3 Section one: About NHS Ayrshire & Arran NHS Ayrshire & Arran is one of 14 territorial NHS Boards within NHSScotland. Ayrshire and Arran NHS Board is responsible for the protection and improvement of the local population s health and for the delivery of frontline healthcare services. The NHS Board membership consists of executive and non-executive members, and is accountable to the Cabinet Secretary for Health, Wellbeing and Sport. Our operational frontline services are provided through four distinct operational units Acute Services and the Health and Social Care Partnerships in East, North and South Ayrshire. You can find further detail on the role of the NHS Board, and our organisational structure on our website NHS Ayrshire & Arran serves a mixed rural and urban population of 376,000. We have a full range of primary and secondary clinical services, covering the mainland of Ayrshire and the islands of Arran and Cumbrae. NHS Ayrshire & Arran covers three local authority areas: East, North and South Ayrshire. There are major areas of widespread deprivation and social exclusion in both rural and urban areas. The healthcare challenge within the area is considerable: our population suffers higher than average rates of coronary heart disease, lung cancer, respiratory illnesses and premature death among males. Ongoing lifestyle issues, such as the prevalence of smoking, poor diet and lack of exercise, are key factors, as is the steadily ageing profile of the population. 5 2020 vision In September 2011, the Scottish Government set out the strategic vision for the delivery of healthcare services in Scotland. Our vision is that by 2020, everyone is able to live longer healthier lives at home, or in a homely setting. We will have a healthcare system where: we have integrated health and social care; there is a focus upon prevention, anticipation and supported self management; if hospital treatment is required, and cannot be provided in a community setting, day case treatment will be the norm; whatever the setting, care will be provided to the highest standards of safety and quality with the person at the centre of all decisions; and there will be a focus on ensuring that people get back into their home or community environment as soon as possible with minimal risk of readmission. In February 2014, Ayrshire and Arran NHS Board approved Our Health 2020 as NHS Ayrshire & Arran s health and wellbeing framework. 8 Acute services Emergency and elective hospital services are provided by our acute services, which includes inpatient, outpatient and day case care. There are two district general hospitals within Ayrshire: University Hospital Ayr and University Hospital Crosshouse. These hospitals provide a wide range of acute services: University Hospital Ayr University Hospital Ayr provides medical and surgical services on an inpatient, day case and outpatient basis. It is the main Accident and Emergency service for South Ayrshire. It provides a number of Ayrshire-wide services including Vascular Surgery, Ophthalmology and Urology. There are approximately 330 inpatient beds at Ayr. University Hospital Crosshouse University Hospital Crosshouse provides medical and surgical services on an inpatient, day case and outpatient basis. It is the main Accident and Emergency service for East and North Ayrshire. Inpatient paediatrics, the Ayrshire Maternity Unit and the main Laboratories for Ayrshire are on the Crosshouse site. There are approximately 600 inpatient beds at Crosshouse Community, mental health and learning disabilities services Following the introduction of Health and Social Integration, the operational delivery for the range of community healthcare services and mental health and learning disabilities services is through the three Partnerships in Ayrshire: East Ayrshire Health and Social Care Partnership North Ayrshire Health and Social Care Partnership South Ayrshire Health and Social Care Partnership 8

9 Community services General medical and dental services are provided throughout Ayrshire and Arran by general practitioners, dentists, community pharmacies and optometry practices. Community nurses, health visitors and Allied Health Professionals are all involved in providing care within our local communities. Out-of-hours general medical services are provided by Ayrshire Doctors on Call (ADOC) within the community. Mental health and learning disability services The majority of our inpatient mental health services, including the Intensive Psychiatric Care Unit, are provided at the newly built Woodland View facility in Irvine. Learning disability services are provided from Arrol Park in Ayr. Elderly mental health inpatient services are provided from Ayrshire Central Hospital in Irvine, and East Ayrshire Community Hospital in Cumnock. Community based services are provided throughout Ayrshire for a range of clinical groups: adults, child and adolescent, elderly and addiction services. For more information on the full range of clinical services provided by NHS Ayrshire & Arran, visit our website Section four: Investing for the future Building for better care We have invested 27.5 million to provide fit for purpose front door services for University Hospitals Ayr and Crosshouse, in order to improve emergency and urgent care for patients who come to hospital for anything other than a scheduled appointment. 9

10 University Hospital Ayr: new emergency department with resuscitation bays, high care areas and cubicles and fully integrated with the minor injury unit and NHS Ayrshire Doctors on Call (ADOC). University Hospital Crosshouse: Combined Assessment Unit being built alongside the existing emergency department which will include 35 en-suite bedrooms, as well as new patient assessment and ambulatory care areas. The unit will provide the physical environment needed to allow patients to be rapidly assessed and either discharged safely or admitted to a specialty ward for further care and treatment. Woodland View We have invested 47million providing a 206 en-suite bedroom integrated mental health and community facility, including older people s long term care and rehabilitation. This development brings together a full range of outpatient and inpatient facilities including: an outpatient consultation area to support a full range of mental health and psychology related outpatient activity; tribunal suite; therapy areas with treatment and recovery spaces that will also be used as flexible clinical accommodation for Allied Health Professions; an inpatient clinical pharmacy and dispensary service to all wards; and a staff library. 10

Ayrshire is situated in South-west of Scotland on the Firth of Clyde, and is characterised by 80 miles of varied coastline, picturesque beaches, rolling green hills and islands. Just a 30-minute drive from Glasgow city centre, Ayrshire is an ideal location for those who want to enjoy city life, with all the benefits of living in a semirural area. Ayr, Irvine and Kilmarnock are the largest towns. However, there are many rural towns, villages and communities throughout the area should you wish to relocate to Ayrshire. Whether you decide to locate to Ayrshire or a neighbouring area, you will find that property prices are more affordable than in other parts of the UK. Local educational standards are very high at primary and secondary level. However, private education is also available in the area. 14

Dimensions Based at Crosshouse Hospital the post holder has an area wide responsibility for providing advice on antimicrobials across all care settings in NHS Ayrshire & Arran (wards and clinics), and Primary care and Community settings. Responsible for the day-to-day delivery of clinical pharmacy services in antimicrobial pharmacotherapy relating to individual patients or groups of patients Undertakes extended prescribing in accordance with legislation. Supervises and trains medical, nursing and pharmacy staff and students regarding antimicrobials Represents NHS Ayrshire and Arran at local, regional and national meetings as agreed with the Principal Pharmacist Prescribing Development and Education Monitors and provides highly specialised advice on antimicrobial usage and expenditure In addition to the specific responsibilities outlined in this job description, the post holder will be expected to fulfil the entire basic pharmacist competencies required as part of registration with the General Pharmaceutical Council. The post holder will be required to act at all times in accordance with medicines and Health & Safety legislation (e.g. Medicines Act 1968, Misuse of Drugs Act 1971, Poisons Act 1972, Control of Substances Hazardous to Health Regulations 2002) and professional obligations as outlined in the Standards of conduct, ethics and performance. 4. Role of Department The aim of the pharmacy service is to assure the quality of patient care associated with the provision of treatment with medicines by: Promoting safe and cost effective prescribing taking into account the clinical needs of individual patients. Providing a wide range of direct pharmaceutical services to in-patients, out-patients, day patients and departments (hospital and community). Ensuring that all aspects of the hospital pharmacy service comply with all statuary (legal) and quality standards. Includes the Medicines Act 1968 and Misuse of Drugs Act Provides information and education about medicines to patients, carers and healthcare workers as appropriate. Ensuring that a competent workforce provides services, by providing the required level of education and training support. Supporting the work of the Drug and Therapeutics Committee and policy development for medicine use. Linking and communicating with primary care practitioners (Community Pharmacists and General Practitioners) to ensure seamless care.

Key Result Areas Planning and Organisation To lead, develop, deliver and evaluate clinical pharmacy services to patients requiring antimicrobials in line with local strategies, policies and procedures to ensure optimum patient care. To provide expert advice regarding the clinical use of antimicrobials To develop, maintain, implement and monitor antimicrobial policies and procedures in conjunction with the wider antimicrobial management team, complying with all relevant standards and legislation, in all care settings including hospital and community To identify and advise on the pharmaceutical aspects of management of risks associated with the use of antimicrobials to minimise risks to patient, carers and all healthcare staff To ensure compliance with data monitoring requirements at both national and local To assist the Principal Pharmacist Prescribing development and education in strategic service planning by horizon scanning for new or changing antimicrobial drug developments, new service opportunities, supporting the development of business cases to support changes in practice or new investment taking into account national strategies and guidelines To participate in local and where required regional and national groups involved in antimicrobial stewardship. To provide a clinical pharmaceutical lead role in the Antimicrobial Management Team and support the Principal Pharmacist Prescribing Development and Education with the work of the Antimicrobial Management Group To work collaboratively with senior pharmacy and medical staff, and the multidisciplinary team ensuring that issues related to the safe and effective use of antimicrobials in the care of patients are appropriately represented. To participate in the development and maintenance of antimicrobial policies and procedures To assist the Principal Pharmacist Prescribing Development and Education in service planning for the antimicrobial pharmacy service taking into account national strategies and guidelines. To support formulary management processes for new antimicrobials Resource Management To contribute to the budget setting process arrangements for antimicrobials and in the production of business plans and horizon scanning for new antimicrobial treatments. To monitor expenditure and drug utilisation trends in antimicrobials across the organisation highlighting areas of concern to the Principal Pharmacist Prescribing Development and Education. To implement policies and procedures for monitoring the use of antimicrobials, identifying areas for cost improvement, promoting formulary management and good prescribing practice. To implement the managed entry of new antimicrobials according to the agreed policy thus contributing to the overall expenditure on antimicrobials To provide line management to the Senior Pharmacist Antimicrobial Pharmacotherapy (Band 7) and to supervise junior clinical pharmacists and technical staff as allocated by the Principal Pharmacist Prescribing Development and Education. This includes prioritising and allocating work and tasks based on patient need

20 To ensure the security of medicinal products in all settings including wards and clinics and the main pharmacy department. Clinical Practice To maintain a regular clinical commitment, by providing a highly specialist level of clinical pharmacy input to the care of patients with particular respect to antimicrobial use, participating in consultant ward rounds and multidisciplinary team meetings as appropriate To identify opportunities for and undertake independant prescribing as appropriate, with particular focus on antimicrobial prescribing, linking with other healthcare practitioners as required. To report medication errors and participate in investigations as appropriate, to ensure that the risks associated with medicines are minimised. To improve pharmaceutical public health by identifying and preventing adverse drug reactions. Reporting suspected adverse reactions on medicines to the Medicines and Healthcare Devices Regulatory Authority To authorise the pharmaceutical aspects of unlicensed medicines particularly those used in antimicrobial therapy having evaluated the evidence for the use of the medicine. Education and Training To develop education and training relating to antimicrobial stewardship for all pharmacy and other healthcare professionals in conjunction with the Principal Pharmacist Prescribing Development and Education To participate in the development and implementation of training programmes for clinical pharmacy, to all grades of pharmacists e.g. undergraduates, pre-registration pharmacists and qualified pharmacists. To identify and address any training needs to improve medicines management in antimicrobial prescribing to support the wider education of healthcare staff e.g. medical and nursing staff To act as a tutor to those undertaking nationally recognised qualifications giving formal assessment where required. To counsel individual patients, groups of patients or carers regarding their medicines to improve understanding and optimise concordance. General To support the Principal Pharmacist Prescribing Development and Education in identifying opportunities for research and audit relating to Antimicrobial stewardship To initiate, undertake, or guide others, in audit and research within antimicrobial utilisation in line with the national standards and frameworks in order to maintain standards and develop the service e.g. pre-registration pharmacist projects, pharmacy technician projects, national and local audits. The post holder will be required to maintain a relevant pharmacy practice commitment, including dispensing and clinical pharmacy, depending on their own experience and service needs. To participate in departmental rotas and / or emergency duty according to service need, as outlined in the contract associated with this post. Equipment and Machinery Regular use of Microsoft Office computer software for and internet enquiries, word processing of reports and policies, producing spreadsheets of management information and statistics and PowerPoint for presentations to staff. Acts as key holder, performs security checks, sets and deactivates department intruder alarm when required. 7b. Systems Regular use of JAC pharmacy stock control and dispensary computer system and electronic prescribing and administration system (EPMA) to input and retrieve information in connection with clinical and dispensing practice and to produce management reports. Regular use of the NHS Ayrshire and Arran Intranet to access laboratory reports for individual patients. Regular use of the monitoring data bases 8. Assignment and Review of Work The post-holder is managed by the Principal Pharmacist Prescribing Development and Education. Works autonomously against objectives agreed with the Principal Pharmacist Prescribing Development and Education. Workload will be mainly self-generated, influenced by patient needs, service needs and local and national agendas; in particular national strategies for reducing antimicrobial resistance Accountable for own professional actions and outcomes - guided by legislation, local protocols, procedures and the local formulary. Accountable for personal decisions taken regarding individual patients in the course of managing their care. As senior antimicrobial pharmacist to plan and prioritise own workload and to manage the workload of the senior pharmacist antimicrobials (band 7) and carries our formal performance review meetings for this post The Principal Pharmacist Prescribing Development and Education carries out formal performance review meetings for this post. The job description is subject to review by mutual agreement between management and the post holder

Decisions and Judgements The post holder will be expected to exercise their own discretion on how to provide and develop the clinical pharmacy service relating to antimicrobials taking account of local and national policies. The post holder is expected to anticipate problems/needs and resolve these in a proactive independent manner Decisions made may be complex, e.g. the calculation of doses based on surface area, the use of unlicensed medicines for individual patients, where there may be limited or only anecdotal information on which to base a decision to treat. The post holder will be expected to advise and influence the Principal Pharmacist Prescribing Development and Education regarding the use of antimicrobials in accordance with published evidence eg evidence for the use of a new medicine, change in indication of an existing medicine, consultant request for and expensive medicine. The post holder is professionally accountable for his/her own actions in advising an influencing nurses, medical staff and other healthcare professionals regarding the treatmen of individual patients on a daily basis e.g. monitoring and adjusting doses for drugs with narrow therapeutic index or in patients with renal impairment. 10. Most Challenging/Difficult Parts of the Job Dealing with unpredictable patterns of work, often working to tight timescales, with frequent interruptions to ensure that each patient s medication regimen is safe and appropriate for its intended purpose and is supplied on time, where there is sometimes limited information to guide decisions Balancing the antimicrobial stewardship agenda against other competing priorities for clinical teams to ensure safe and effective use of antimicrobials. Influencing others to comply with policies and accept antimicrobial resistance as a high priority Challenging the decisions of consultant, other medical staff and practitioners from other professions to ensure that antimicrobial prescribing is evidence-based. Keeping up to date with a rapidly changing knowledge base. Developing partnerships working between primary and secondary care 11. Physical, Mental, Emotional and Environmental Demands of the Job PHYSICAL EFFORT / SKILLS: Computer keyboard skills used daily. Driving car between the different hospital sites. Lifting and handling skills used occasionally to lift and move boxes and containers. Standing for periods of time to perform checks on dispensed prescriptions. Standing for periods of time on a daily basis while participating in the multidisciplinary ward rounds. Walking between wards and the pharmacy on a daily basis. Standing or sitting in awkward positions on a daily basis for periods of time in the wards to input and retrieve clinical data at a patient s bedside, notes trolley and computer terminals. MENTAL EFFORT / SKILLS: Frequent periods of prolonged concentration required every day when reviewing

24 prescribed medicines, counselling patients, writing reports or performing checks on dispensed items where an undetected error could result in serious patient harm. Required to concentrate continuously and apply mental attention at all times. A high degree of speed, accuracy and attention to detail is expected in all duties.
