1 Service Re-design consultation 2 nd December th February 2014
2 GP Appointment with Service User Primary Care Step 1: Sandwell GP s will make a referral into BCPFT s Secondary Care Mental Health service (via the Primary Care Hub) to a Single Point of Referral One telephone number BCPFT s Single Point of Referral Secondary Care Step 2: Following a referral the Trust will either manage a face to face or telephone triage of the Service User depending on need and presenting risk. A team of senior clinicians will carry out an initial assessment enabling a care plan to be established detailing the service users immediate care and support needs, this would include: Needs assessment Risk assessment Crisis management Clustering, care pathway management Once accessed a Service User will either be supported by a multidisciplinary Specialist Community Mental Health Treatment Team or will be admitted to an inpatient service
3 Needs Assessment completed from Single Point of Referral Specialist Community Mental Health Treatment Team Inpatient Services Edward Street, Hallam Street, McArthur Unit Step 3: Multi disciplinary teams will allocate resources at the point of referral meeting the needs assessment of each service user, delivering outcomes and interventions quicker. Working towards recovery and discharge where possible and working in partnership with Primary Care link workers Step 4: Inpatient services will continue to be managed from Edward Street (Older Adults) and Hallam Street (Adults), McArthur (PICU). All inpatient services will be gate-kept by Home Treatment.
4 Key Benefits proposed from Primary and Secondary Care changes to referrals: Clear and robust care pathway across the adult lifespan leading to a more seamless approach in service delivery. Greater clarity around acute admission and discharge criteria (Clinical management of gatekeeping inpatient beds). Reduction of inappropriate admissions through improved community team crisis management.
5 Key Benefits proposed from a Single Point of Referral: An initial Point of Referral resourced with trained expertise to effectively and accurately support complex needs Introduction of out-of-hours services across the lifespan, and shift of care closer to home. An effective triage system which can establish an initial care plan and allocate resource to meet the support needs of the service user.
6 Key Benefits proposed from a Specialist Multi-Disciplinary Mental Health Treatment Team: Workforce development opportunities, multi-disciplinary approach Improved case management as a result of improved staff competencies and capacity. Efficient use of resources to support potential growing demand Greater partnership working and integration between teams, removing communication and administrative barriers. Solid foundation for preparing the care pathway for the MH tariff (Payment by Results). Greater clarity and purpose around the role and importance of community teams.

Feedback from this engagement will help to finalise the operational service delivery plans and working protocols.
