1 Learning Opportunities Directory for students nurses during practice placements at the of Chester Hospital NHS Foundation Trust Guidance You are expected to co-ordinate spokes and short visits during your placement to ensure you receive a breadth of experiences during your training.your mentor should support you in identifying relevant spokes and short visits that are suitable for your learning needs and where possible follows the patients journey. Prior to organising a visit please consider the following: Ensure visits are arranged at appropriate times avoiding initial, midway and final assessment dates. Visits should not be arranged during University simulation days or during University-allocated spoke weeks. Ensure visits do not compromise time with your mentor. Visits should be relevant to your level of learning. Identify which learning outcomes you aim to achieve from your visit. Identify with your mentor any specific competencies within the domains which are to be assessed /evidenced whilst on the visit. During your visit Ensure you take your Practice Assessment Record (PAR) to each visit. Ensure the appropriate member of staff records your attendance (in hours) within the PAR and completes either the page titled spoke practice experience (for visits that are one week or more) or the page titled short visits (anything less than one week).your hub placement mentor will not be able to account /sign for any hours of practice performed on spokes or short visits if there is no evidence recorded within the PAR. If you are not able to attend your visit please contact the relevant person. Following the visit Reflect on what you have learnt from the visit. Discuss with your mentor how the visit has enhanced your practice. Give feedback to the placement staff if you are given the opportunity to do so, or within your PARE placement evaluation. 1

To nurse patients with ophthalmic conditions, providing pre & post-operative care. Opportunity to visit Pre-assessment clinic, observe minor & major eye surgery. An insight in to the role of the Specialist Nurse Stroke Specific. An introduction to assessment and management of the acute Stoke, TIA from referral to discharge or transfer of care. Pre hospital protocol and local pathways. Includes primary and secondary prevention advice. An introduction to CT Head. An overview of the patients journey, using an interdisciplinary approach, your part in their progress. To develop an understanding of stroke research and eligibility criteria for stroke trials. Expand knowledge of consent and ethical considerations such as vulnerable adults taking part in research. Gain underpinning knowledge of causes and symptoms of stroke. Beneficial to have background knowledge of anatomy, physiology and causes of stroke. Opportunity to gain insight in to the varied clinics held in GOPD such as General Gynae Clinic, EPAU, Fertility, Urogynae, Colposcopy and Pre- Operative Assessment. Experience the patient journey from GP referral to diagnosis. Opportunity to observe the varied clinics, breast screening process and breast care nurse workload. Excellent opportunity to observe the wide variety of clinical procedures carried out in IR that covers all specialities within the Trust. IR is based in Radiology and consideration should be taken in possible or confirmed pregnancy. Observe team supporting/advising & educating staff, patients & families. Take part in clinical surveillance, audit & clinical reviews. Develop knowledge of risk assessment and policies and procedures related to infection prevention & control. Observe pacemaker implants & pacemaker generator changes, cardioversions, implant of loop recorders, angiograms, pre & post-operative care of day case patients within the unit. 1 day or up to 2 week visit Monday-Friday. Care of Elderly, Learning Disability Tuesday Friday all day Day visit or up to 1 week Trinity ward (33) Trinity ward (33) Women s unit (40) Spoke allocation on a Wednesday Friday. Observe exercise assessments and the delivery of cardiac rehabilitation in hospital and or community locations. You will observe a number of clinic examinations/tests including ECG s, ECHO, Lung Function tests. Endoscopic procedures including bronchoscopy, colonoscopy, cystoscopy, gastroscopy, insertion of PEG. Opportunity to observe pre-operative preparation, assessment and consent. Post-operative care. Students will have opportunity to visit the pathology unit and observe the role of the histologist in studying the microscopic structure of tissue samples Learn about the role of the OT and Physiotherapist. Observe health promotion to patients and families. Observe various patient treatment pathways, rehabilitation planning, attend MDT meetings and follow the patient on a home visit. Students will gain an understanding of the role of the tissue viability nurse and the service provided. This includes the management of wound care, clinical care, education and research. Opportunity to observe and take part in pressure ulcer assessment, prevention and treatment. Management of complex discharges, completion of health assessment forms, liaise with social services and multi-disciplinary team. Supporting patients in their own homes following discharge. Working with multidisciplinary team. Importance of effective discharge planning and prevention of re-admission. Opportunity to observe assessment of inpatients on wards and on A&E. Work with other agencies including GPs, social services. Accompany nurses during community visits. 1 day visit Cardiology- Cathedral ward 42/CCU Half day- 1 day visit 1-5 day visit Monday- Friday. Students may visit the unit for a ½ day visit Monday-Friday Ideally should be incorporated as part of the patient journey 1-5 day spoke visit Monday-Friday, beneficial for 1st year students to have 6 weeks prior experience before spoke visit. 1-2 day short visit. Beneficial to follow patient journey. Visits will be Monday -Friday 1-5 day visit/spoke. 3

Completion of psychiatric health assessments and liaising with social services, carers and other mental health professionals to help facilitate early discharge. Assessments of patients with dementia, delirium and challenging behaviour. Role of Heart failure service Urodynamic studies, lithotripsy, bladder instillations, histology clinic, transrectal ultrasound and biopsies. Assessment procedure prior to surgery, physical observations, patient counselling and preparation. Students shadow the colorectal team and visit departments relevant to the patient s pathway E.G. Endoscopy, CT scanning, outpatients department and theatre. There are learning opportunities attached to the placement along with clinical competencies. Students are able to spend a proportion of the time studying with resources provided by the department. Podiatrist, foot clinic, patient education, consultant review clinic, insulin education programme, diabetic dietician, diabetes in pregnancy, antenatal clinic. Active research, data collection, informed consent procedures, clinical trials, new medications, observe preparing & spinning samples. ECG s, venepuncture & vital signs. Home visits, Cardio-Respiratory Lab, Respiratory Clinic, patient education, ward visits, disease differentiation, TB Nurse specialist, Lung Cancer Specialist Nurse. s suffering from conditions of the lower limbs (wounds or nonwounds) linked to vascular surgery and dermatology. Specific placement opportunities: Dealing varying age group; Wound care; Aseptic technique/ clean technique; Experience role of N. Practitioner; assessment (detailed); Communication skills-mdt; Dressing types and techniques; Bandaging skills; OPD service/ambulatory care; Cross linking/relationship + primary and secondary; Psychological issues and body image. 1 day visit. Students should have some experience in caring for patients with Dementia prior to visit ½ day with in-patient heart failure team Care of Elderly ward 43, 33, and 34. Cardiology or A&E 1-5 day visit from Monday- Friday. Ward 52, day visit Monday-Friday Any Surgical ward 1-5 day visit Monday-Friday recommended following at least 6 weeks experience on ward or prior knowledge of Colo-rectal speciality. Opportunity to work with a variety of specialist nurses including Irritable bowel nurse, PEG and Nutrition Nurse and Liver specialist nurse. Observe clinics, MDT meetings, patient advice service. Attend nurse led clinic where patients are diagnosed with skin cancer. Observe full skin examination, advice on sun protection, treatment plans and assessment of patient holistic needs and support. 20 bedded unit with aim to discharge patient within 72 hours. Opportunity to work with multi-professional teams including frailty team, social services, rapid response and discharge liaison team. Practice assessment and discharge skills and basic fundamental skills Focus on admission avoidance. Opportunity to attend healthy aged clinics at EPH, Falls clinics, inpatient assessment and working collaborative with various hospital services and social services Students will gain an increased understanding of the impact of a cancer diagnosis on the patient and family. There will be the opportunity to observe how the centre team respond to9 the wide variety of issues which are presented by people affected by cancer. This can include emotional issues, financial queries, finding out about lo9cal services, end of life and bereavement enquiries. 1-5 day visit Monday- Friday.
