Our Children s Services Team and Developmental Services Ontario agency have both assumed additional roles related to the good work that they do. The Passport Program continues to experience unprecedented growth as the result of substantial investments by the Ministry of Community and Social Services. March 31, 2018, marked the final day of Liz Prendergast s tenure as the Executive Director at CSCN. Liz has been the senior leader at CSCN since the agency s inception. On behalf of the Board of Directors I thank Liz for her steadfast commitment to the agency and, more importantly, to the individuals and families we serve. Liz s presence at the agency and in the board room will be sorely missed. Earlier in the year Judy Howard announced her retirement from the agency. Judy was the Manager of Finance and Administration at CSCN since the agency opened its doors. We thank Judy for her 25+ years of dedicated service to the agency. We wish Liz and Judy many years of good health and happiness in their retirement. I thank my Board colleagues for their dedication and support this past year. You each bring a unique set of skills and talents to the board room. Your loyalty to the organization and to the work we do as a Board is commendable. 2 P a g e /2018 AN N U A L R E P O R T Executive Director s Message As we close out operations for , and review the year in retrospect, there have been many challenges and accomplishments. In all program areas there have been policy enhancements and some additional resources aimed at addressing the tremendous demand for service. In the Children's Services area, the provincial government has enhanced their focus and commitment to responding to children and youth with special needs. In Developmental Services, both the application process through Developmental Services Ontario and the Passport Program were augmented through an increased focus on policy and enhancements to funding for services and supports. The new investments in Developmental Services are yielding increased access to service for those individuals who are Passport Program attempting to enter the service system. As ever, the Staff and Management teams at CSCN have reacted with flexibility and competence when facing the many changes that occurred throughout the year - always keeping the focus of all that they do on the people we serve. Our Community partners have continued to work collaboratively to flex their capacity in order to respond to the ever increasing demand for services and support. Their partnerships within the sector have been a valuable ingredient to many much needed responses to people in dire need. Our Board of Directors has continued to work tirelessly to provide fiduciary oversight and support to the organization through a number of transitions. The Boards unwavering support and expertise contributes to strengthening the organization and our ability to provide quality services to individuals and families. On a personal note, I wish to express my sincere appreciation to the Staff, Managers, Board Members, Community partners, and last but not least, the people involved in our work for their amazing commitment and tireless efforts to make a difference. I begin my retirement from this work with confidence that the strong work ethic and alignment with the agency's values will ensure continued success in all that is undertaken. Liz Prendergast Executive Director The Passport Program is a program funded by the Ministry of Community and Social Services (MCSS) that helps adults who have a developmental disability be involved in their community and become as independent as possible. Funding can be used to provide the supports necessary to assist individuals with developing social skills and networks, teach activities of daily living, and to participate more fully and independently in their community. Passport Funding can also provide caregiver respite for primary caregivers of an adult with a developmental disability. Funding can also be used to cover costs related to Person-Centred Planning. The Passport program launched In 2005 and has grown significantly. In the inaugural year CSCN worked with 54 passport funding recipients and in 17/18 it grew to 3,517 funding recipients. In 2017/18, new Passport funding approvals were initiated in July 2017, that continued until March 31, This equated to 242 funding approvals that totaled $4,806,900 in new funding in the South West Region. With the introduction of Bill 148, the Fair Workplaces, Better Jobs Act, 2017, each Passport recipient received a 15% increase to their Passport allocation. In fiscal 2017/18, recipients received a pro-rated increase for 3 months for the period from January 1, 2018 to March 31, This 15% increase amounted to $5,945,845 in additional funding in the South West Region. At the conclusion of the 2017/18 fiscal year, 3,517 individuals were receiving Passport funding across the South West. This represents a 2.1% increase over the number of people in receipt of Passport funding at the end of the previous fiscal year (3,444). On March 31, 2018 Passport funding approvals in the South West Region totaled $48,304,840. This represents a funding increase of more than 25% over the previous fiscal year ($38,651,505). Funding approvals for individuals are based on information collected by Developmental Services Ontario using both the Application for Developmental Services and Supports (ADSS) and the Supports Intensity Scale (SIS). Funding approvals are based solely on the unique support needs of each individual. At the conclusion of the fiscal year 2,555 people remained on the Passport waiting list in the South West (a 19% increase over ). Some of these individuals are waiting for a first time approval, others are eligible for an increase in Passport funding as the result of a significant change in circumstances. 3 C o m m u n i t y S e r v i c e s C o o r d i n a t i o n N e t w o r k P a g e 3 D e ve l o p m e nt a l S e r v i c e s O nt a r i o S o u t h We st R e g i o n Developmental Services Ontario (DSO) agencies are the single point of access to adult developmental services funded by the Ministry of Community and Social Services (MCSS), under the Services and Supports to Promote the Social Inclusion of Persons with a Developmental Disability Act, There are nine DSO agencies in Ontario, one in each of the former nine Regions of MCSS. DSO South West Region is a division of the (CSCN). The DSO agency operates out of five locations in the South West Region. Offices are located in London, Owen Sound, Sarnia, Stratford, and Windsor. In 2017/18, DSO agencies received additional resources to introduce dedicated Service Navigators to their staffing complement. This equated to 3 dedicated positions in the South West. The role of the Service Navigator is to assist people with developmental disabilities and their families/caregivers by providing service navigation and information about MCSS developmental services, and other community resources. This support will be available throughout the service pathway from intake to application to accessing MCSSfunded services, including supporting people during changes in need/circumstances and while waiting for ministry-funded services. As a result of an additional investment by MCSS in Housing Coordination we were able to increase our complement of Service Navigators to 4. Each of these staff now perform a Housing Coordination function, in addition to their dedicated Service Navigation role. The role of Housing Cooordinators is to provide information about community resources, housing, agencies and groups that support people exploring innovative options. Year-End Data Summary During the fiscal year, 1,090 application requests were received by the DSO agency in the South West Region. 783 Application Packages were completed during the same fiscal year. 64 applicants were matched to a group living support via the DSO agency in 2017/ people accessed the services of an Adult Protective Service Worker, 45 people were linked to Supported Independent Living, 189 people accessed employment services, and 47 people accessed a respite service. For additional information about Developmental Services Ontario and the application process, please visit: or C h i l d re n S e r v i c e s Ontario Special Needs Strategy In February 2014, the Ministries of Children and Youth Services, Community and Social Services, Education, and Health and Long Term Care launched the Ontario Special Needs Strategy. The strategy s stated vision is: An Ontario where children and youth with special needs get the timely and effective services they need to participate fully at home, at school, and in the community as they prepare to achieve their goals for adulthood. CSCN was selected to become the Coordinating Agency for Coordinated Service Planning (CSP) for the service delivery areas of Elgin, Oxford, London-Middlesex, Huron, and Perth, under the Special Needs Strategy. In 2017/18, work was underway to develop this new support for families. Phase One of the CSP began in October 2017, with four Service Planning Coordinators (SPC) recruited by CSCN. CSP officially launched February In fiscal , 45 children were supported with coordinated service plans. Late in 2017, the Ministry of Children and Youth Services announced funding to introduce Fetal Alcohol Spectrum Disorder (FASD) Workers to the service system. Three FASD Workers were recruited by CSCN, one Worker has been assigned to work in each of the following service delivery areas: London/ Middlesex, Huron-Perth and Oxford-Elgin. FASD workers are aligned with each of the Coordinated Service Planning agencies. The FASD worker role is to deliver child, youth and family-centered service to children and youth with FASD and/or suspected FASD and their families, by leading the development of a support plan Children s Services Data New Referrals 556 Local Resolution Conferences 185 Service System Resolution Conferences 120 WrapAround Families 39 Plans with Transitional Aged Youth 31 Complex Special Needs Families 94 Developmental Services Ontario

These services allow them to live, work and participate in a wide range of activities in their communities. Community agencies deliver most of the available services and supports. The Ministry of Children and Youth Services (MCYS) is working with government and community partners to develop and implement policies, programs and a service system that helps give children the best possible start in life, prepare youth to become productive adults and make it easier for families to access the services they need at all stages of a child's development.

The key function of CSCN is to coordinate access to various services and supports for children and adolescents with complex needs who may require a response from more than one service provider. Participation in the CSCN process is required for children with a developmental disability who are pursuing accommodation supports and for children and adolescents seeking admission to a residential based treatment program or to the WrapAround process. Our role is to link individuals and families to appropriate services and supports in their community. The work of CSCN is intended to augment not replace the good planning that is done by our service provider colleagues across the area. The Children s Services division of CSCN serves a five county catchment area that includes: London-Middlesex, Oxford, Elgin, Huron, and Perth Counties. Developmental Services Ontario South West Region is a division of the. CSCN is the Passport Program delivery agency, across the former Ministry of Community and Social Services South West Region. The DSO and Passport divisions of CSCN serve the following communities: Bruce, Chatham-Kent, Elgin, Grey, Huron, London-Middlesex, Oxford, Perth, Sarnia-Lambton, and Windsor-Essex.
