1 Carolinas HealthCare System Job Description Title: Certified Nurse Practitioner for VAD/Heart Transplant Job Code: Effective Date: JOB SUMMARY: The Certified Nurse Practitioner (NP) provides comprehensive primary care services, acute care services, and extended care services in the area of nursing and medicine for individuals, their families, and communities. This care is provided within the NP's specialty area(s), settings, and scope of practice. The nurse practitioner may assume the roles of provider, clinical expert, educator, administrator, researcher, case manager, and consultant. Licensure is held jointly by the state Board of Nursing in which employed and the Board of Medicine. NP s have prescriptive authority in the state of North Carolina under a practice agreement with a supervising physician. The Nurse Practitioner will support the clinical operations of the with primary focus on clinic services for heart transplant and VAD patients from evaluation, treatment, facilitation of the transplant or VAD event, follow up care and patient teaching. This practitioner will be responsible for coordinating delivery of patient care in collaboration with the Transplant Surgeons with supervision by the Transplant Administrator. ESSENTIAL FUNCTIONS: 1. Assesses patients health status by eliciting appropriate medical, family, and social history, performs complete or system specific physical examination as appropriate, compiles and orders additional objective information such as laboratory/radiology/exams as appropriate per protocol. 2. Coordinates comprehensive pre-transplant clinical evaluation of transplant candidates, including initial history and physical examination, referrals to other health care providers for consultation and testing and patient education pertaining to transplantation. 3. Interprets existing health data to identify wellness needs and risk factors related to chronic and acute disease processes; assesses human responses to actual and potential health problems. 4. Prescribes a variety of modalities to meet wellness and illness needs including medical, nutritional, physical, and traditional nursing treatments. 5. Facilitates high quality, cost effective, patient focused care or family centered care.

2 Page 2 of 4 6. Monitors the transplant plan of care and revises as necessary. Implements a monitoring system to ensure current clinical data is available for VAD patients or patients on the transplant waiting list. 7. Collaborates with Transplant/VAD Surgeons and Physicians. 8. Appropriately documents all patient visits, phone contacts, correspondence and progress in medical record. 9. Selects interventions appropriate to age, growth and development level when planning and performing care. 10. Provides age appropriate health counseling and support to transplant/vad population. 11. Provides Transplant and VAD education to referral sources, satellite clinics and staff members in clinical areas where VAD care might be provided as appropriate. 12. Participates in development and revision of policies, procedures, and protocols to meet standards of care. 13. Participates in quality improvement activities in accordance with the mission of Carolinas HealthCare System. 14. Assures open and clear communications with patients, their families and the health care team members. 15. Provides an environment conducive to optimal patient care and personnel safety in compliance with all local, state, federal, and institutional guidelines. 16. Evaluates personal performance by monitoring the responses from patients, their families, support personnel, health care team members, and the general public. In addition, further evaluate performance by comparing actual outcome to expectations and regulation. 17. Assists with surgical implantation of all ventricular assist device types. 18. Rounds daily on VAD patients and directs the daily management through coordination with the cardio-thoracic surgeon and cardiologist. 19. Coordinates and manages comprehensive clinical care of the patient through all phases of care, including post discharge. 20. Provides all transplant or device education and teaching to patient and family, identifying a support person to assume responsibility for the care post discharge.

3 Page 3 of Provides education, orientation and annual competency training to all facility staff who may provide care to the device patient population. 22. Collects, records and maintains VAD database. 23. Actively participates in VAD and Transplant QAPI programs. 24. Responds to call 24 hours per day when assigned. 25. Participate in Program meetings, projects and events. MARGINAL FUNCTIONS: 1. Attend appropriate clinical and research seminars and conferences. 2. Maintain involvement in professional organizations. 3. Review and maintain currency of related professional literature. 4. Participate in the research and publishing of new initiatives and groundbreaking treatment intervention. 5. Perform other related duties as assigned. PHYSICAL REQUIREMENTS: Fast-paced clinic and inpatient setting. Involves standing, walking and bending. EDUCATION, TRAINING, AND EXPERIENCE: Graduate of an Accredited School of Nursing, Master of Science in Nursing. Graduate of an Accredited Nurse Practitioner Program. Certification in accordance with specialty area by a nationally recognized accrediting body. Maintains national Nurse Practitioner certification. Maintains prescriptive authority. Current NC Board of Nursing License. American Board of Transplant Coordinator Certification. (If applicable) CPR Certification Minimum of 5 years nursing of which experience will include a combination of end stage organ disease, transplant care/exposure and/or outpatient transplant setting or as a nurse practitioner as determined by the administrator/medical director of specialty area.
