3 Aims and Objectives Court House Care strives to offer the highest standards of care with a personal touch and has recently undergone some major refurbishment with development of the gardens to enhance the experience of staying here. Residents are accepted for long term, short term, respite care, day care and holiday stays, should space be available and we feel confident that we can fully meet their needs. Our aim is to help our residents enjoy life. We do this by: Providing a homely, relaxed environment within which individuals are supported to achieve personal goals. To promote and maximise independence through person centred support. To enable individuals to participate as fully as possible in all aspects of their home environment and within the community. To encourage and enable individuals to make informed choices To encourage and enable individuals in maintaining existing relationships with family and friends along with developing new friendships. Our Values: INDIVIDUAL RIGHTS: We will respect and promote the rights of each individual within our care. We will promote and enable each individual we support to be active citizens of society, promoting their own individual qualities. PRIVACY: We provide personalised assistance with personal care whilst promoting and maximizing independence. Individuals we support are encouraged to utilise their own personal space. Care and support staff recognise the importance of promoting privacy. DIGNITY: We undertake to treat the individuals physical and social needs with the utmost respect and dignity, supporting their beliefs or choices in a tactful and friendly manner. INDEPENDENCE: We aim to maintain independence as much as possible, to enable individuals to make choices and to think and act for themselves. We aim to foster a sense of doing with. rather than doing for.. We enable the individuals we support to live their lives in their own way within the community of their home. 2 of 17

4 SECURITY: We aim to maintain a safe environment where the individuals we support are protected and can feel secure to live their lives as independently as possible. CHOICE: We aim to promote and provide practical assistance to enable the individuals we support to make and express choices where ever possible, this being a key to their independence, dignity and self-respect. Whilst helping to limit unreasonable risks, we also acknowledge that some risk taking may be desirable to add value to individual s lives. FULFILMENT: We continually support and encourage the individuals we support to identify, work towards and achieve their aspirations within their capabilities and to set new goals as their needs change. 5 Philosophy of Care Court House aims to provide residents with a supportive homely environment, in which to maintain their independence as much as is possible and enhance the quality of their lives. This support is intended to meet the needs and aims of the resident as jointly identified by staff at Court House Care, the individual and their representatives. Carers strive to promote and maintain the dignity, individuality and privacy of all residents within a supportive, caring atmosphere using person-centred care and support. This is sensitive to the individual s ever changing and developing needs. Such needs may be medical/therapeutic (for physical and mental welfare), cultural, psychological, spiritual, emotional and social. Each individual resident is seen as an individual and will be actively encouraged and supported to participate in the development and review of their care plans. A cornerstone to the Philosophy of Care is the continual training of all staff in order to provide better person centred support. We have a continuous training programme to ensure that these high standards are maintained in line with the latest developments in care practices. We continuously monitor against the appropriate legislation as regulated by the Care Quality Commission (CQC) and then aim to exceed these standards. Lorna has had many years experience in setting up and running businesses and has supported others in developing social businesses and social enterprises. As the proprietors, they continue to be focused on enhancing the experience the residents have whilst in the care of Court House. 8 Staffing Core staffing levels are maintained at above industry norms, with currently a Manager, a Finance & QA Manager, 6 Senior Care Assistants, 19 Care Assistants, 2 cooks, an Activity Co-ordinator, 2 apprentices, 3 House Keepers and 1 Handyman / Gardener. Staffing may increase at times when residents need additional support. The Home s staff is selected for their qualities of reliability, integrity, skill, friendliness, care, knowledge and professionalism. They are carefully screened and references are always checked thoroughly. During induction all care staff are trained in line with the new Care Certificate based on 15 standards which individuals need to complete in full before they can be awarded their certificate. Existing staff are also undertaking this training as a refresher to their existing qualifications. The Home has a programme of NVQ/QCF training with both on-line and in-house training. Regular fire safety training and manual handling is carried out within the home. Courses include:- SOVA Manual Handling Fire Health and Safety First Aid Dementia awareness And many other specialist training specific to the individual needs of residents as they are identified Carers come from a variety of backgrounds and have a wide range of skills and abilities. Ongoing training and development is linked to the needs of the people who use the service and the services aims. Team and individual training needs are identified via team meetings, supervisions and appraisals. Court House supports staff with both in-house and external training opportunities. The Qualifications and Credit Framework (QCF) training is encouraged for all staff. Court House currently has nearly 100 percent of its staff either working towards or having achieved these standards, with almost 50% having achieved QCF level III or above and 4 members of our team having reached level 4 or 5. We also have two apprentices currently being supported into the care sector as a further commitment to enhancing the care sector workforce. This is significantly above the Sector Skills Council Workforce targets. 7 of 17

9 Accommodation Registration Court House (main home) is registered for up to 23 people over the age of 65 years. Court House Cottage is registered for an additional 6 people giving us a total registration of 29. Main House The main house has 20 bedrooms, mostly single rooms with en-suite facilities, with the option of twin bedded rooms for those couples who might prefer to share. Downstairs: Six Single rooms with en-suite bathroom/ wet rooms, one twin with en-suite bathroom, four singles with en-.suite facilities. One standard room with hand basin. Upstairs: Six single rooms with en-suite facilities, one twin with en-suite facilities and one standard single rooms with hand basin. All rooms have a secure lockable cupboard for the residents own use, and suitable furniture should they not wish to bring their own. Social Rooms: There are two lounges and a dining room. Residents are encouraged to use these public rooms; however, residents who choose to stay in their own rooms are free to do so. The Cottage The Cottage is designed to accommodate individuals with a desire to live in a smaller group, who may wish to live in a style closer to their previous home life. Residents of the cottage are able to receive the same level of support as in the main house, however it is hoped that they will feel able to be fully involved in the daily routines of the cottage. There are two suites on the ground floor each with their own private lounges, bedrooms, kitchenettes and wet rooms. Upstairs there are four large bedrooms with their own ensuite wet rooms. In addition, there are shared spaces including a large lounge, dining room / conservatory. A shared kitchen and laundry facilities are available where residents are supported in managing their daily lives. The cottage also boasts its own secure garden even though residents of the cottage are still able to enjoy the benefits of the grounds of the main house. Individuals we support Both Court House and the Cottage cater for individuals aged 65 years upwards with a variety of frailties including dementias. Should individuals require additional support beyond that achievable within our normal staffing resources to fully meet their needs or wishes, this would be discussed with the resident and/or family member. 8 of 17

10 Admission Following an initial enquiry, potential residents and their legal representatives are encouraged to visit Court House. An assessment will be undertaken, at home, Court House, or in hospital, to ensure we can meet the needs of the individual, in terms of their physical, mental and social capabilities. Should the potential resident and /or their representative wish to take a place, a trial period will be discussed and arrangements made. During the trial period, it is recommended that alternative home environments be retained should a return to a home be desired. Potential residents will be encouraged to be as actively involved as possible in their care plan. We actively encourage use of all of the services and facilities provided by Court House and the community at large. The services at Court House are not registered to provide nursing care and we are not able to support individuals with very complex care needs. The team are supported by local healthcare professionals including the GPs, Mental Health team and District Nurses when required. Respite service is offered should a place be available and the assessment demonstrates that we would be able to fully meet their needs. Person Centred Care Each individual is encouraged to maintain independence whilst receiving services at Court House. Whilst the service allows for varying degrees of support to be delivered, the emphasis will be for individuals to do as much for themselves as possible. Individual residents will be continually and actively encouraged to participate in the planning of their service and in providing regular feedback as to its appropriateness. Care Plans Individual care plans will be tailored to meet the differing needs of each person, based on initial and ongoing assessments and seek to promote independence and to enhance the quality of their life. Care plans outline how Court House Care will support the person in meeting their needs, wishes and perhaps most importantly to enable each individual in our care to enjoy life. Care plans will document the individual s ability to make decisions, and how they are supported to fully participate. Evidence of agreement by the individual to receive care and support is documented through the care planning process. Once developed the care plan will be regularly reviewed to ensure that it correctly reflects the resident s needs and wishes. Individuals will be supported to comment and review their own care plan using techniques which best support their individual abilities. Where appropriate a circle of support (Court House staff, family / legal representatives and other healthcare professionals) would be used to enable residents who may lack capacity to engage in the process to review the plan. Significant changes to the care plan would be made and these would be agreed by the individual where possible, their circle of support and the Registered Manager. All amendments to the care plan will highlighted to senior staff and then cascaded to the entire team. 9 of 17

11 Key working Each individual at Court House Care will be allocated a key worker who will be primarily responsible for the co-ordination of the Care Plan with the individual and acting as advocate. Efforts are made to ensure that individuals personal preferences are considered regarding the allocation of key-worker. Changes in key-worker may be made from time to time at the request of the individual or staff member, or as a result of changes in staffing or service needs. Service Reviews Reviews within Court House are undertaken on a continual basis in an informal manner between relatives, the resident concerned if they have capacity and the manager. An annual more formal service review also takes place. The Review will discuss all aspects of the individual s needs and whether the service is appropriate. In addition to Service Reviews, multi disciplinary meetings may be held for those residents who lack capacity and significant decisions relating to their care need to be made, these would normally include their family members, their GP or other NHS representative, the registered manager and possibly other experts that could have a valid input to the decision being made. Financial Arrangements and Fees We are committed to providing value for money within our comprehensive and caring and supportive service. The fees charged are dependent on: 1. the type of room offered, and 2. the type of support package and needs of the individual resident. Basic fees will be confirmed once the initial assessment has been carried out and these are subject to review. Where a higher level of care is noted in the initial assessment period, fees would be agreed during that period before permanent placement is confirmed. Some individuals may require additional support and staffing resources to fully meet their needs. Where necessary, this is negotiated at the earliest possible time with the relevant parties. Social Activities, Hobbies and Leisure We try to make it possible for our residents to live their lives as fully as possible. As part of the assessment process we encourage potential residents to share with us as much information as possible about their social, cultural and leisure interests as a basis for helping them both settle in quickly and enjoy the experience of living at Court House. We aim to help residents enjoy as wide a range of individual and group activities both inside and outside the home, to carry on with existing hobbies, pursuits and relationships as well as explore new hobbies and experiences. We have a wide range of activities available. We hope that friendships among residents will develop and that residents will enjoy being part of the community at Court House but there is no requirement for any residents to join in any of the communal social activities. There are two lounges; an activity lounge and the quiet lounge, the latter being ideal for those who enjoy reading. Delivery of daily newspapers is easily arranged. Residents are supported to access social and recreational interests within the local community such as visiting the local café or garden centre. Should assistance be needed to undertake these activities Court House will make every effort to assist although there may be additional charges to cover any costs incurred. We try to ensure that the home is a real part of the local community and encourage visits from the Church, local school, voluntary organisations and students. Family and Friends Each person who lives at Court House and the Cottage is encouraged to retain and develop as much as possible contact with friends and relatives but residents can choose whom they see and when and where. If a resident wishes their friends and relative are welcome to visit at any time convenient to the resident. Some consideration should be given to any activities that the person might have made a commitment to 11 of 17

13 when arranging visits. Additionally, they are asked to respect that they are living within a communal setting and noise levels should be limited if visits are late in the evening. There are currently no facilities for friends or relatives to be accommodated overnight at Court House or the Cottage. Pets Whilst we acknowledge the fact that many people have pets for company during their lifetime and that they may wish to bring an animal with them when they move, the management has a responsibility to all the residents with regard to Health and Safety. This is not to say that we do not permit pets. The Manager will treat each case dependent on need and the resident s ability to properly care for the pet, along with the number of pets already at the Home. Many pets require access to the grounds so rooms without direct access to the outside will not normally be suitable. Please discuss with the Manager should a pet be desired. Furniture, Fittings and Personal Effects Court House Care provides fully furnished accommodation, and therefore individuals are not required to bring items of furniture into the home. However, if a person wishes to bring certain items with them this can be discussed with the Manager. A flexible approach will enable a new person moving to Court House and the Cottage to settle and adjust more easily to their new surroundings. Individuals are expected to bring their own personal effects. Meals Individuals are encouraged to choose their own meals and will be supported in making choices. Meals at Court House are prepared by experienced and skilled cooks under the direction of the Manager. Within the cottage all individuals are positively encouraged to participate in the preparation, serving and clearing away of meals depending on their level of ability. Special dietary requirements can usually be catered for, particularly taking into account health, ethnic and religious considerations. Alternatives to menu choices on offer will also be available. Laundry A full laundry service is available for individual s personal laundry within the main house. Laundry facilities are available within the cottage for individuals to undertake their own laundry with supervision and are encouraged and supported to pack away laundry, to keep tidy drawers and wardrobes. Religious and Political Activities Individuals are supported through the Person Centred Support Planning to identify and express a religious beliefs or political persuasion if they so wish. Individuals are free to practise their own religion and follow their own political persuasion and will be appropriately supported in doing so, provided their activities do not impinge upon others living in the home. 12 of 17

14 Privacy and Dignity Individuals living at Court House Care are encouraged to regard it as their home. Each person s bedroom is regarded as private space which will not be entered without their permission, except in an emergency, or when conditions give cause for concern. Individual s dignity will be respected at all times and any support with personal care tasks will only be offered in private. Smoking and Alcohol It is recognised that individuals we support who have capacity are able to choose to purchase and consume alcohol if they so wish. Individuals are supported through health action planning to ensure that alcohol consumption does not have an adverse effect on health and wellbeing such as interactions with medication. Individuals who lack capacity to make decisions on the effects on their health on the consuming alcohol are supported through a best interest process by people who know them well including care professions such as their GP to assess the effect of consuming alcohol and a person centred health action plan is created to support this activity. Court House Care has a no smoking policy. Residents or their family members, relatives or friends who wish to smoke will be requested not to smoke within the home. Fire Safety The Home has a modern Fire Alarm System fitted, which is fully compliant with current regulations. Staff are instructed in Fire Prevention and Drills. This includes use of the Homes fire appliances, evacuation, muster points, raising the alarm, etc. Residents are informed of the emergency procedure during admission. All staff undertake full Fire training every 6 months. Night staff have additional training conducted within the Home. All fire systems and alarms are tested weekly by a designated person, and tested by an authorised third party every 6 months. Records are kept of all tests. All fire-fighting equipment will be checked annually by a qualified fire extinguisher maintenance engineer. Wherever possible, furniture, fixtures and fittings are made of fire-resistant/ fire-retardant fabrics and materials. Medication 13 of 17

15 If a resident chooses to self-medicate, following a capacity assessment, then all help and advice will be given to support this choice. For residents who choose not to self-medicate or are deemed not to the capacity to do this safely, medications will be managed by fully trained staff and dispensed in accordance with the doctor s instructions Telephone & Wifi Residents can request to use the Home s telephone in private at any time. Incoming calls can also be taken in the same way. Residents may have their own private line or mobile at their own expense. Wifi is available free of charge through the main house and the cottage. Leaving or Temporarily Vacating If a resident wishes to leave Court House Care, then four weeks notice must be given of this intention, or four weeks fees paid in lieu of notice. These conditions are waived during the four-week trial period. If a resident temporarily moves out of the Home (e.g. to receive hospital treatment) the bed will be retained unless otherwise agreed however normal fees would apply. If due to changing levels of ability which would mean that Court House could no longer meet the resident s needs, and that we therefore have to ask the resident to leave, these condition may be varied. In the event of a bereavement Court House Care reduces the notice period to two weeks payment of fees. Bereavement In the unfortunate event of bereavement, the family can expect every possible support and consolation from staff. Whereas funeral arrangements are usually made by the next of kin, the Manager can be relied upon to assist and explain what is required. Quality Assurance Within the Court House and the Cottage there are various systems which ensure that close monitoring is maintained on all of the Home s services and procedures. Quality assurance is regularly undertaken to ensure that all areas of the home and service are maintained to the desired standard. An important part of our quality programme is to involve the residents and their relatives. We regularly ask for comments on the Home, the staff and services we provide and value your feedback. Complaints 14 of 17

16 If as a resident, relative or visitor, you feel that there is cause for complaint please raise the matter with the Registered Manager, either in person, phone, in writing or by . All complaints will be responded to in the form the complaint is made in.
