These services include: Copay Assistance Program* Charitable Foundation referrals Free Drug Program for patients who are uninsured* Bridge Program can provide treatment for patients due to delays in coverage* Uninterrupted treatment for patients already taking PTC products *For qualified patients. 4 HOW WE HELP YOU GAIN ACCESS TO THERAPY Once patient enrollment has begun, PTC CaresTM helps manage each step of the ongoing process Prescription written Welcome call You and your healthcare provider will need to complete and submit a Prescription Start Form (PSF), which acts as both a prescription and a consent, to receive support from PTC Cares and access to PTC products. Your PTC Cares case manager will call you, upon receipt of your Prescription Start Form, to confirm your insurance and demographic information, assist with potential questions you may have about the medication or your medical condition, and discuss the process and next steps to obtaining treatment. Insurance benefits investigation Your PTC Cares team will conduct benefits investigation and prior authorization support. What about refills? Your PTC Cares team will confirm your prescription receipt and call you to schedule the next refill to ensure it arrives on time. 4
5 Explanation of patient assistance Specialty pharmacy delivery Your PTC Cares team will call you to explain your insurance benefits, Patient Assistance Programs, and any available resources such as copay assistance or other forms of support to assist you in obtaining the product. Your PTC Cares team will call you to confirm your delivery information. Upon confirmation, you can request a tracking number so you can monitor where your delivery is throughout the shipping process. Your case manager will provide personalized, ongoing support and customized solutions before the first prescription and throughout treatment. 5
