3 Dear Colleague, Welcome to the 14th Annual IHA Stakeholders Meeting! As Alternative Payment Model emerges as the latest buzzword and aspiration in the national dialogue on value, here in California, paying for value is the emerging reality. For the Value Based P4P program, this year marks the tipping point in the transition to value as the majority of P4P incentives to physician organizations will be value-based. It also marks the first time Total Cost of Care results will be made publicly available alongside the quality results. After early challenges in this year s collection of cost data, the Total Cost of Care results were successfully generated with the broadest representation of members yet, including for the first time Anthem, Kaiser Permanente, and Western Health Advantage. Sharing these results with a broader audience advances a more transparent and personal dialogue around value in healthcare. These accomplishments wouldn t have been possible without the effort, hours, and expertise of the P4P committees, as well as support provided from the IHA staff. We are pleased to share the progress that has been made! We are also excited about the outstanding speakers in today s agenda. We will hear from UnitedHealthcare, Blue Shield, and Cigna about their first years of measuring and paying for value with Value Based P4P. Later, breakout sessions will cover many topics relevant to improving quality and cost of care, including reducing overuse and variation, opportunities for alignment and standardization in performance measurement, and effectively managing total cost of care. We ll close with a panel discussing what it takes to achieve excellence in healthcare. In keeping with national priorities for health care, we are fortunate to recognize the accomplishments of the physician organizations that demonstrated not only the highest level of quality achievement, but of resource stewardship. We look forward to presenting the Excellence in Healthcare award to exceptional physician organizations that demonstrate strong performance in quality, patient experience, and Total Cost of Care. Additionally, we will recognize the physician organization in each P4P region that achieved the greatest overall quality improvement. Our heartfelt congratulations to all the organizations that work so hard to provide high quality, affordable care. In the session, the provider system partners will discuss their approaches to reducing overuse, and Consumer Reports will share information and resources aimed at supporting consumer and provider engagement in the Choosing Wisely initiative. An important method to identify that unwarranted care is variation analysis. Dr. van Duren will describe specific projects, goals, the measures used to drive change and the outcomes achieved. Participants will leave with an understanding of the role Variation Analysis plays in identifying and eliminating unnecessary care, and how to implement such programs. Michael van Duren, MD, Vice President, Variation Reduction, Sutter Health 2B: Effective Approaches for Managing Total Cost of Care Los Angeles (2nd Floor) This session will highlight a new Total Cost of Care playbook being developed by the California Quality Collaborative through a series of interviews with high value physician organizations from across the country. Torrance Memorial Health System will explain the approach and tactics they used for successfully managing Total Cost of Care. Suzanne Tinder Hansen, MHA, Health Care Strategic and Operational Consultant Carol Ecklund, RN, MN, Director of Medical Management, Torrance Memorial Health System Moderator: Jeff Rideout, MD, President and CEO, Integrated Healthcare Association 2C: Creating Meaning from Data Through Mapping La Jolla (2nd Floor) This session will provide an opportunity to explore a new data resource, and to understand how it can inform performance measurement and quality improvement at the plan, provider, and community level. IHA s online tool, HEDIS by Geography ( allows users to examine performance on six clinical quality measures and three resource use measures by product line (including HMO and PPO) as well as geographic unit (e.g. ZIP code, county, and Covered California region). Several plan and provider reactors will share their thoughts on how the tool and data can be useful in their work related to quality improvement and utilization management. 6 Parag Agnihotri, MD, Medical Director, Sharp Rees-Stealy Medical Group Dr. Parag Agnihotri currently serves as the Medical Director for Continuum of Care for Sharp Rees-Stealy Medical Group, San Diego. His work focuses on reforming the delivery of health care by building multidisciplinary teams to implement the three part aim for improving the health of the population. He is a California HealthCare Foundation Fellow. CMMI has designated him as an Innovation Advisor. Dr. Agnihotri is board certified in Internal Medicine and Geriatric Medicine. Khuram Arif, MD, MBA, Medical Director, Managed Care, Mercy Medical Group Dr. Arif completed his pediatric residency at SUNY Brooklyn and obtained an MBA from UC Davis. As a Board Certified Pediatrician he practiced for over 10 years at Mercy Medical Group. He functioned as Site Medical Director for 7 years, Department Chair Pediatrics for 5 years and helped usher in transparency of clinical quality metrics in that role. He then transitioned into the role of Associate Medical Director for Managed Care and currently serves as the Medical Director, Managed Care for Mercy Medical Group. Michael Belman, MD, MPH, Medical Director, Anthem Dr. Belman leads clinical strategy for new payment programs with a special emphasis on California. The key element of this initiative is the Enhanced Coordinated Care Program recently introduced for Anthem PPO members. Dr. Belman also supports the Anthem Enterprise Payment Innovation leadership team. Prior to Blue Cross, Dr. Belman was Director of Pulmonary Medicine at the City of Hope Medical Center until 1987 and subsequently Associate Director of the Pulmonary Division at Cedars Sinai Medical Center. Dr. Belman received his medical degree from the University of the Witwatersrand in Johannesburg, South Africa. Page 6

7 Sarah Brooks, MSW, Program Monitoring and Medical Policy Branch Chief, California Department of Health Care Services Sarah Brooks currently serves as the Program Monitoring and Medical Policy Branch Chief for the Department of Health Care Services (DHCS). Prior to joining DHCS, Sarah served as the Assistant Director of Policy for the California Association of Public Hospitals and Health Systems. While in this position she focused on health care coverage issues with a specific emphasis on the Medicaid and uninsured populations. Sarah earned her BSW at the University of Kansas and MSW from the University of California, Berkeley. Larry deghetaldi, MD, Division President, Palo Alto Medical Foundation, Santa Cruz Larry deghetaldi, MD is president of the Palo Alto Medical Foundation Santa Cruz. Dr. deghetaldi is a boardcertified family medicine physician who has served in a number of leadership positions within the Santa Cruz Medical Clinic since he began practicing in He represented the clinic in its affiliation negotiations with Sutter Health and additionally chaired the Clinic Management Board. Dr. deghetaldi has served on several boards of directors including the California Medical Association, Sutter Maternity & Surgery Center, and the Central California Alliance for Health. He is also a member of the IHA P4P Governance Committee. Carol Ecklund, RN, MN, Director, Medical Management, Torrance Memorial Health System Carol Ecklund is a dedicated nurse leader with 21 years of local and international nursing experience. Carol has experience in leadership, education, and advanced clinical practice areas including: Hospital, Ambulatory/ Outpatient, Managed Care, Accountable Care Organization, Community and Academic Health Systems. She has worked for Torrance Memorial Health System since 2008 and has held the positions of Oncology Clinical Nurse Specialist, Director of Medical Surgical & Critical Care Nursing and is currently the Director of Medical Management for Torrance Health IPA. Lindsay Erickson, MSPH, Manager, Value Based P4P Program, Integrated Healthcare Association Lindsay Erickson leads the Value Based Pay for Performance Program. In this role, she manages the day-to-day operations involved in measuring, reporting, and rewarding physician organizations for performance on quality, cost, and resource use. Prior to joining IHA, Lindsay worked at the Governor's Office of Planning and Budget for the State of Georgia, where she worked to develop and evaluate policy recommendations. Lindsay earned a Master of Science in Public Health with a concentration on health policy and services research from Emory University and a BS from UC San Diego. Ginamarie Gianandrea, Program Coordinator, Value Based P4P, Integrated Healthcare Association Ginamarie Gianandrea supports P4P program communications and operations, with a focus on managing the clinical quality measure set. She is also responsible for coordinating meetings and events. Prior to joining IHA, Ginamarie was the Project Coordinator for the California Joint Replacement Registry. Previously, she supported various programs as an administrator at the Pacific Business Group on Health. Ginamarie received a BA with honors in Women and Gender Studies from San Francisco State University. Page 7

8 Ann Marie Giusto, RN, Director, Variation Reduction, Sutter Health Ann Marie Giusto serves as the Director of Variation Reduction for Sutter Health. In this role, she directs a team across the system that engages physicians to discuss opportunities to address unwarranted variation and thereby increase quality and decrease costs. To date, over 400 projects have been launched with over $30 million in savings. Prior to joining Sutter Health, Ann Marie was the Vice President of Quality for the California Hospital Association and has held leadership positions at Safeway, PBGH, and Blue Shield of California, and consulted at acute care hospitals in California and New Mexico. She holds bachelor degrees in Economics from UC Davis and in Nursing from the University of San Francisco. Peggy Haines, RN, Vice President, Quality Management, Health Net Peggy Haines is a healthcare professional with over thirty five years experience. She has worked at Health Net for the last twenty years with progressive management experience and varied scope of responsibilities. For the last twelve years Peggy has been the Vice President of Quality Management with strategic planning and direction for the Clinical Quality department, Wellness and Health Education, Provider Credentialing, Provider Delegation Oversight, HEDIS, NCQA Accreditation and Medicare Stars program. Peggy graduated from Texas Christian University with a Bachelors in Nursing. Suzanne Hansen, MHA, President, Tinder Hansen Consulting, LLC Suzanne is President of Tinder Hansen Consulting, LLC, a Minnesota-based strategic and operational advisory firm for health care organizations. For over 21 years, Suzanne held various roles at HealthCare Partners including Vice President, Operations of a medical group and IPA delivery network that served over 200,000 patients in Southern California. In 2013, Suzanne joined Accretive Health as Senior Vice President and General Manager of the Population Health division that delivered solutions to integrated delivery networks making the transition from fee-for-service to value-based reimbursement models. Suzanne enjoys leading teams that create lasting solutions for the local medical ecosystem. Amy Nguyen Howell, MD, MBA, Chief Medical Officer, CAPG Dr. Nguyen is a board-certified family practice physician. She oversees all clinical programs at CAPG and supports advocacy work in Sacramento and Washington, D.C. She also contributes to NCQA s Clinical Programs Committee which provides guidance on provider quality measures and requirements. She has most recently served as the Chief Medical Officer at Easy Choice Health Plan, where she oversaw clinical direction of medical services. Dr. Nguyen graduated with honors from Howard University School of Medicine and earned her MBA from Cal State Fresno. Ruth Idah, MBBS, Director of Quality Management, PIH Health Physicians Ruth Idah is the Director of Quality Management at PIH Health Physicians in Whittier, California. Ruth has primary responsibility for quality management for the PIH Health Physicians Medical Group and IPA. Ruth oversees activities for patient safety and ambulatory quality management, credentialing, HCC/risk adjustment, incident reporting, regulatory quality programs including IHA P4P and CMS Stars. Ruth obtained her medical degree in Nigeria and a MPH from UCLA. Page 8

9 David Joyner, MBA, Chief Executive Officer, Hill Physicians Medical Group; Chair, IHA David is the CEO of Hill Physicians and PriMed, the management services organization serving Hill. David served as the Chief Operating Officer of Hill Physicians for two years before taking the CEO role. Previously, David served for fourteen years as an executive of Blue Shield of California. His most recent position at Blue Shield was senior vice president large group and specialty benefits where he oversaw $2 billion in revenues. David received his MA in chemistry from the University of Oxford and his MBA from The Wharton School at the University of Pennsylvania. Ron Kwok, MD, Diabetes Care Physician Champion, Sharp Rees-Stealy Medical Group Dr. Ron Kwok received his Bachelor s degree at UC Berkeley in 1991, his MD degree at the University of Chicago School of Medicine in 1995, and completed his Internal Medicine Residency at UC San Diego Medical Center in He has been working as a full time staff physician for Sharp Rees-Stealy Medical Group since 1998, and has been the physician champion for the Sharp Rees-Stealy Diabetes Management program since October of Sarah Lally, MSc, Project Manager, Integrated Healthcare Association Sarah Lally works on knowledge dissemination of key findings related to IHA s work and impact, policy analysis to support organizational strategy, grant development and management, and project work related to IHA s payment reform portfolio, maternity care, and California s safety net. Prior to joining IHA, she worked at the Royal College of Physicians of London on a lung cancer quality improvement project. Sarah earned a Master of Science in Public Service Policy and Management from King s College London and a Bachelor of Arts in Political Science from Villanova University. Page 9

10 Lance Lang, MD, Chief Medical Officer, Covered California Dr. Lang serves as Chief Medical Officer of Covered California, the state health benefits exchange. He also holds an appointment of Clinical Professor of Family and Community Medicine at UCSF. Most recently Dr. Lang served as Clinical Director for the California Quality Collaborative (CQC). His work at CQC included leading an Advanced Primary Care Practice Coaching Collaborative. His consulting practice also includes support for a clinical transformation project by Compass Oncology, a division of US Oncology in Portland. Janet Lundbye, MPH, Regional Vice President, Network Strategy, UnitedHealthcare Since 2009, Janet Lundbye has held various Network Management positions at UnitedHealthcare. Currently, she is the Regional Vice President of Network Strategy for the West Region for all lines of business within United. As such, Janet leads the expansion of value-based models designed to disrupt and reshape healthcare through new contract models, product changes and key provider network strategies and initiatives across the West Region. Janet has over 26 years of experience in the managed care industry in both California and Texas. Prior to United, Janet served in leadership positions at Anthem Blue Cross, Aetna and PacifiCare. Christy Mokrohisky, MBA, FACMPE, Vice President, Population Health, St. Joseph Heritage Healthcare Christy serves as the Vice President of Population Health Management at St. Joseph Heritage Healthcare where she leads program design, implementation and outcome measurement for population health. She works in partnership with payers and purchasers in value-based contracts to improve quality outcomes, engage patients and reduce the total cost of care. She oversees government and commercial health plan quality, risk adjustment and performance programs. Christy holds a BA and an MBA from Marquette University. Robin Morrisey, RN, NP, Manager, Outpatient Programs, Scripps Whittier Diabetes Institute Robin Morrisey is a registered nurse and manager of diabetes services at Scripps Health in San Diego. She has practiced nursing since 1993 and has specialized expertise in chronic disease management, diabetes care, clinical research, and healthcare operations. She received her Master s Degree in Nursing from the University of San Diego and is a board certified adult nurse practitioner and certified diabetes educator. Robin is a member of the American Diabetes Association and has presented at the national meetings. Her team recently received a Quality Award from Scripps for successful outcomes in a Diabetes Care Management Pilot. Khanh Nguyen, PharmD, Director, Clinical Support, Hill Physicians Medical Group Khanh Nguyen, PharmD, is the Director of Clinical Support for Hill Physicians Medical Group. Khanh manages quality improvement programs, clinical initiatives and medication utilization for the organization. She has led efforts in the development and implementation of innovative clinical programs and tools that improve patient outcomes, reduce physician variation and control healthcare costs. Khanh is a graduate of the University of California San Francisco, School of Pharmacy; her undergraduate studies were in Molecular and Cell Biology and Business Administration at the University of California, Berkeley. 11 Athena Philis-Tsimikas, MD, Corporate Vice President, Scripps Whittier Diabetes Institute Athena Philis-Tsimikas is the Corporate Vice President of the Scripps Whittier Diabetes Institute at Scripps Health in San Diego, California. She leads the Scripps Diabetes Care Line which creates programs and delivers diabetes services across 5 Scripps hospitals, 25 outpatient facilities and the community. She is board certified by the ABIM in the subspecialty of Diabetes and Endocrinology and is an Associate Clinical Professor of Medicine at UCSD s Division of Endocrinology and Metabolism. She has devoted her more than 20-years of experience to research and clinical efforts improving diabetes assessment, education and management. Jeff Rideout, MD, President and CEO, Integrated Healthcare Association Jeff Rideout is President and CEO of the Integrated Healthcare Association (IHA). He is responsible for management of all IHA programs and activities, including the Value Based P4P program, performance measurement and payment innovation initiatives, and efforts to enhance the adoption of health information technology and improve healthcare affordability. Prior to IHA, Dr. Rideout was Senior Medical Advisor for Covered California. Dr. Rideout also holds academic appointments with Stanford and the UC Berkeley Haas School of Business. Dr. Rideout completed his residency training in internal medicine at UCSF. He received his medical degree from Harvard Medical School and his undergraduate degree from Stanford University. Tory Robinson, Senior Manager, Quality Improvement, Blue Shield of California Tory Robinson leads Blue Shield of California s Quality Improvement efforts for the HMO and ACO lines of business. As part of this role, she drives strategy and day-to-day operations for BSC incentive programs, including Value Based Pay for Performance. Prior to joining BSC, Tory held leadership roles in performance and quality improvement with Kaiser Foundation Health Plan in Northern California, where she focused on Home Health, Hospice, Palliative Care, Skilled Nursing Facilities and Behavioral Health. She is an expert in identifying critical measures for performance improvement, as well as the design of large scale, collaborative process improvement efforts. Tory graduated Cum Laude from Yale University, with distinction in her major. Beccah Rothschild, MPA, Senior Outreach Leader, Consumer Reports Beccah Rothschild is the Senior Outreach Leader for the Choosing Wisely campaign at Consumer Reports. She is responsible for leading the consumer-facing component of the campaign, specifically with Choosing Wisely grantees. The Choosing Wisely campaign provides resources for consumers and physicians to engage in important conversations about the overuse or misuse of medical tests and procedures. Prior to joining Consumer Reports, Beccah was the Director of Health Literacy Projects at Health Research for Action, a center in the UC Berkeley School of Public Health. Shelley Rouillard, Director, California Department of Managed Health Care Shelley Rouillard is responsible for protecting consumers rights and ensuring a stable health care delivery system in accordance with federal and state rules. Rouillard came to the DMHC with more than 20 years of experience as a health care consumer advocate. She has an extensive background in government and health care, including health care policy, provider contracting, network development, and health care quality performance. Rouillard holds a bachelor of arts degree in social work from Rutgers University. Page 11

12 Michael van Duren, MD, Vice President, Variation Reduction, Sutter Health Michael van Duren, MD, MBA, serves as Vice President of Variation Reduction for Sutter Health. Dr. van Duren uses a grassroots approach to engage physicians in analyzing practice patterns to reduce variation, increase quality and lower costs. Under his leadership, the clinical variation reduction program has grown to more than 400 projects and $30 million in savings. He also trains clinicians in leadership, change management and the art of communicating with patients. Dr. van Duren serves as co-chair for CAPG s Clinical Quality Leadership Committee and is the Chair of IHA s Technical Payment Committee. Peter Welch, President, CIGNA HealthCare of Northern California and Pacific Northwest Peter Welch is President and General Manager for Cigna's Northern Pacific markets which span the Northern California region and the Pacific Northwest states including Washington, Oregon, Idaho and Alaska. Prior to joining Cigna in 2000, Peter held a variety of leadership positions at United HealthCare and The Travelers Insurance Company. He currently serves on the Executive Committee for the California Association of Health Plans (CAHP), a trade association representing the largest California Health Plans. Manager, Performance Measurement The Performance Measurement Manager will report to the Senior Vice President, Programs and Policy, and will serve as a key resource and expert on quality and resource use performance measurement. The candidate will spend the majority of his/her time supporting the Value Based P4P program, but may also contribute to other IHA performance measurement activities and projects. Senior Project Manager The Senior Project Manager will report to the Senior Vice President, Programs and Policy, and will support assigned projects and grants, managing the strategy, project plan, and deliverables for successful project completion. Responsibilities include program development and implementation, knowledge translation and dissemination to share key findings from IHA's experience working on delivery system alignment and payment reform, and policy analysis to guide organizational strategy. Prior to IHA, she co-directed the American Institutes for Research s Health Policy and Research Group, a team of over 70 health services research professionals.
