2 Cashless Claim Process Customer goes to Network Hospital with his health card Reimbursement Claim Process Customer gets admitted in Non-Network hospital for treatment & intimates the TPA or LVGIC within 24 hours of hospitalization Hospital verifies customer details & faxes the Cashless claim request to TPA TPA assess the Cashless claim request with the policy benefits & sends At Discharge Customer settles the bills fully & sends the claim documents to TPA within 15 days from the date of discharge Cashless Approval to Hospital Cashless Denial sent to Hospital Query Letter to Hospital Reimbursement Claim processed by TPA Patient Discharged after paying Non Medical / Inadmissible expenses Patient admitted and has to pay the bills Query Reply faxed by Network Hospital to TPA Claim Approved Claim Repudiated Query letter send to the customer TPA Receives Documents from Hospital & forward claim for Payment to LVGIC Patient can claim for Reimbursement Claim payment settled by LVGIC to the customer through Cheque / NEFT Repudiation Letter is sent to customer Customer sends the query reply to TPA LVGIC Process the Claim Payment to Hospital In Emergency hospitalization intimate the TPA/LVGIC within 24 hours of hospitalization or 24 hours prior to discharge from the hospital * In Planned Hospitalization Intimate the TPA/LVGIC at least 48 hours before hospitalization. In Emergency hospitalization intimate the TPA/LVGIC within 24 hours of hospitalization or 24 hours prior to discharge from the hospital * In Planned Hospitalization Intimate the TPA/LVGIC at least 48 hours before hospitalization. * Claim payments are made through online bank transfers only. Please submit cancelled cheque of the policy holder and relevant details as mentioned in Section G of claim form
