4 ER Alternative With emergency-room wait times on the rise, urgent-care facilities are finding a place on the Main Line. By Emily Riley Dr. Geoff Winkley of Doctor s Best Immediate Medical Care. In the crush of evening rush hour, on a slippery surface in a garage or in the backyard pool, the old rule holds fast: Accidents happen. Yet emergency-room wait times are increasing and so are the number of visits where patients leave before their medical evaluation. Press Ganey, a leading healthcare media agency, found that patients spent an average of four hours and seven minutes in emergency departments in 2010, a 31-minute increase since So what s a person to do when there s a non-life-threatening medical situation that needs immediate attention? The ER at the nearest hospital used to be the only option. Not anymore, now that urgentcare facilities are on the rise in our area. Nationally, their numbers have risen by 300 per year during the past two years, and they are poised to maintain that growth rate through Most doctors offices close around 5 p.m., with no availability on weekends or holidays, says Dr. Madhavi Cherukuri of Doctors Express in Downingtown. 6 of four hours. If you go to an urgent-care center, you can be in and out in as little as 30 minutes. Everything from rashes, asthma attacks and fevers to animal bites, burns and sprains are fair game at an urgent-care facility. The rise in these types of practices is directly related to the relatively poor service people receive when they re attempting to access our healthcare system, says Dr. Geoff Winkley, a board-certified emergencymedicine physician who opened Doctor s Best Immediate Medical Care in Berwyn late last year. If you have a cut, are you going to wait for hours in the ER or wait a week to see your doctor? You need someone to figure it out quickly, and we enable patients to get care for illnesses in a manner that least disrupts their lives. With nearly 20 years of experience in ER and patient care, Winkley sees about 14 patients a day with his 15-person team. He treats any manner of illnesses and injuries, all of which are typically addressed in minutes. The most serious cases are handled first, as they would be in an emergency room. The availability of resources, staff and open rooms provides a surprisingly efficient and attentive environment. I was an ER doctor for almost 20 years, so I know they have to take care of a large group of very sick people, in addition to the urgent class of patients, says Winkley. They always prioritize the sickest people first, so when you go there, you re usually sitting behind all the nursing-home infections, strokes, heart attacks and so on. It s a matter of allocation of resources, and that doesn t allow for the urgent-care class to be managed efficiently. It s worth noting that Doctor s Best accepts certain insurance plans and Medicare, though additional charges may apply above the standard co-pay. Patients without insurance have a self-pay option for an average-level consultation. The facility also has the necessary immunizations, X-rays, lab materials and paperwork for conducting all the required physicals for sports, camps, schools and the like. And while it might be easy to mistake Winkley s facility for your family doctor s office, there are crucial differences. The staff is made up of licensed physicians, registered nurses, physician s assistants, paramedics and certified radiology technicians all ER trained. We re singularly focused on the service aspect of what we do, Winkley says. If you require medicine, we can provide that easily or cost-effectively, and have it ready at your local pharmacy. If you need referrals, we make that happen with extreme ease for the patient definitely more so than what they re used to. Traditionally, homecare s reputation has been somewhat dubious. Often, a visiting nurse or other medical professional simply checks in during specified hours or days of the week. And while this may benefit some seniors, others require a more flexible and accommodating schedule. How we re different is that we don t send someone to a client just because it s what the client wants, says Corea. We re acting in the best interests of the client, which sometimes means we have to refer them elsewhere. 14 Hazy Diagnosis Does a new set of guidelines bring us any closer to solving the mystery of Alzheimer s? By Emily Riley It s official: For the first time in 27 years, there are new criteria and guidelines for the diagnoses of Alzheimer s disease. The most common form of dementia, this incurable, degenerative, terminal condition is projected to affect one out of every 85 people globally by 2050, according to the United Nations Department of Social and Economic Affairs. Alzheimer s disease is a function of aging, says Dr. Howard Caplan, a neurologist at Lankenau Hospital. Every five years past the age of 65, your chances for developing the disease double. By 85, you re looking at a 50-percent chance of having some form of the disease. It s the epidemic of the 21st century. The initial diagnostic criteria for the disease were published by the Alzheimer s Association and the National Institute of Neurological and Communicative H14 Healthcare guide fall 2011/Winter 2012

After a thorough evaluation of the impact of the past three decades of advances, researchers have broken down the disease into three distinct stages: preclinical, mild cognitive impairment, and dementia due to Alzheimer s disease. The preclinical stage, Caplan says, may or may not involve symptoms associated with a progression toward Alzheimer s. Rather, it s most closely associated with short breaks in memory or other mild mental lapses attributed to natural aging. Mild cognitive impairment begins when these lapses become significant and noticeable, but don t necessarily compromise day-to-day living. Many, but not all, go on to develop the third stage, where mental and behavioral symptoms greatly impair a person s ability to function in daily life. While the new classification system more clearly delineates symptoms and treatment options, diagnosing Alzheimer s and monitoring its progression is not as simple as picking a category. As of right now, we have nothing specific that we can look at to make a definite diagnosis, says Caplan. At its earliest onset, symptoms could also be attributed to depression or a different type of dementia that s not necessarily Alzheimer s. We don t know for sure until we ve established that the brain cells have begun to degenerate and by that point, it s too late. The second notable revision involves biomarkers or physiologic indicators of the disease s onset. Similar to blood glucose levels in diabetes patients and cholesterol levels as a measure for heartdisease risk, the biomarkers could potentially provide a reliable and calculable indication of the presence or absence of Alzheimer s. Currently, Alzheimer s biomarkers are limited to cerebrospinal fluid (which would involve a spinal tap) and the ratio between tau proteins and betaamyloid plaques in the brain. For doctors and patients, the new criteria and guidelines don t provide any shortterm answers. We don t know what causes Alzheimer s, and we have no definite way yet of making a clinical diagnosis without looking at the brain, Caplan says. But from a research standpoint, any new data is encouraging. Anything that brings more awareness is a step in the right direction, says Caplan. This means that there could be more of a push for research and exploring treatment options. We re limited at the moment, and it s an incredibly frustrating disease. We have to find what can be fixed.
