2 2 Tracking Trends Number of Latinos that gained insurance 4.2 Million coverage from 2013 to 2015 Who is gaining coverage? A lthough the Affordable Care Act (ACA) was passed in 2010, some of the biggest gains in insurance coverage have been seen in the period since January That is when new provisions for state Medicaid programs and subsidized insurance plans took effect and state marketplaces began operating. 1 According to the latest figures released by Health and Human Services, 16.4 million formerly uninsured people have gained coverage under the ACA since the law took effect. This number includes 2.3 million young adults (aged 19-25) who gained coverage between 2010 and the start of open enrollment in October 2013 due to the ACA provision allowing young adults to remain on a parent s plan until age Overall, the rate of uninsured US adults dropped from 20.3 percent in 2013 to 13.2 percent in 2015 a 35 percent reduction. 3 According to the report, 4.2 million Latino adults have gained coverage. The uninsured rate declined across the board among race and ethnic groups; however, Latinos gained the most with a 12.3 percent drop in the rate of uninsured from 2013 to Blacks had a drop of 9.2 percent during the same period and the uninsured rate among Whites declined by 5.3 percent. 4 According to the Commonwealth Fund, young Latino adults and Latinos with the lowest incomes have benefitted the most. 5 Yet, having historically had the highest uninsured rate, Latinos remain the group most affected by a lack of insurance coverage, with current estimates ranging between 25 6 to 29.5 percent. 7 1 Quealy, K and Sanger-Katz, M. According to the Pew Research Center, Latino immigrants are the hardest hit. Foreign-born Hispanics are more than twice as likely to be uninsured than US-born Hispanics, with 39 percent uninsured versus 17 percent. 8 Foreign-born Hispanics who are non-citizens have an even higher rate of being uninsured at 49 percent, with children being in particular risk. Foreign-born Hispanics younger than 18 are almost three times as likely to be uninsured than their US-born counterparts, with 34 percent being uninsured compared to 12 percent. 9 A variety of factors are cited as the cause for this disparity, including a lack of culturally specific outreach programs, language barriers, financial concerns, frustration with the complexities of the ACA, and fears that applying could jeopardize a family member s immigration status. 10 In addition, awareness of the health insurance marketplaces among Latinos who prefer to speak Spanish is significantly less than those who primarily speak English (35% vs. 65%). 11 Other vulnerable populations also remain. There are stark differences in the rates of uninsured Latinos among states that have expanded Medicaid and those that have not. Texas and Florida two of the states that did not expand Medicaid account for the largest proportion of Latinos who are uninsured, yet are potentially elgible for coverage. 12 In these and other states that have not expanded Medicaid, 3.7 million poor adults fall into a "coverage gap" and will likely remain uninsured. 13 In the absence of the expansion, these individuals remain ineligible for Medicaid and do not earn enough to qualify for premium tax credits to purchase Marketplace coverage, which begin at 100 percent of the federal poverty level (FPL). Hispanics account for about a quarter (24%) of uninsured adults in the coverage gap. 14 Despite making great gains, younger Latinos and those with lower incomes tend to be the ones who remain uninsured. Eighty percent are between the age of and 40 percent are under 35. Six out of ten uninsured Latinos have incomes below 138 percent of poverty. Poor Latinos who live in states that did not expand Medicaid are particularly at risk 58 percent of low-income uninsured Latinos live in these states. 15 To summarize, it is clear that Latinos have made great progress in gaining insurance coverage over the past two years. After decades of having the highest rate of uninsured, for many Latinos, it may be the first time that they have had access to health care. 4 4 Reducing Health Care Disparities Pfizer engages with local and national Hispanic leaders throughout the US to help address health care disparities that affect Latino communities. This section describes some of the specific activities Pfizer is taking in this regard. Leadership Engagement T o gain a better understanding of the health care issues and needs of the Hispanic community, Pfizer conducted a national leadership survey among attendees of the National Hispanic Caucus of State Legislators (NHCSL) in November Results of the survey identified two major obstacles that prevent Hispanics from obtaining health care services and treatment: access to care and cost, which were named by almost 40 percent of the respondents. Diabetes was ranked as the number one health issue among all people surveyed, followed by obesity and access to health care services. These issues were named to be the most critical after health care insurance coverage, at a time when messaging about the Affordable Care Act was becoming more ubiquitous in the Hispanic market. All respondents indicated that access to health care services was the most important way to improve health among the Latino community. Diversity in Clinical Trials Increasing Latino participation in clinical trials is one way to continue closing the gap in health care access for Latinos. By increasing participation, we ensure that new medicines that are developed are just as safe and effective for Latino populations as they are for other groups. This is vital as, more and more, new specialty medicines are being discovered to treat highly targeted patterns of disease. Pfizer is committed to improving access to clinical trial information for patients and to increasing the representation of diverse populations in its trial investigations. To that end, Pfizer has undertaken several initiatives to help increase diversity in clinical trials. Examples include: ww Pfizer launched Find a Trial, a suite of web tools for patients seeking information on clinical trials (pfizer.com/findatrial). Designed to complement existing resources such as postings on clinicaltrials.gov, every actively enrolling Pfizer study is also listed on Find a Trial with, in many cases, greater depth of information. ww In March 2015, Pfizer sponsored a Capitol Hill briefing with the Congressional Hispanic Leadership Institute to discuss the topic. A distinguished panel of doctors and experts from the FDA s Office of Minority Health, Morehouse School of Medicine, National Hispanic Medical Association, PhRMA and Pfizer presented to policymakers, community advocates and health care leaders. The effort served to educate and engage leaders on the importance of increasing diversity in clinical trials to help Latinos understand the clinical trial process and how important it is to ensuring effective treatments are available when needed. ww Pfizer is participating in the I m In campaign founded by the Pharmaceutical Research and Manufacturers of America (PhRMA) and the National Minority Quality Forum (NMQF) to improve health by increasing clinical trial participation among traditionally underrepresented populations. 6 6 Reducing Health Care Disparities Pfizer RxPathways From the Pfizer perspective, we believe anyone who is prescribed one of our medicines should be able to get access to that medicine regardless of their insurance status. I n 2014, as the health care landscape was rapidly changing, Pfizer introduced Pfizer RxPathways, formerly Pfizer Helpful Answers, a remodeled patient assistance program in response to changing patient needs. Access to medicines is a cornerstone of Pfizer s commitment to health care. For more than 25 years, Pfizer has offered prescription assistance programs to help eligible patients get access to their Pfizer medicines. Today, this assistance is provided through Pfizer RxPathways, which helps eligible patients get access to their Pfizer medicines by offering a range of support services, including insurance counseling, co-pay help, providing Pfizer medicines for free or at a savings, and more. During the past 5 years ( ), Pfizer s patient assistance programs have helped nearly 2.5 million uninsured and underinsured patients across the US receive over $7 billion worth of medicines for free or at a savings.1 Providing Valuable Resources Pfizer continues to support our partner organizations in their public education activities by regularly providing advocates with patient assistance resources and tools so that they can supply information to those in need about the services we offer. 7 Reducing Health Care Disparities 7 The Pfizer Corporate Affairs Multicultural Center of Excellence Over the past decade, Pfizer s outreach and work in the burgeoning multicultural communities of the US have been spearheaded by Pfizer RxPathways, in efforts to increase awareness of the company s patient assistance program and help the uninsured and underinsured get access to Pfizer medicines. The coordination of this has led to the development of the Pfizer Corporate Affairs Multicultural Center of Excellence (MCoE), a new centralized resource at Pfizer for external partners and Pfizer colleagues within the US to assist with multicultural efforts. The MCoE aims to raise the level of multicultural competency and engagement at Pfizer. By reaching out to diverse communities, the MCoE seeks to build strong partnerships with representative organizations, and drives Pfizer to connect and collaborate with these organizations. As a result the MCoE will help reduce and eliminate health disparities in the US among diverse patient populations. The MCoE seeks to partner with organizations that help address the following common goals: 1. Diversity in Clinical Trials 2. Health Care Reform 3. Disease Awareness 4. Access to Health Care and Medicines It has established a Multicultural Advisory Council that will help guide the group to enhancing its work and engagement with multicultural organizations throughout the US. 8 8 Reducing Health Care Disparities At Pfizer, we have learned that partnerships are often key to unlocking complex challenges. Pfizer has a long history and tradition of working with community based organizations and other partners to address health issues affecting communities across the globe. The NYLatinoHealthSummit@Pfizer was the first signature event of the newly formed Pfizer Corporate Affairs Multicultural Center of Excellence. It was held in March 2014 at Pfizer s World Headquarters in New York City in an effort to explore new partnerships and build new relationships that could facilitate solutions to health disparities affecting New York s Latino communities. The Summit brought together advocates, health care providers, policy experts, and community leaders to focus on issues in four major areas: disease prevalence, research, access, and policy. The specific topics covered in these four key areas during this inaugural event were diabetes and obesity, diversity in clinical trials, access to medicines through patient assistance programs, and policy issues related to the Affordable Care Act. After a series of presentations and discussions, a number of issues and factors that contribute to Latino health care disparities were identified. These ranged from complex issues such as poverty to education and health literacy. To work on solutions to these, Pfizer is supporting the Hispanic Federation to lead a Latino Health Working Group, which began in February 2015, to work toward developing sound policy positions to address the issues. 9 Reducing Health Care Disparities 9 Guiding Patients in Need to Better Health Working with Mayor Kasim Reed, the National Association of Hispanic Nurses (NAHN), and the National Black Nurses Association (NBNA), Pfizer is helping connect uninsured and underinsured people of Atlanta to affordable health services. The 2nd edition of the Atlanta Regional Health Guide provides a comprehensive list of health care resources including contact information and health tips. For prescription assistance, resources like Pfizer RxPathways are also included. The publication is one in a series of bilingual health guides, available in English and Spanish, that have been published through this collaboration since 2008, to help provide access to affordable health services for people who are uninsured or underinsured. Guides have also been published for Arizona, New Mexico, Texas, Miami, and the Bronx, NY. Promotores Outreach Program Since 2013, Pfizer RxPathways has been working with Día de la Mujer Latina to implement the Promotores Outreach Program as a train-thetrainer model to educate promotores from various states about the program. Together, Pfizer and Día de la Mujer developed a national training module for patient assistance resources that has been used in promotores training programs in Texas, Georgia, New York and Florida. Over 100 promotores have been trained, empowering a network of community health workers with the ability to share information about Pfizer RxPathways and other resources that provide access to medicines with patients in need. In addition to promotores trainings, Pfizer RxPathways has sponsored Día de la Mujer Latina to host health fiestas in local communities. Here, promotores engage with the community to educate them about a variety of health topics, including patient assistance resources. Events were held in Houston, Fort Worth, McAllen, El Paso, Corpus Christi, Austin, Miami, and Atlanta.
10 10 Outreach, Innovation and Recognition PFIZER HAS BEEN A LONG-TIME SUPPORTER of the Congressional Hispanic Caucus Institute's (CHCI's) annual conference and Hispanic Heritage Month activities. Pfizer engages with key opinion leaders and community leaders to address policy and social issues that are key to the continued advancement of Hispanics in the US. In 2014, Pfizer sponsored a workshop on Science, Technology, Engineering, Arts and Math (STEAM) at the annual CHCI conference. Partnerships That Make a Difference Through Pfizer RxPathways and the Corporate Affairs Multicultural Center of Excellence, Pfizer proactively seeks out partners across the country to help increase awareness of patient assistance, diversity in clinical trials, health care reform, and diseases. By working with patient advocates, key opinion leaders, community-based organizations, and health care provider groups, Pfizer reaches local communities across the US to help patients get access to the Pfizer medicines they need. THE CUBAN-AMERICAN NATIONAL COUNCIL (CNC) provides human services to persons in need from all racial and ethnic groups. During the past four years, Pfizer has partnered with CNC to support the Hispanic community through health educational efforts at a national level and locally in Florida and the New York tri-state area. Additionally, Pfizer has trained CNC s health advocates in Miami on the Pfizer RxPathways program and shared educational materials for them to distribute at its community events and yearly health fair. Pfizer partnered with CNC in 2014 by sponsoring the quarterly Matter of Balance/Asunto de Equilibrio, a series of health education workshops for seniors. PFIZER HAS PARTNERED WITH THE DOMINICAN WOMEN'S DEVELOPMENT CORPORATION (DWDC) to help educate community health workers from Hispanic areas, particularly in Northern Manhattan, by training promotores de salud from this organization and providing educational materials, which are distributed at their health events and community offices. The DWDC has identified the need for Latinas to organize around critical issues and seek solutions to systemic problems affecting their families and community. Their focus is on sexism and discrimination, high housing costs, low-achieving schools, high dropout rates, limited access to health care services, attacks on reproductive rights, high teenage pregnancy, and high poverty rates.
11 Outreach, Innovation and Recognition 11 PFIZER HAS TRADITIONALLY PARTNERED WITH THE NATIONAL ASSOCIATION OF LATINO ELECTED OFFICIALS (NALEO) to provide elected officials with policy information through the support of health related workshops held during their annual conference. With this partnership, Pfizer has continually kept Latino elected officials informed of issues important to both Pfizer and the community served by NALEO. In 2014, Pfizer supported the Educational Fund 's conference and was recognized as the workshop convener for a session on Crafting Strong Policies in Support of Building Healthy Communities, open to over 1,300 conference attendees including a long list of national and local government officials and school board members. PFIZER HAS SUPPORTED THE LEAGUE OF UNITED LATIN AMERICAN CITIZENS' (LULAC'S) annual conference throughout the US and Puerto Rico and in 2014, Pfizer participated by being the Presidential Sponsor of the event in New York City. Pfizer welcomed attendees at the Partnership Luncheon, where Angelica Wong, vice president of Pfizer Global Supply and executive director of the Pfizer Latino Colleague Resource Group, spoke on increasing access to health care for Latinos and the importance of working together to achieve that goal. The event connected Pfizer with more than 2,500 LULAC members, community leaders and consumers who congregated to learn about offerings from a variety of companies, civic, and governmental organizations, military and political groups and academics. In 2015, Pfizer will continue to work with LULAC to educate the Latino community about Pfizer RxPathways through participation in community health fairs. PFIZER HAS FOSTERED AN ENDURING PARTNERSHIP WITH THE NATIONAL COUNCIL OF LA RAZA (NCLR), considered the largest national Hispanic civil rights and advocacy organization in the US, by sponsoring their annual conference where relevant health related information is discussed as part of their capacity building sessions. As an organization, NCLR reaches millions of Hispanics each year in 41 states, Puerto Rico, and DC, with projects such as applied research, policy analysis, and advocacy, providing a Latino perspective to social, economical, and health related issues, and provides capacity-building assistance at the state and local level to advance opportunities for individuals and families. In 2014, Pfizer RxPathways sponsored the Annual Conference and participated in the Health Summit. Thousands of program materials were distributed to NCLR affiliates, conference attendees, and local consumers who visited the Pfizer booth during the Family Expo.
12 12 Outreach, Innovation and Recognition PFIZER, THROUGH ITS EFFORTS WITH THE NATIONAL ASSOCIATION OF SCHOOL NURSES (NASN), has distributed educational materials to thousands of nurses working with students' and their families' communities across the US. In 2015, Pfizer is planning to work with the organization to develop a new initiative that will engage students and school nurses on a community level so that both groups can have a deeper understanding of the Pfizer RxPathways program and be able to relay their knowledge about it to parents, family, friends, and other community members. PFIZER HAS CONTINUED TO GROW ITS PARTNERSHIP with the National Association of Hispanic Nurses (NAHN) through board participation, policy-building workshops, conference participation, webinars to their members, and support of educational activities held throughout the year, for both the national and local organizations. In 2014, Pfizer sponsored the 39th Annual Conference, where Pfizer colleagues presented an important plenary session on clinical trials and vaccines. Pfizer s connection also extends to the local chapters of NAHN through activities that include nurse trainings and chapter briefings on the Pfizer RxPathways program, as well as other topics. PFIZER S PARTNERSHIP WITH THE LATINO COMMISSION ON AIDS has strengthened ties and provided support to advocacy building in the Hispanic community, through its support of the annual Cielo Gala entitled Designing a World Without AIDS. The event recognizes leaders in the fight against HIV/AIDS and in the battle against Hepatitis C and reaches over 400 health care, business, and community leaders. EACH YEAR, THE NATIONAL HISPANIC COUNCIL OF STATE LEGISLATORS (NHCSL) awards the Corporate Leadership Award to a company that supports the mission of the organization. In 2014, Pfizer was the recipient of that award because of its policy and community efforts to increase health access for Hispanics. NHCSL provides a forum for information exchange and member networking; supports leadership training; acts as a liaison with sister US Hispanic organizations; promotes public and private partnerships with business and labor; and partners with Hispanic state or provincial legislators and their associations represented throughout the world.
13 Outreach, Innovation and Recognition 13 PFIZER BEGAN ITS INVOLVEMENT WITH THE CONGRESSIONAL HISPANIC LEADERSHIP INSTITUTE (CHLI) in 2009 to broaden its support of Congressional education and leadership initiatives and its engagement with Hispanic leaders on important health issues. In 2014, Pfizer sponsored the CHLI Congressional Briefing Series, focusing on 1) The Economic Situation in Puerto Rico; 2) Barriers to Prosperity for the Hispanic Community; 3) Net Neutrality vs. The Open Internet; and, 4) Our Wireless World: Where we are Today and Where are we Going? Pfizer also sponsored CHLI's annual gala and leadership awards. PFIZER AND THE NATIONAL HISPANIC MEDICAL ASSOCIATION (NHMA) HAVE BEEN PARTNERS SINCE the founding of the organization based on their synergistic goals to eliminate health care disparities affecting Latinos and improve the overall health of the community. Pfizer has consistently worked with NHMA to strengthen the organization and has participated in its annual conference by presenting on a number of topics, most recently on Key Health Policies Impacting Health Care Delivery to Latinos and Diversity in Clinical Trials. This partnership also addresses policy issues that will assist in empowering Hispanic physicians and other health care professionals to improve the health of Hispanic populations through Hispanic medical societies, resident and medical student organizations, and its public and private sector partners. PFIZER HAS A STRONG PARTNERSHIP with the Hispanic Federation supporting their health fairs, community outreach efforts, and annual gala. Additionally, Pfizer has participated with the Federation to bring Affordable Care Act (ACA) information to health advocates from its member organizations. Pfizer has also participated in briefing events around the ACA and how its own patient assistance program can help those in need. Together with the Federation, Pfizer launched the Latino Health Summit in 2014, which brought together advocates, public officials, health care providers, policy experts and community leaders from NY, focusing on health disparities. After this event, the Latino Health Working Group was formed under the direction of the Federation to continue efforts toward solving the issues that were identified.
