3 MEDICATION SAFETY DURING TRANSITIONS OF CARE: A TOOLKIT FOR SENIOR CARE PHARMACISTS This publication is the product of the Medication Safety and Transitions of Care Workgroup as part of the Innovations and Business Development Committee Structure of the American Society of Consultant Pharmacists (ASCP), a non-profit professional society. ASCP is dedicated to empowering pharmacists to enhance quality of care for all older persons through the appropriate use of medication and the promotion of healthy aging. This document has been developed as an educational tool to be used by consultant and senior care pharmacist members of ASCP, however, is a valuable resource for all pharmacists caring for seniors in many various practice settings. The toolkit is indeed, a resource and guide for pharmacists who can have a positive impact on outcomes as they guide seniors and their families safely through care transitions. Seniors and their families are important partners for the Pharmacist engaged in care transitions and so too are the many other members of the care team. The information included herein, is intended to enhance the pharmacist s ability to practice successfully in this environment. This toolkit, sponsored by the Board of Directors of the American Society of Consultant Pharmacists, was developed over a two-year period by a volunteer workgroup of senior care pharmacists representing multiple areas of consultant and senior care pharmacy practice. Workgroup membership included representation from multiple university colleges of pharmacy, health information technology and managed care organizations, Department of Veterans Affairs, independent consultant pharmacy practices, LTPAC corporate and site-specific consultant pharmacy practices and hospital pharmacy practice. On the following page, ASCP Leadership would like to acknowledge these individuals whose dedication and hard work made this publication possible.

5 TOOLKIT OVERVIEW AND GUIDE TO USE This toolkit provides vital information and resources to Consultant and Senior Care Pharmacists providing medication management services to patients during care transitions, and is focused on three main areas: Medication safety during transitions of care Managed Care implications for transition of care programs and medication safety pharmacist clinical services Technology which empowers best practices for assuring medication safety for patients transitioning through care settings. How to use this toolkit: The toolkit is designed to be a reference tool for pharmacists and not a didactic textbook intended for an end-to-end read. Pharmacists may be expert clinicians regarding addressing medication safety issues, but not have a working knowledge of the managed care environment that is driving change, the technology tools to document their patient care encounters or the multitude of resources available. This kit brings those tools and resources together in one place for easy access. The Table of Contents is a good place to start and will give pharmacists a quick overview of the major sections of the toolkit. It is suggested that a next step would be a review of the Section V Overview Slide Deck. This deck will provide more detail on the content of each of the sections. The pharmacists can then access the content and resources that most meet their needs. The content in the various sections is presented in formats that may best meet the needs of the pharmacist. Some content is provided as text documents designed to provide basic information about an unfamiliar area. Other content is provided in presentation decks: both for providing new learning for the pharmacist, but also as a potential presentation vehicle by the pharmacist to colleagues and healthcare practitioners, prospective customers or other stakeholders as they build their individual practices. Resource content is presented via hyper-links which point pharmacists directly to a wealth of additional information and resources about medication safety, managed care, transitions of care, health information technology, legislative and regulatory bodies and other stakeholder organizations. Whatever your personal and practice needs are, the toolkit is designed to bring pharmacists to a baseline level of competence and understanding, and then provide the tools and resources to take it to the next level. The information contained herein is designed to serve as a guide. The material is not intended to be exhaustive and is subject to change. The information is correct to the best of the knowledge of the developers. It is the responsibility of the health care professionals to use their professional judgment for sage and effective drug therapy, compliance with governmental regulations and agency guidelines.
