1 Right Place, Right Time Providing less restrictive options for mental health care in the community: Progress of Step Up Step Down facilities in Metro North Metro North Health Forum 2017 Presented by Shirley Anastasi, Manager Governance 31 st May 2017
2 What is a Step Up Step Down facility? Based on Victorian Health Prevention and Recovery Care (PARC) service model (adult and youth) Designed to provide services for people who are becoming unwell and people who are in the early stages of recovery from an acute illness and need a short period of additional support and consolidation. The pathways to enter a SUSD facility can be described as a step up from the person s place of residence or step down from an inpatient unit. Least restrictive, recovery focuses care in the community, alternative to admission.
3 Model of Care Delivered by a combination of clinical and non-clinical staff and relies on a collaborative partnership between MNMH and an NGO. Access to the service is on a voluntary basis, consumers years of age who: No longer require acute inpatient level clinical intervention and treatment but would benefit from short term intensive treatment and support in a residential setting post-discharge from an acute inpatient admission (stepdown); or Require short-term residential support to prevent further deterioration or relapse, which in the absence of this option may lead to admission to an acute mental health in-patient unit (step-up) Consumers must be currently receiving active treatment with MNMH to be eligible for placement. It is anticipated that the length of stay will vary depending on consumer need from days to weeks (28 days maximum, average 14 days).

Needs analysis, architectural design completed by Brisbane Housing Company 2014/2015 PHN Innovation funding MNHHS Health Service Strategy Some patients are either admitted to our hospitals or remain in our hospitals when they could be better supported in less restrictive environments in the community. commits to: provide an alternative to hospital admission and support the recovery of consumers with a mental health diagnosis in the community through a Step up/ Step-down model of care. August 2015, MNHHS Board provided approval to develop existing site at Nundah as a SUSD facility.
5 Where have we been. MNMH has identified the need for 3 SUSD facilities. Nundah purpose built facility 2 others Northern (Red/Cab) and CBD October December 2016 opportunity to go out to market to invite NGO s to tender to provide a building and non-clinical operational staffing for two additional facilities Challenges: Short time frame for NGO s to identify a property and develop a tender Building to suit QH requirements Costs of refurbishment Outcomes: Unsuccessful proposals for CBD solution Continuing to negotiate with a preferred tender for a Caboolture location

8 What it looks like day to day MNMH clinical staff business hours Mon-Fri, including P/T Consultant Psychiatrist and Registrar who have clinical governance responsibility MNMH TL and NGO Service Manager Mon-Fri NGO non-clinical staff 7 days includes peer support workers NGO non-clinical staff overnight MNMH Mental Health Clinicians maintain case management responsibility and provide in reach to consumers NGO non-clinical provide programs within SUSD setting, links with community programs, and provide outreach follow up as relevant when a consumer is discharged Consumers are actively supported to continue with their recovery plan and transition to return to their usual residence Communal activities will be encouraged e.g. psycho education groups, healthy lifestyles programs, shared meal preparation/eating
9 Nundah facility Where are we now Tender for NGO non-clinical staffing and operational management out closed 29 May 2017 Hoping to finalise by end of July 2017 Construction delays have moved the commencement date from 1 October 2017 to approx. late November/early December 2017 Model of Service (MOS) and Furniture, Fixtures and Equipment User Group with consumer/carer input Awaiting NGO partner to come on board to develop MOS, staffing requirements, recruitment Will bring on QH and NGO staff at least one month prior to do training, set up processes etc Naming of the new facility we have gone full circle - Nundah House, Nundah PARC, SPA, CPR, Nundah House 9
