1 ISPAN Iowa Society of PeriAnesthesia Nurses Volume 24, Issue 2 Summer 2010 President s Message: Hello, Cathy Tigges As the new fiscal year begins for ISPAN and we plan for our future it will serve us well to reflect for a few minutes on our recent successes. Many changes have occurred in just a few years and as a professional organization, we would do well to take stock not only of where we are, but how we got there. Hopefully, this shows our continued progress in quality patient care. Summer Greetings from the ISPAN Board ASPAN has been a leader in promoting positive changes in the perianesthesia arena. We are regarded as experts in our field. Many organizations who have included ASPAN members on their committees. Among these are the National Association of Clinical Nurse Specialists, American Society of Pain Management Nurses, American College of Surgeons, American Society of Anesthesiologists, American Association of Nurse Anesthetists, and Surgical Care Improvement Project. Collaboration among all of these groups is necessary for providing coordinated care for our patients. The National Priorities Partnership (NPP) is charged with developing what will become a world class, affordable health care system. The role for the frontline bedside perianesthesia nurse is not yet determined. However, as an organization, we are helping to draw the road map for this adventure. It will take all of us in health care to accomplish this. The NPP has established national goals, but as we all know, it must start locally. Keeping up to date on recent developments is vital. Through ASPAN s Journal of PeriAnesthesia Nursing (JoPAN), Breathline, ISPAN s newsletter, as well as other health care sites we are able to stay educated about current issues. As the saying goes If we do not move forward, we are falling behind. I find that so true in our profession. ASPAN does an excellent job of keeping us moving forward. Every JoPAN issue is filled with relevant research articles and hot topics. I encourage you to read them and share them with your co-worker (Better yet encourage them to join ASPAN). The ASPAN web site is also a good resource to find new information. If you have not already checked it out, please go see what it has to offer. Reminders! Minutes from the January 26, 2010 meeting distributed and reviewed. III. Treasurer s Report: Sandy Hoctel. Copy submitted for the minutes. Younkers Community Day: Did not meet matching requirement. Membership list (at the time) 121 current members/59 renewals. IV. Newsletter Editor s Report: Sandy Gardner: Summer submission deadline is June 30th. Discussion and decision made to produce 3 instead of 4 issues annually. Ideas from other components: Have a new member packet and have one individual responsible for making and sending them out. Identify ISPAN contact persons for each hospital/ ambulatory center. Job shadowing for Board members with invitations to ISPAN members who may be interested in becoming active on the Board. V. Governmental Affairs Report: Helen Riedesel: M.S. & H.R. attended the Iowa Legislative Day in Des Moines on 2/18/2010. Were able to meet with multiple key legislators and committee persons to discuss Healthcare Bill. Year-by-year timeline of the Health Reform distributed. VI. Fall Conference Report: Molly Schrader: To be held Saturday, October 16th in Cedar Rapids, IA at Mercy Hospital Education Center. Five speakers confirmed with the suggestion of a speaker on generational Diversity. S.G. will design the brochure and Sandy Hoctel will arrange the printing through IMMC/ Des Moines. VII. National Conference: Dispersal of stipend checks to Officers/ Active Board & Ad Hoc Board members. Component Night. Discussion/ decision to donate Component night funds as follows- $200 to the Foundation. S.H. will send the check to the ASPAN Office in order to ASPAN's 30th National Conference Reinvest In Your Potential April 3-7, 2011 Sheraton Seattle Hotel, Seattle, Washington It s never too early to start planning for the National Conference. This year we will be joining ASPAN members from all over the country and international guests in the beautiful Northwest. Consider making it a priority to be able to attend the 30th annual event. Things you may not have thought of to help you find your way to Seattle. Can you apply for hospital scholarships, education grants, hold bake sales, or request sponsor support to help you fund your goal of attending? Stay on the lookout for new information as it comes out. We will keep you informed as new information gets to ISPAN. have ISPAN recognized on the Foundation Donor list. Iowa Night Out will be held on Tuesday evening at a location to be determined. Appetizers provided by ISPAN. VIII. New Business: Work session planned for update of the ISPAN bylaws and job descriptions. B.B. distributed drafts of documents with discussion points from material presented at the fall CDI. VIII. 4 Page 4 ASPAN s New President ( ) Kim Kraft, BSN, RN, CPAN The American Society of PeriAnesthesia Nurses (ASPAN) at its 29th National Conference (April 18-22, 2010) in New Orleans, LA, welcomed Kim Kraft into her new role as national president. Ms. Kraft, BSN RN CPAN, will serve a one-year term, and her presidential theme is Reinvest in Your Potential. Kraft says, As the organization for perianesthesia nursing, ASPAN must reach out to all members and provide them with the programs and services needed to support their practice, assist them in reaching their professional potential, and position them as ambassadors for ASPAN s core purpose and values. Her goal involves implementing the operational and strategic plan already in place to guide the ASPAN Board to redefine the organizational infrastructure, thus insuring more robust opportunities for members to become engaged and involved, and maximizing their potential. Prior to her current office, Ms. Kraft served as Vice President/President-Elect, Regional Director, Chair of the Membership & Marketing Committee, and was a member of numerous committees serving to fill other vital roles throughout ASPAN. In 2007, she was the recipient of ASPAN s Excellence in Clinical Practice Award. The web site will continue to evolve; your input will be helpful. In our green society communication has become so much easier and your ideas help us to grow. The web is a wonderful thing, it has made obtaining CEU s so much easier, and we can go to the web anytime to get our continuing education. But, I hope you do not dismiss the importance of attending conferences. Over the past year I have attended ISPAN Fall Conference, University of Iowa Perianesthesia Conference, as well as ASPAN s National Conference in New Orleans. Besides providing us with a wealth of information, we also are able to do some networking. When we sit down beside someone from another facility we find that some problems are universal. By talking with them you may find things that have worked for other facilities. Having the opportunity to ask questions of the presenter, we are able to better understand the topics. So please join us this fall in Cedar Rapids for ISPAN s Fall Conference. In closing, I want to thank all of you for being a member of your professional organization and encourage you -- if you haven t already -- to become certified. 5 Page 5 Region II Report Hello ISPAN members, It is my honor to be your ASPAN Region 2 Director. It has been an amazing past few months. For most of you that don t know me, I have been a perianesthesia nurse since 1981 and an ASPAN member for over 25 years. I am currently a Preop/PACU/Postop RN IV staff nurse at Children s Mercy Hospital- South Campus. As your liaison to ASPAN, I hope to hear from all of you and please let me know what your needs/concerns are. What best practices are you all involved in that we can share nationally? I met some of you at the national conference in New Orleans. It is my goal to be available to you as much as you need me to be. The RD s are busy getting ready for the Component Development Institute (CDI) to be held in Louisville, Kentucky on September 10-12, The brochure should be available to the component officers now. As in the past, ASPAN will waive registration fees for two designated ISPAN members. It is customary to send members that are being groomed to assume leadership in the component. It is part and parcel of how succession planning for the state and ASPAN can be achieved. On a personal note, I have been busy planning my only daughter s wedding. She and I accomplished this under two months (that s right, just when I got home from conference) and the fruition of our efforts will culminate this Saturday June 26, I will be able to relax the next day I guess. Please keep in touch. I am only a phone call or away. 6 Page 6 ISPAN Governmental Affairs Report The Iowa Legislature (adjourned on March 27, 2010). Governor Culver signed the Nursing Workforce Legislation on April 21. This legislation aims to address the state nursing shortage and seeks to understand the nursing work force needs. The law includes education initiatives and a nurse educator scholarship-in-exchange-for service program. The Centers for Medicare/Medicaid Services (CMS) has released a Proposed Rule for implementation of the Braley hospital payment provision, which was Section 1109, Payment for Qualifying Hospitals, in the House Reconciliation piece of Health Care Reform. This provision provides additional FY11 and FY12 funding to hospitals in counties that fall in the lowest quartile of Medicare spending. Under the Proposed Rule for FY11, Iowa will receive $12.52 million, which is the most per capita funding of any state, and the 3rd most funding based on actual dollars. This is significant step towards finally addressing geographic inequities in Medicare and the reimbursement to Iowa hospitals. This is one of a number of provisions related to Geographic Disparity and Value that Rep. Braley, secured, along with the help of Senator Harkin, Reps. Boswell and Loebsack, and other members of the Quality Care Coalition The future of health care is closely tied to the future of nursing. Advanced Practice Nurses represent a valuable resource in the national effort to provide timely, affordable health care and the demand for their services is expected to escalate due to the critical shortage of primary care physicians, an aging baby boomer population, continued efforts to improve quality and the introduction of medical homes. Iowa hospitals employ advanced practice nurses in a variety of roles, including certified clinical nurse specialists, certified nurse practitioners, certified nurse mid-wives and certified registered nurse anesthetists. Recently, specialty physician groups have attempted to restrict current nursing practice by declaring chronic interventional pain management the practice of medicine and not within the scope of practice of other health care professionals. These same groups have also attempted to block the supervision of fluoroscopic radiography by advanced practice nurses. Advanced nursing practice enhances patient access to effective, integrated and coordinated health care and provides a much needed link to the most vulnerable of our population, such as the elderly and those living in rural and underserved areas. Chronic pain can be debilitating and often becomes the defining factor in patient s lives. Without relief, or the hope of relief, many patients lose the ability to eat, sleep, work and function normally. Certified Registered Nurse Anesthetists have been safely and successfully treating chronic pain in Iowa hospitals for two decades. To date, the Iowa Board of Nursing cannot identify a single disciplinary action taken against a Certified Registered Nurse Anesthetist relevant to chronic interventional pain management. Any legislation that would limit the ability of Certified Registered Nurse Anesthetist to treat patients with chronic pain would have far reaching implications. For example, some Iowans would have to travel over 100 miles and contend with waiting times of six weeks to see a pain management physician both of which would be difficult for someone experiencing chronic pain. It is the belief that Nursing is the organization with the knowledge, expertise and legal authority for regulating and enforcing rules of nursing practice and opposes any attempt by medical specialties to regulate nursing practice in order to advance their own proprietary interests. (1) Federal: The Quality of Care Coalition has been formed to provide members of Congress a forum to transform the healthcare system to reward value in care and make evidence-based, quality care the standard, regardless of location. It will also continue to address geographic variation in Medicare reimbursement. The group has formally registered as a Congressional Caucus. 1. 7 Page 7 29th Annual National Conference Representative Report The thirteenth American Society of Perianesthesia Nursing Representative Assembly meeting was held April 18 th 2010 in New Orleans, LA. Representing the Iowa chapter of American Perianesthesia Nursing was the current president Cathy Tigges and the immediate past president Molly Schrader. The first item on the agenda was candidates for the National Offices made presentations to the assembly. Time was given for questions and answers. After a short break the vote for the officers occurred. You don t see very many workshops or conferences, hospitals have had their awards banquets, and things haven t revved up for Fall as yet. As an ISPAN/ASPAN member, this is a great time to start encouraging your friends to become a member. Remind them about discounts on the Fall ISPAN conference, discounts for the ASPAN national conference, additional credit for career/professional ladders and for some additional benefits from magnet status. For you, the existing member, remember to have the new recruit write you name on the application sheet. For every member you recruit, you have a chance to win a full registration to the next year s ASPAN conference. I know times are tough and for many of us, finances are tight. 9 Page 9 How Doctors Think By Dr. Jerome Groopman Popular Author Readers may remember my 2004 review of Dr.Groopman s previous book, The Anatomy of Hope. I had enjoyed hearing him speak on radio and then at my local library at that time. In this latest book, Dr. Groopman continues his storytelling when relating case histories. Don t miss the introduction of this book, where he relates the years-long, complicated course of a patient s illness. When a physician starts fresh to hear her painful story, he hears clues that lead to the correct diagnosis for her. The patient is so relieved when a complete change in her treatment begins to make her feel better after such a long time being sick. Local Connection The author started his internship at Massachusetts General Hospital in July of 1976, just as I was finishing my first year as a nurse there. His tale of his first harrowing night as an intern reminds me of my first nights as a nurse: caring for 36 patients with my wonderful nurse s aide, Maddie. Dr. Groopman discusses the difference between learning medicine catch as catch can in the old days versus learning preset algorithms with a decision tree today. He feels today s method works as long as the patient s signs and symptoms are clear-cut but falls apart if the symptoms are vague and test results inexact. He further states that this method may discourage new doctors from thinking outside the box. However, he does acknowledge that it is risky to go on intuition alone. As Much Art as Science I often tell patients that medicine is as much art as science, even with all the technology available to us today. Dr. Groopman states that physicians really need information from patients and their families to help provide key clues to what is wrong. He actually learned this when he was the patient himself. Chapter 7 tells of his experience seeking treatment for a painful right hand. He consulted with SIX hand surgeons over a three year period and got four different opinions about what to do about it!!! It s most interesting that Dr. Groopman says that misdiagnoses are not necessarily medical mistakes. He further states that many doctors remember their misdiagnoses, analyze them, remember them and incorporate those memories into their thinking. How Do Nurses Think? When I did a brief search of How do nurses think, I only found one study done on this subject by Iain Graham at Southern Cross University. Mr. Graham s findings show that there is a dynamic nature of how nurses think and experience nursing. To conclude, I highly recommend How Doctors Think for its highly readable storytelling. It s August and I am wondering where the summer has gone. If you are like me and many of my colleagues, as we near Fall, we start thinking of school - for our children, grandchildren, and maybe even ourselves! I, for one, can t pass up the new school supplies aisle without checking it out. Those new crayons, pencils and notebooks always seem to epitomize new beginnings, hope and anticipation. It is this time of year that CEU offerings are starting to show up in your mailbox (ISPAN s included!) and you decide where you want to invest your time and money. I will hope that when you make those decisions, you consider what your workshop choices mean to you professionally. Will this offering help you professionally? Will you gain new knowledge or expend on the skills and/or education that you already possess? Will you just be getting enough CEU s to renew your license? Will you just put in the time, sitting quietly or doodling? Or will you be present and engaged open to what is being said or discussed? I know, not all speakers are created equal, but most have put in a great deal of time to create a presentation for you. I always hope that I can come from a workshop with one or two pearls that I can t wait to tell my colleagues. You know, that tip, fact, statistic, or means to do something different/better/faster the list goes on. If I have gotten a pearl from a conference, I consider it a good conference. Perhaps I am too idealistic, but after 30-some years of nursing, I still think gaining new ideas or skills is a positive thing. It shows that we are still capable of growth, evolution and development. I like to think that this is one of the special hallmarks of nurses. That, as we pass from novice to experienced, we become more and more adept at finding those pearls from the classes, situations, and life events that we share. The next time that you go to a conference, listen, engage, and find your pearl to amaze your colleagues, and yourself! Plus, take time to smell the new crayons! Well, that s my 2 -worth.
