2 Workers Compensation Key Points Responsibilities of Injured Employee: Report all work related injuries immediately to your supervisor Obtain immediate medical treatment if condition is urgent Complete the Job Related Injury Report Complete acknowledgement that district participates in a Texas Certified Healthcare Network and that injured employee must seek non-emergency treatment from a participating provider Obtain non-emergency medical care if needed

3 Medical Treatment: Time away from work on day of injury or day following will be compensated as time worked. Delayed medical treatment will be sought on employee s own time. All follow up treatment will be scheduled on the employee s own time (either non-work hours or employee's leave time) Return to Work Program: District does have a Return to Work Program and employees are expected to comply with the program If employee is placed on modified duty, district will attempt to accommodate limitations in employee's assigned position/department for a designated length of time If limitations can not be accommodated in assigned position/department, modified duty will be sought elsewhere in district If no modified duty is available to accommodate limitations, employee will be off work & will receive temporary income benefits at approximately 70% of average weekly wages up to a set maximum

6 SCUCISD Workers Compensation Injury Reporting Process With TXHCN As of October 1, 2011, the Schertz-Cibolo-Universal City ISD will be participating in a Texas Healthcare Certified Network for its workers compensation self-insured program. All employees have initially been notified of the district s participation in this network. However, should an employee be injured the district is required to also notify that employee again. The following process will be followed to accomplish this: 1. Employee reports, or suggests, an injury occurred due to performance of job duties, or the supervisor is aware of incident possibly resulting in injury, the law requires that it be reported as an on-the-job injury. 2. Employee is provided the Job Related Injury Report to complete at that time. Report should be completed immediately unless emergency medical attention is needed. 3. Employee is provided a copy of the TXHCN Employee Notice of Certified Network in either English or Spanish depending on their language, employee is reminded that the district is participating in the network and asked to review the material and again sign and return the acknowledgement page at the back of the packet. 4. If employee is based on a campus where there is a nurse, it is helpful if the employee is evaluated by the nurse. 5. If further medical treatment is needed, employee is to be provided a Medical Authorization Form and directed to Concentra Medical Center (which is in the network) for a baseline evaluation of the injury. Concentra Medical Center is open 8:00am to 5:00pm M-F. If treatment is necessary outside of these times, employee may seek treatment with another participating workers' comp provider. 6. After this first visit to Concentra, the employee does have the ability to seek medical treatment with another provider within the parameters outlined in the certified network notice. Charges for treatment by a provider outside the network for those employees covered under the network will not be covered by the district's workers' compensation program. Additional providers may be found at 7. Once the employee is taken care of, the supervisor or designee will then a. notify the Risk Manager by phone or of the incident, the name of the injured employee, and whether the employee will be seeking medical treatment, b. complete the remaining paperwork (Employer's First Report of Injury & Workers' Compensation Supplemental Form), c. forward the above two forms along with the Job Related Injury Report from the employee and the signed acknowledgement of the district's participation in the certified network to the Risk Manager as soon as is feasible. There is an 8 day timeline to get the claim reported to the carrier and either accepted or denied. 8. All employees should also be reminded that the district does have two administrative policies on job related injuries and modified job duties. These policies are posted on the Employee Benefits page as well as included in the Compensation Booklet. Employees should review these policies so they are aware of the procedures.
