2 Session 1: Preventing Readmissions Through Safe Transitions Lisa Johnson, MSN, RN, GCNS-BC Senior Care Manager Care Management MultiCare Good Samaritan Hospital Puyallup, WA This session will highlight current evidence-based models of care transition services and provide insight on how to utilize and adapt these models to develop a program appropriate for your community. The presentation will also highlight why it really takes a community to address the issues of hospital readmissions and chronic disease. 1. Describe models of current EBP care transition services 2. Identify limitations of the models 3. Describe strategies to overcome these limitations Session 2: Ethical Dilemmas: Strategies to Assist Surrogate Decision Makers with Tough End-of-Life Decisions Donna Foliart, MD, MPH Medical Director Palliative Care John Muir Medical Center Walnut Creek, CA Making decisions regarding end-of-life medical care for a loved one who no longer has decision-making capacity is extremely difficult. Surrogate decision makers may have limited understanding of the medical issues and are uncertain as to the extent of their loved one s illness and prognosis. The result is that family members may insist upon care that the medical team feels is futile. Dr. Foliart will discuss ethical dilemmas at end-of-life, explore reasons behind demands for care that has more burden than benefit, and review ways to assist families in making medical decisions. Four cases highlighting these issues will be discussed. 1. Discuss ethical dilemmas at end-of-life 2. Explore why surrogates may want futile care 3. Strategies to improve collaboration, as well as a discussion about a successful dyad model will be presented. 1. Identify historical perspectives of case management practice 2. 4 Session 5: Health Care Reform Update: Challenge & Opportunity Patricia Blaisdell, MA, MS Vice President Post Acute Care Services California Hospital Association Sacramento, CA The passage of the Affordable Care Act (ACA) in 2010 set into motion wide-ranging changes to the financing and delivery of health care services, including expanded access to care and an emphasis on care coordination, quality of care and patient outcomes. The current presentation will provide an update on the implementation of the ACA with a particular focus on implications for case managers. An update of other significant state and federal issues will also be presented. 1. Understand current status of the ACA and federal health care reform implementation 2. Identify & understand the components of health care reform (HCR) that affect hospitals and other providers, including the post-acute care continuum 3.
