5 Jeramon non kom Kindergarten immunization requirements for the Kwajalein School System If you have a child who will be starting kindergarten for the school year, certain immunizations are required: Diptheria, tetanus, acellular Pertussis #5 (DtaP) Inactivated polio vaccine #4 (IPV); must have one polio after age 4 Measles, mumps, rubella #2 (MMR) Varicella (chicken pox vaccine) Hepatitis B vaccines: series of three shots (HBV) Tuberculosis skin test Written documentation must be provided to the school for the immunizations received. Immunizations must be verified by the school nurse at Kwajalein Hospital. Immunization hours at Kwajalein Hospital are Tuesday through Saturday, from 1 p.m. to 4 p.m. Questions?
