "I feel safe. Staff come quickly if buzzer pressed" "Staff are nice and treat me with dignity and respect". Self assessment We did not ask the service to complete a self-assessment before inspection. From this inspection we graded this service as: Quality of care and support Quality of environment Quality of staffing Quality of management and leadership not assessed not assessed What the service does well A requirement and recommendation made from two upheld complaints had been met. We found resident's health and wellbeing needs were being met. Care plans we looked at had clear guidance for staff to offer support in a consistent way which is important for residents with dementia. page 2 of 8

3 Overall the quality of care plans was good. They were written in a person centred way that detailed how residents preferred their care to be carried out. They were reviewed regularly to ensure they continued to meet resident's changing needs. If there were any concerns there was evidence health professionals were contacted for advice and guidance. Regular meetings and surveys gave residents and families opportunities to give their opinions. We looked at results of recent surveys carried out. These were very positive and showed people were mainly happy with the care delivered. There was an action plan for any issues raised. A varied activity programme had been developed and during our visit there was a variety of activities taking place. This ensured a stimulating environment which promoted positive health and wellbeing for residents. We looked at accidents and incidents and found they were well recorded. They were audited to look for any trends with actions taken to try to prevent re-occurrence. We observed staff interacted with residents in a warm friendly way. Staff were well-trained and offered support in a caring, professional manner. Staff did not rush residents if they needed assistance but let them take the time they needed. They demonstrated they knew residents well and supported them in the way they preferred. Staff had opportunities to raise issues and discuss any concerns as they had regular team meetings and annual appraisals. There was a plan to support staff further with regular supervision sessions about to start. A range of audits were carried out by the management team to ensure they monitored all areas of the home. This included medication audits and when we looked at medication we found everything as expected. What the service could do better Although the service had met the requirement around pre-admission paperwork we have made a recommendation this area continues to improve. New pre-admission paperwork had been developed however we found some areas of the pre-admission assessment that were not carried over into the care plan. (Recommendation 1) When we looked at care plans for a resident with stress and distress we found recordings of incidents could be improved as we did not get a clear picture of this persons behaviour over a period of time. (Recommendation 2) We did not find risk assessments for the use of bedrails or the rational for keeping bed rails in some cases. This meant we did not find clear evidence of why some residents had bedrails and if they were required. (Recommendation 3) We found gaps in daily oral care records. This meant it was unclear if residents had had their oral care carried out or not. We also found food recording charts could be improved to include how much food residents had eaten. For example recording showed a resident had eaten toast but not if it was one slice or two. Food charts are generally used if there is a concern over someone losing weight therefore it is important they are accurate. (Recommendation 4) page 3 of 8

4 We found induction paperwork was poor. There was lack of detail around shadow shifts such as what tasks new staff had observed and what the new staff member had been observed carrying out. We would expect clear evidence that staff were observed carrying out tasks and they were deemed competent. We would also expect to see evidence they had been supported through regular supervision. The induction should clearly show the new staff member is deemed competent.
