2 New Team Members Welcome KATY LANIER, RN Katy recently joined BJISG on May 22, She comes with four years of nursing experience. She recently moved to Ludowici from Cornersville, TN when her husband was stationed at Fort Stewart. Katy and her husband recently had a daughter, Kyleigh, who is 4 months old. Their two German Shepherds are also excited about living in Georgia. SHELBY EDWARDS, LPN Shelby comes to us with 3 1/2 years of nursing experience, most recently with Coastal Manor Nursing Home. She just graduated from South Georgia College with her Associates in Nursing and will be taking the NCLEX (RN exam) soon. Shelby is originally from Ludowici and has a 6 year old little boy that keeps her very busy! Welcome to the team, Shelby! SKYLER CREWS Skyler is a 2017 graduate of Brantley County High School and is very excited to be a part of the Bone & Joint Institute of South Georgia. 3 JULY 4th JULY 4TH - VOLUNTEERS NEEDED July 4th falls on a Tuesday and all our clinic locations will be closed. However, as you are aware, it is not uncommon for this holiday weekend to result in a number of orthopaedic injuries. To maintain access to orthopaedic services we plan to be open with a skeleton crew on Monday July 3rd, and are looking for volunteers. Our needs are as follows: Jesup Clinic Location: -Provider -Nurse -Med Assistant -Xray Tech -Checkin/Checkout -Phone Room x2 -Business Office x2 Waycross Clinic Location: -Provider -Nurse -Med Assistant -Xray Tech -Checkin/Checkout We will need approximately fourteen (14) volunteers, if possible. Please Amanda Dowdy, Nancy Freeman and myself if you are willing to volunteer to work on Monday July 3rd, Thanks. 5 OSHA Compliance A Guide to Compliance with OSHA Standards The mission of the Occupational Safety and Health Administration (OSHA) is to save lives, prevent injuries, and protect the health of America s workers. As part of the Department of Labor, OSHA and the states that operate OSHA-approved state plans establish guidelines and standards to promote worker safety and health that apply to every workplace in the United States, including medical practices. This information provides only a glimpse of the most frequently found hazards in medical offices. Many other standards may apply. BLOODBORNE PATHOGENS STANDARD (29 CFR ) This is the most frequently requested and referenced OSHA standard affecting medical offices. Some basic requirements include: A written exposure control plan, to be updated annually. Use of universal precautions Consideration, implementation, and use of safer, engineered needles and sharps Use of engineering and work practice controls and appropriate personal protective equipment (gloves, face and eye protection, gowns) Hepatitis B vaccine provided to exposed employees at no cost Medical follow-up in the event of an exposure incident Use of labels or color-coding for items such as sharps disposal boxes and containers for regulated waste, contaminated laundry, and certain specimens. HAZARD COMMUNICATION (29 CFR ) The hazard communication standard is sometimes called the employee right-to-know standard. It requires employee access to hazard information. The basic requirements include: A written hazard communication program A list of hazardous chemicals (such as alcohol, disinfectants, anesthetic agents, sterilants, mercury) used or stored in the office A copy of the Material Safety Data Sheet (MSDS) for each chemical (obtained from the manufacturer) used or stored in the office IONIZING RADIATION (29 CFR ) This standard applies to facilities that have an x-ray machine and requires the following: A survey of the types of radiation used in the facility, including x-rays Restricted areas to limit employee exposures Employees working in restricted areas must wear personal radiation monitors such as film badges or pocket dosimeters Rooms and equipment may need to be labeled and equipped with caution signs EXIT ROUTES (29 CFR Subpart E , , , and and ) These standards include the requirements for providing safe and accessible building exits in case of fire or other emergency. It is important to become familiar with the full text of these standards because they provide details about signage and other issues. OSHA consultation services can help or your insurance company or local fire/police service may be able to assist you. The basic responsibilities include: Exit routes sufficient for the number of employees in any occupied space A diagram of evacuation routes posted in a visible location. ELECTRICAL (Subpart S-Electrical 29 CFR to 29 CFR ) These standards address electrical safety requirements to safeguard employees. OSHA electrical standards apply to electrical equipment and wiring in hazardous locations. If you use flammable gases, you may need special wiring and equipment installation. In addition to reading the full text of the OSHA standard, you should check with your insurance company or local fire department, or request an OSHA consultation for help. 6 OSHA Compliance A Guide to Compliance with OSHA Standards REPORTING OCCUPATIONAL INJURIES AND ILLNESSES (29 CFR 1904) Medical offices are currently exempt from maintaining an official log of reportable injuries and illnesses (OSHA Form 300) under the federal OSHA record keeping rule, although they may be required to maintain records in some state plan states. If you are in a state plan state, contact your state plan directly for more information. All employers, including medical offices, must report any workrelated fatality or the hospitalization of three or more employees in a single incident to the nearest OSHA office. Call (800) 321-OSHA or your state plan for assistance. HELPFUL RESOURCES OSHA makes every effort to make information about its regulatory requirements readily available to the public. The full text for each standard in this brochure is available on the OSHA website at You can search for a specific subject by using the alphabetic index near the top of the home page or by clicking on Laws and Regulations under Compliance Assistance, then clicking on OSHA Regulations (Standards- 29CFR). 10 Important Information REVENUE CYCLE: DENIALS - You all can assist in trying to decrease the number of denials we are seeing by improving on our coding in the clinic. I feel as though this is important information to pass along to you so that we can hopefully see improvement in these trends in the coming months. When a visit or procedure is denied, a great deal of time is spent trying to get these resolved by either resubmitting with the corrected information or providing documentation showing medical necessity. The billing team also corrects a lot of modifiers on the back side that are not uniform across the board for all of the different payers. This is a very overwhelming task! AREAS FOR IMPROVEMENT: When a closed treatment is charged for a fracture, a -57 modifier (decision for surgery) should be applied to office visits. All other procedures, including injections, a -25 modifier should be applied to the office visit. When a patient is in a global period from either surgery at the ASC or hospital or a fracture, use a (no charge office visit). They are in a 90 day global. If they are seen for a different problem, an office visit can be charged with a -24 modifier. For arthrograms, (ex hip arthrogram) indicate laterality (left or right) on the CPT code for the injection. Office visits are being requested for EMG s. They are asking for a diagnosis code to support doing a bilateral procedure and documentation for medical necessity. Some of the EMG s are being denied based on NO medical necessity for the opposite side. I am looking into this further and will provide more information soon. At this point, do not change the process of ordering bilateral studies if that is what the providers want. If the patient is having symptoms on both sides, then there needs to be a diagnosis code in the chart for both sides.
