2 Where the client is at the centre of everything we do These include: Wide Choice of Support Options Supported Living Supported Housing Day Centre Residential Care Nursing Care Floating Outreach Choice and Opportunity People who use our services are as involved where possible in the development of their own care plans, and are supported in making informed choices about all aspects of their lives using a person centred approach. Clients are encouraged to be as involved in running the organisation as they want to be. For example, St Anne s Board (the organisation s governing body) has a strong client membership and clients are involved in a variety of ways in the recruitment of all staff who will support them. There are a number of client forums and clients also help to give feedback in surveys and as part of our internal inspections an visits. We are constantly looking at ways clients can be more involved - if this is what they want. St Anne s avoids a 'one size fits all' approach but rather treats everyone as the individual they are.

4 Fernlea Nursing Service About Fernlea Fernlea was established in 1998 and is a registered Nursing Home for people with Learning Disabilities. The Care Quality Commission has assessed Fernlea as providing 2 star good quality outcomes. We are continuing to build upon our success by promoting choice, rights, dignity and respect on a person centred approach for long term clients aged between 18 and 65 years of age promoting equality and diversity. Staff working in the home ensure that clients are always at the centre of everything that we do. The house has been purpose built on one level to enable access to all areas for clients. Fernlea is a 2 sided integrated bungalow supporting those who challenge the service and those with complex health needs. Each side has 3 to 4 bedrooms, fitted with a variety of adaptation to meet peoples support needs including tracking aid and hoists. Each side also has a shower room, bathroom, kitchen/diner living area, and garden space. Each client has their individual bedroom decorated to their own individual tastes. Two bedrooms in the part of the service for people with complex health needs have direct access from their bedroom to the bathroom. The staff team consists of nurses and support assistants providing 24 hour support. Each client has their own primary nurse and key worker who co-ordinate their care. Clients are supported to access community services and the service works with a variety of agencies to promote a multi disciplinary and holistic approach to care. The home also has access to 2 vehicles which are adapted to ensure all clients can access community facilities.

5 Oxfield Court Nursing Service About Oxfield Court Oxfield Court is a registered nursing home providing support for up to 24 people with varying degrees of learning disabilities. Some of the people have complex needs and some may challenge the service. The people living at Oxfield Court live in four bungalows. A combination of highly skilled registered nurses and support staff are provided with specialised training to meet the needs of those who live there. 24 hour care is provided. The home has attained Gold Standards Framework accreditation in End of Life Care. All the people living at Oxfield Court are supported to access all community services; they have support from CTLD (Community Learning Disabilities Team). To access the service you will need to complete a St Anne s application form for services. You will need to be assessed as requiring a nursing service and be referred by CTLD. Care Staff encourage a person centred approach and aim to meet the individual needs of the people living there. Staff use the person centred planning process to identify peoples needs and encourage independence, choice, respect and inclusion. People living at Oxfield Court are involved in the day to day running of the home, care and leisure. Client meetings are held at least quarterly. Various communication systems are promoted in order to meet individual needs. Activities & Social Life Community presence and participation are important to people; staff support those living at Oxfield Court to access activities and services of their choice within the local and wider community. Family links are supported, activities that people currently enjoy include: Reminiscence/Chat groups: Swimming, Pubs/meals out, Walking Bowling, Theatre, Rebound Therapy, Hydrotherapy, Concerts, Day Trips, Gateway, Cinema, Adapted Cycling, Ice Skating

6 Smithies Moor Lane Nursing Service About Smithies Moor Lane Smithies Moor Lane is a six bedded nursing home for adults with a learning disability. We provide 24 hour care and support to the people who live here. The bungalow has 6 individual bedrooms and there is level or ramped access to all areas. The home has been fitted with a variety of adapt ions to meet people s support needs including tracking, hoists and bathing equipment. There is a secure garden area to the rear of property with seating area and flower beds, which some clients help to maintain. Our aim is to provide accommodation that is a home rather than simply a house by providing a supportive environment with a fully trained staff team who ensure that people are treated with dignity and respect at all times. We encourage and support people to make choices and decisions about their lives and provide person centred support to ensure each individuals dreams and aspirations are developed into reality wherever possible. People are supported to lead active lives and to maintain and develop relationships with family and friends. The home has 2 vehicles, enabling clients to access community facilities and leisure activities. Different activities that people currently enjoy are hydrotherapy, shopping, dining out, cinema, parks, trampoline and day trips. As we are a nursing home there is a Registered Nurse (Learning Disability trained) on duty 24 hours a day and there are also support assistants who are trained to meet the needs of the individual clients.
