11 11 How Big Is the Problem of Readmissions from SNFs Accounts for more ED visits than any other residential settings About ½ of residents have at least one ED visit, and ½ of these have two or more per year More than 1/3 of residents are admitted to a hospital at least once a year, more than 40% have two or more admissions
