1 ALASKA AIR CARRIERS ASSOCIATION FAMILY ASSISTANCE PLAN Required Update Due: October 2,200O ADDENDUM A The purpose of this Addendum A is to formally adopt and incorporate the following into Promech Inc (company name) Family Assistance Plan. 1. With respect to the term transportation the definition is amended to include an accident involving a foreign air carrier that occurs within the United States 2. With respect to the term attorney the amended definition includes no Solicitations from lawyers, associates, agents, employees, or other representatives of an attorney for 45 days from date of accident. 3. The definition of passenger is expanded to include any employee of an air carrier or foreign air carrier aboard an aircraft, and any person aboard the aircraft without regard to whether the person paid for the transportation, occupied a seat, or held a reservation for the flight. 4. With respect to flight reservation information distribution, an assurance is made that, upon request of the family of a passenger, the air carrier will inform the family of whether the passenger s name appeared on a preliminary passenger manifest for the flight involved in the accident. 5. Promech Inc. (company name) will provide adequate training to the employees and agents of the company to meet the needs of survivors and family members following an accident. This training does not necessarily include every employee and may be satisfied by formation and use of a Care Team. 6. Promech Inc. (company name) will, in the event that a U.S. aircraft accident involving major loss of life, occurs on foreign soil, the U.S. air carrier will consult the NTSB Family Affairs Office in Washington D.C. /-r after hours (emergency) v d further, request the NTSB to provide notification to the U.S. Department of State Duty Officer - This Addendum A is hereby incorporated and made a part of Promech Inc (company name) Family Assistance Plan, dated October 4,200O. Family Assistance Plan Purpose: In the unlikely event of an aviation disaster, this manual will provide guidance for the employees Of Promech, Inc. Upon notification of a crash involving overnight hospitalization or fatality, initiate the airline s Incident Command System (ISC). All decisions/contact must be routed through the Incident Commander. Incziient Commander Name of Incident Commander: Mark Easterly Location of Command Post: 1515 Tongass Ave, Ketchikan AK.
