2 4 Remote and Rural project update: Newsletter has been sent out. Update on Small Isles new project which consists of a visiting service by two GPs travelling around 4 small isles on 2 days per week with additional Videoconference support. New community health support worker post created also who has been trained to deliver medicine by remote IT equipment/dispenser. Large campaign to recruit GP s which was very successful as areas can now merge. Plans to involve advanced nurse practitioner, pharmacist, midwife and paramedic.. Education/training session took place in Oban which was very successful. Key principles were established to take forward to Scottish Government. Ann Pilley emphasised the importance of engaging with the community not only to inform but to obtain feedback on the developing services this part of the communication being more difficult to achieve. to circulate summary from workshop RC stated that although this is good to hear about projects throughout A and B he was keen to know how this is relevant to Islay? responded by stating that it was apparentat the workshop that in terms of multidisciplinary working and education, Islay is well ahead of most areas and it is beneficial for all areas to learn from other projects. There was a discussion regarding community engagement and suggestions on how to communicate and receive feedback from the local community. Awaiting feedback on how successful RCOP booklet/questionnaire was which was made using colour and graphics for patients to understand more easily. stated well informed staff also helps. to provide update in the next ileach. Martine to Community Support worker job description and recruitment pack to MS 5 Dental Recruitment/Facilities Update: Margaret Formby will be retiring in April 2015, however she has agreed to stay on until August working two days per week. Marcin Paradowski, currently Dental Practitioner in Dunoon, has applied for the post (subject to checks) and should be in place by August. Marcin s future plans involve also opening up his own private practice and he has expressed interest in the Birch Drive premises. However, the premises are small and may not be suitable, especially since there is a lack of available parking. There is to be a meeting today with the CHP Architect, Quantity Surveyor and Estates manager to discuss the possibility of accommodating two dental suites in the hospital. The intention is to finalise plans and present a capital bid to NHS Highland. There was brief discussion regarding the difference between personal dental service and general dental service. EW stated that personal dental service is a salaried post under NHS. The General dental service is an independent practitioner who covers both private and NHS treatment. The role of the PDS NHS practitioner is also to provide emergency care to unregistered patients and treat vulnerable people. Ewen clarified that for a GDS dentist to lease NHS premises, the Page 2 of 5
3 proportion of NHS to private dental treatment would need to be 80:20. 6 Matters arising: Recruitment: Lindsay Beattie, the Occupational Therapist who has been covering the OT post on Islay from Campbeltown has been appointed to the permanent full time post start date to be confirmed. She is covering Islay 3 days weekly at present. Also recruited is Roy Cameron, the Physiotherapist who has been providing physiotherapy from Mid Argyll since the start of January Roy will also be taking up a permanent full time post and his start date is to be confirmed. Notice to go in Ileach. Housing: is in the process of arranging a lease with West Highland Housing Association for a house in Bowmore for incoming clinical staff. This arrangement exists already in Mull and the CHP is setting up a similar arrangement on Colonsay. In each of these three areas, it is difficult to find accommodation for staff due to their popularity as tourist destinations. There was a discussion on the Flybe leaflet and RC stated that this was very useful. Another 5000 have been printed for the MAKI locality and 1200 will be distributed locally. Terms of Reference: AB suggested this group s name to be changed to Islay and Jura Health and Social Care Services Review to reflect the purpose of the group. It was agreed that the group must evolve with the integration looking at other areas e.g. transport, social care etc. There was discussion regarding the use of VC for telemedicine to enable patients to have a consultant consultation without having to travel. KD stated that this has been successful in some cases and conditions although does not work for all patients and patients do still have the choice to be seen in the mainland. A draft management structure has been developed and has been out for comment by senior staff. The Integration Manager will be called the Local Area Manager for Islay and Jura. The most senior management posts should be in place by early summer with the next tiers following. highlighted the importance of this group not dealing with day to day operational issues. AB also highlighted it is important not to take out points within TOR which have been dealt with as this shows what good work has been achieved so far. 7 GP Update: Work is progressing quickly in Port Ellen surgery with new disability access, additional walls and a new fire system -all of which are positive improvements. Information was recently provided to the community regarding the change of opening hours in Rhinns Practice and resulted in no feedback from the community being received. This arrangement is on a 3 month trial basis within which period feedback will be welcomed. The change includes extended hours being offered each Monday for a trial Page 3 of 5
4 period of 3 months. When the surgeries are fully integrated, a Saturday service will likely be offered at the hospital for patients who work during the week. All changes will be advertised in the Ileach. It has been confirmed that the Carr Gomm service will continue as usual past 31 March and until there is a decision made on the bid to continue the service over a longer period. AB stated that the new server has been installed. Expecting IT on site next week to progress integration of the IT systems for the three practices. 8 MacMillan Update: The Macmillan Architects will be visiting Islay on 9 th April to progress the plans for the two rooms one end of life care room ad one overnight room for relatives. stated it is hoped plans will be finalised this year and building works start next year. 9 Integration Update: said that the integration between health and social care will be progressing over the next couple of years. GH told of the generic Health and Social Care Worker post in which a member of staff has been working on a fixed term contract for 3 months in both community and ward to progress re-ablement and to facilitate timely discharge home. This post finishes at the end March % of this work has been in supporting OT and Physio from a re-enablement perspective, continuing exercises etc. This prevented admission on three occasions. RC stated that this is a big part of integration as looking after patients in hospital costs a great deal more than looking after at home. It was hoped that this post would support the community nursing team also however this didn t happen due to training required and short term contract. It is hoped that this role will continue at some point in the future. has completed a plan on how to progress the continuation of this role. 10 Draft Action Plan: To be updated for and discussed at the next meeting 11 Communications and engagement Sub-Group: Are responsible for conveying information from the Implementation group and encouraging/enabling feedback from the community. There was discussion regarding different ways to engage with the community through a variety of groups such as Grey Matters and the Community Council. RC stated we need to use modern tools such as Facebook and twitter. GH suggested knocking on doors as the vast majority of the Islay and Jura population do not attend local groups but recognised as time consuming. suggested involving CVS or local youth from the high school to collect this information. to have discussion with Caroline Cecil to decide if the Comms and Engagement group has to continue as a separate group or not. 12 Any other business: Anne Horn highlighted that within the terms of reference is confidentiality and members must be careful not to identify patients evrn if that is indirectly, when discussing services ALL AP raised the issue of transport to and from Islay Hospital to attend OP Page 4 of 5
5 clinics etc. It was suggested asking a 3 rd party to come up with a solution. To be discussed at the next Islay and Jura Transport Forum meeting. MG There was discussion round what clinics are available on Islay locally. GH stated that some procedures are not carried out on Islay as specialised equipment is required and also consultants usually travel to Islay on limited time flying in and out within the day. It was suggested the Health sub group from community council might arrange to meet with Marcin Paradowski, the newly recruited dentist. There was also discussion around informal arrangement regarding M.Formby OOH cover.
