2 VERSION 3-15 th Audiology Adult (17 years +) Service Description: Inhealth Netcare assessment, hearing aid fitting, follow up. GSTT Complex audiology services as tinnitus, balance (referrals through ENT). This department also provides a comprehensive rehabilitation service, including specialist tinnitus, vestibular and balance rehabilitation. St Georges - Complex audiology services as tinnitus, balance (referrals through ENT), specialised diagnostic and rehabilitative services for patients presenting with dizziness, imbalance, hearing loss and tinnitus, and also provides specialist learning disability clinics. Inhealth, GSTT and St Georges will provide ingoing repair and management of hearing aid after their 8 week follow up appointment How to refer: Direct access. Referral criteria: For Inhealth, prior to referral you should ensure wax has been removed from patients ears to prevent cancellation or delay of the assessment. For GSTT, Over 60 years of age No excessive ear wax No active discharge or history of discharge No skin disease in ear canals No perforated or inflamed ear drums Page 2 of 23

3 VERSION 3-15 th No earache No tinnitus (other than the occasional/transient) No sudden onset of hearing loss or sudden deterioration of loss No fluctuating loss (beyond that associated with colds) No markedly asymmetrical hearing loss No vertigo (not the common unsteadiness of old age) If the patient already wears an NHS hearing aid it would be helpful to include where it was fitted, to enable GSTT to contact them for further information. Clients can be seen at home, within clinics, in our office and any other location where appropriate. The service aims to deliver timely accessible services to high quality, evidence based care delivered by staff with the right skills & competencies, whether it be in the home. The Community Children s Nursing Team provide care to the following care groups: 1. Acute Caseload - Acute caseload consists of those children and young people requiring short-term intervention or nursing care from the team. Such as administration of intravenous drugs, wound management, eczema intervention e.g. wet wraps. 2. Chronic Caseload - Chronic caseload consists of those children and young people who are requiring long term support over several months or years. 3. Primarily a clinic based service with limited home visiting capacity. Also works closely with selected General Practices in developing heart failure knowledge, skills and processes for identifying patients and managing them better. Are in receipt of a continuing care package and who require specialist respite care due to the complexity and intensity of their condition. 3. The appointment covers: o spirometry testing to confirm a COPD/asthma diagnosis o review of drugs o smoking assessment and advice o discussion re lifestyle and support o onward referral, where applicable, to pulmonary rehabilitation Pulmonary rehabilitation is a set number of classes where patients engage in exercise appropriate to their stage of disease. The team adopts a holistic view of health which encompasses both physical health and mental health and well-being; building local capacity by facilitating education & training as well as engaging and developing partnerships with key service providers and recipients. Their core work includes :- Translating health promotion theory into practice by developing, supporting and managing community based health promotion project Providing a range of generic and tailored training opportunities to increase the capacity of front line staff and community providers to deliver, monitor and evaluate effective health promotion activities for priority groups. Providing consultancy, advice and guidance to a range of providers and voluntary sector groups, promoting community development and partnership working to increase the relevance, appropriateness and effectiveness of local health promotion activity. Bladder & bowel assessments undertaken by generic nurses for housebound adults, children and adolescents through the general adult community nursing and childrens nursing teams with a focus on the promotion of continence. Children with special needs and enuresis can be referred to the childrens bowel and bladder service. 2. A Continence Specialist Nurse providing training, advice & support to generic nurses across the borough. The team runs open-access nurse-led health clinics in homeless hostels and day centres. The core business of the team is to facilitate and provide joint working between homeless and mainstream services. SLAM manage the staff rota s, supervision and triage the referrals. The team are based with SLAM but work across LCH and other health and voluntary sector sites in Lambeth. The PWP s provide brief evidence based psychological interventions to people with mild to moderate depressions and anxiety problems. Urgent patients all seen within 3 days Contact: Fax referral form to service centre Phone with urgent referral/ for advice also via service centre MyAction Service Description: Lambeth MyAction service delivers an intervention to patients, and their families, who are identified as very high-risk of developing CVD but have not yet had a cardiac event. These individuals are identified after having an NHS Health check either at their GP practice or in a community setting, carried out by the NHS Health Checks outreach team. They and their partners are offered the programme in a community health centre or other convenient community venues. The MyAction is a 8-12 week programme delivered by a multidisciplinary team of a lead nurse, dietitian and physical activity specialist. The programme covers the following topics:- Lifestyle Habits (smoking, diet, physical activity) Cardiovascular Risk Factors (anthropometrics, blood pressure, blood lipids and blood glucose) Cardio-protective Medications Psycho-social Factors (Health Related Quality of Life, Anxiety and Depression, Illness and Risk Perception) Family and Social History presentation of cardiac symptoms and medical history Contact Name: Jacqueline Quinn Telephone: lam-pct.myaction@nhs.net Page 15 of 23

16 VERSION 3-15 th NHS Health Checks Outreach team Service Description: This is a team of two specialist nurses who offer CVD risk assessment and advice aiming to reduce the risk factors of those vulnerable to developing vascular disease and to detect early indication of disease. The service is targeted at adults (male and female) between the age of 40 to 75 years who reside in Lambeth or who are registered with a Lambeth General Practitioner (GP) and who have no previous diagnosis of cardiovascular disease. Most of the health checks are provided through general practice, however, the LCH service has adopted an outreach focus with clinics are provided in various locations around Lambeth to maximise access for hard to reach groups. The checks involve: Risk assessment - will constitute questions to establish factors in individuals family history, lifestyle and other conditions that may affect their levels of risk - Appropriate tools and methodologies, using Near Patient Testing to measure individuals physical indicators of risk of Cardiovascular Disease - Use of JBS2 tool to calculate the individuals level of risk Communication of risk and advice on how to reduce/manage the risk Onward referral - to general practice and behaviour change interventions for individuals at high risk (>20%) in order to obtain support to manage the risk Nutrition and Dietetics service Contact Name: Tessa Thomas Douglas or Thelma Bangura Telephone: / lam-pct.nhshcteam@nhs.net Adults Currently the Lambeth Community Adult Nutrition & Dietetics service can see the following patients: Care home/nursing home residents Page 16 of 23

Exclusions: Babies under 6 months of age Children who are so severely disabled that they are unlikely to be able to participate in any type of behavioural hearing test Children who use, or are known to require, hearing aids Children who are referred because they recently suffered bacterial meningitis or a skull fracture Referrals should be made using the Audiology referral form. Podiatric Surgery Service description: alternative to orthopaedic foot and ankle surgery based in the hospital - operate under local anaesthesia on a daycase basis.
