5 BACKGROUND Respite care is an essential part of the overall support provided to carers and those helping to sustain the caring relationship, enabling carers to have a life alongside the caring role, promoting health and wellbeing and preventing crises. Respite care encompasses a wide range of different short term services. The common factor is not what service is provided but its purpose to provide a break which is a positive experience for the person with care needs and the carer. Respite can be categorised as indirect respite and direct respite. Care Facilities Built Design Support Network for Relatives The Design Team at Frankham has been involved with respite and residential care work for many years and members originate from different backgrounds in healthcare design. There are a number of key areas of design which contribute to the overall design of respite care work and are critical to the delivery of the service. Achieving good design requires careful planning from briefing stage and ongoing communication with users and stakeholders throughout the process. 7 STANDARDS Anyone who undertakes a caring role for a friend or relative who is an older person or has a learning or physical disability or suffers a mental health problem may be able to access respite care services. Respite needs are identified via an assessment under the NHS and Community Care Act The duty on social services to provide or arrange services is triggered only for those people with eligible needs, that is needs above the threshold for services line. When a Community Care Assessment is carried out, the Authority makes a decision about whether to provide support or not to individuals. Respite is crucial in enabling many carers and service users to protect their health, prevent crises and continue living at home. Decisions about provision will form a central element of local strategic planning for respite. Frankham s design team will provide accommodation to the right standards, ensuring provision of services takes place primarily under a variety of regulations including: The National Assistance Act 1948, Section 21 and Section 29 The Chronically Sick and Disabled Persons Act 1970, Section 2 The Health Services and Public Health Act 1968, Section 45 The National Health Service Act 1977, Section 21 (Amended 2006) Mental Health Act 1983, Section 117 (Amended 2007) Disability Discrimination Act 1995 Carers and Disabled Children Act 2000 Mental Capacity Act 2005 Safeguarding Vulnerable Groups Act 2006 Health and Social Care Act 2008 Compliance guidance criteria of CQC (2010) 7

9 TYPES OF RESPITE PROVISION Traditional models of Respite Care (such as day care and institutional Respite Care) have been supplemented with more innovative models including; in-home respite, host family respite, home day care and outward bound breaks. Holiday services may provide additional support so that the carer can choose to reduce their involvement in caring during the break. Modern themes of respite care encourage communication with careful, sensitive, interactional dialogue, observational skills and authentic contact, including attentive listening. As well as Therapeutic alliance and Emergency Home Respite services. Frankham s designs are based around the requirements of: Taking account of the information and support from stakeholders. Key objectives and aspirations are at the forefront of design development. Information about the range of services: eligibility criteria and their cost is made available at all times. Solutions provided in a range of settings: residential care, nursing homes, supported housing, day care, and individuals own home. 10 SHORT BREAK Ade Adepitan, Short Break Centre, Brent Client: London Borough of Brent Contract value: 2.1 million Start date: July 2009 End date: 2013 Disciplines: Architecture, Interior Design, Mechanical and Electrical, Structural Engineering, CDM, Building Surveying, Employer s Agent, Topographical Surveying This is a new centre of excellence in Brent for local children and young people aged from five to eighteen, with profound and multiple learning difficulties, complex health needs, physical disabilities and autistic spectrum disorders. They support young people to achieve maximum mobility and independence and provide opportunities to make friendships through social and community events whereby the family might benefit from using the centre. The centre has eight beds in total which includes one bed which can be used on an emergency basis. The Short Break Centre offers a range of activities and leisure options taking into account the age, ability and preferences of children and young people having a short break. Emphasis is placed on engaging children in activities to improve their quality of life, development of communication, social and independence skills as well as their physical and emotional well being. The building is divided into two wings. Each wing has been specially designed and equipped. Four bedrooms in the Indika Wing offer suitable facilities for young people with mobility needs including those young people who are wheelchair dependent. This part of the building is equipped with appropriate tracking hoists. The other part of the building, Gabriel Wing, offers four bed rooms that are specially designed to meet the needs of children and young people with autistic spectrum disorders. Users of 1 Clement Avenue, which the new centre replaces, voted for it to be named after the Paralympic wheelchair basketball medallist, Ade Adepitan. Emphasis is placed on engaging children in activities to improve their quality of life, development of communication, social and independence skills as well as their physical and emotional well being... 10

12 resource Centre Sandbanks Resource Centre, Feltham Client: London Borough of Hounslow Contract Value: 8 million Start date: October 2010 End date: December 2012 Disciplines: Architecture, Mechanical and Electrical, Structural Engineering, CDM, Employer s Agent, Frankham Consultancy Group Ltd were appointed as part of the Design Team to provide full CDM Co-ordinator services for the construction of Sandbanks Resource Centre for Older Persons. The project involved the development of a residential care facility providing high quality accommodation and care for the elderly. The construction was carried out in two phases to ensure the continuous provision of care. The Centre comprises 60 single en-suite showered bedrooms located on ground, first and second floor levels. The Centre also incorporates secure landscaped gardens and activity spaces, dining areas, assisted bathing and showering facilities and a hairdressing saloon. Internet access is available throughout the building. Ancillary accommodation includes a dedicated kitchen, laundry and staff facilities. 12

14 Care Home Widmore Road, Bromley Client: London Borough of Bromley Contract Value: 900,000 Start date: February 2011 End date: June 2011 Disciplines: Mechanical and Electrical, Structural Engineering Frankham Consultancy Group were commissioned to provide a full multidisciplinary service, to re-plan and refurbish an existing care facility to provide 12 bedroom en-suite accommodation for adults with learning disabilities on behalf of the London Borough of Bromley. Frankhams were employed to provide full design services from inception to completion, in Architecture, Structural Engineering, Mechanical & Electrical Services and Quantity Surveying. Though the existing property has been used as a care facility in the past, spatial requirements and regulations meant significant changes were required to accommodate en-suite facilities, as well as rationalise the existing layout. Additionally a single storey lift and ceiling hoists added to promote ease of access for all. Significant internal alterations presented the design team with challenges to maximise the accommodation within the confines of the existing structural parameters. While new Mechanical and Electrical equipment increased the performance and efficiency of the building without compromising accommodation. A tight construction programme led by client requirements for early occupation, meant the multi-discipline team had to work efficiently with the contractor to provide a quality building, to a high specification and on budget. 14

16 Care home Greenwood, Dalmeny Road, Bexhill-on-sea Client: East Sussex County Council Contract Value: 250,000 Start date: 2009 End date: 2011 Disciplines: Architecture, Structural Engineering Greenwood is registered to provide accommodation for up to sixteen adults with a learning disability. It is part of services provided by East Sussex Social Services and its purpose is to provide respite (short stays) care. There is a manager who has day-today control. The premise is a two storey purpose built unit situated in a residential area. There are eighteen bedrooms all with a wash hand basin and some with over head hoist tracking. People have access to seven bathrooms six of which are assisted or have a walk in shower, two large lounges, a smaller lounge, a dining room and art and craft or quiet room. There is also small kitchen which could be used for independence training. In addition there is an upstairs separate flat with lounge, kitchen/diner, bathroom and bedroom. There is a garden with lawn, patio area and flower bed and also an enclosed courtyard with shrubs. Both areas have seating. The home is suitable for wheelchair access on the ground floor. There is limited parking in the front of the premise and additional on street parking nearby. The home is situated approximately 30 minutes walk from the centre of Bexhill and its town centre amenities. The staff compliment consists of two DPS managers, duty officers, senior care officers and care officers. There are two wake night members of staff and one sleeping in. 16
