1 NOTE Subject:- Visit of Hon'ble Health Minister to Karnataka and Tamilnadu on 14/09/2008 to 17/09/2008. Hon'ble Health Minister, Prof. Laxmi Kanta Chawla accompanied by Sh.Satish Chandra, IAS, Secretary Health-cum-Mission Director, NRHM and Dr.Rakesh Kumar Sharma, Director, Ayurveda visited Karnataka and Tamilnadu from 14/09/2008 to 17/09/2008. The important observations of the team are given in the following paragraphs. We can also benefit from some of their initiatives. KARNATAKA 14/09/2008 The team visited some of the Health Institutions in the districts of Bangalore, Ramanagram, Mandya and Mysore. (i) (ii) (iii) (iv) All the staff members of the health institutions have been given identity cards which are displayed by the members. Parturition Register A printed register has been prescribed for all the Health Institutions for recording the details of the institutional deliveries. Some of the columns of the register are Parturition No., IP No., Date & Time of admission, Education of patient & husband, Name of patient, husband & address, Blood group & investigation, Place of ANC, BPL/SC/ST, History/Complaint of patients, Diagnosis, Labour (1 st, 2 nd & 3 rd stage), time of birth, date of birth, sex of baby, birth weight, AGPGAR Score, Mode of delivery & remarks, delivery conducted by. Parturition is very useful register and we should also get such register printed and circulated for our health institutions, especially the upgraded ones. (Dr.V K Goyal, Asstt Director (EPI)) Child Care Kit - Karnataka Govt. from its own funds gives a kit costing Rs.1250 containing the utility items for the mother and the newly born child. This is given to BPL families and it contains napkins, towels etc. Rogi Kalyan Samitis Such Samitis have been constituted for all the Health Institutions upto Primary Health Centre level. The members of PRIs head these 1
2 committees and they are very effective. We should also review the constitution of such Committees for all Health institutions and constitute such Committees in PHCs and Mini PHCs. (DHS/SPM) (v) (vi) (vii) Pharmacy Store Medicines are stored in a neat and systematic manner. They have their own Bin Card containing details like from whom received, date of receipt, quantity received, issue, balance, initials of the Medical Office I/c of Stores. A copy of this Bin Card is enclosed herewith. Besides there is a board outside the store indicating the types and quantity of medicines available. (DHS) OPD Timings The Health Institutions are open even on Sundays and other notified holidays from 9:00 a.m. to 1:00 p.m. (DHS) Progress Charts The Health Institutions display the Progress Charts giving details of the main activities. (DHS) TAMILNADU 15/09/08 to 16/09/08 The team visited the Health institutions in districts of Ooty, Coimbatore, Chennai and Kancheepuram. (i) (ii) (iii) Purchase of Medicines Tamilnadu has created a separate Corporation for purchase of medicines and equipments for the hospitals and the medical colleges. We have received a CD containing the essential details of the working of the Corporation and the State Government can also think of creating such a body/wing in the Health Department. (MD PHSC) Varumun Kappom Thittam (Health Camps at Sub Centres) The State Health Department organizes one camp every week in one of the sub centres of the PHCs. The specialist are mobilized and they camp during the day at a Sub Centre covering about 5,000 population. (DHS/SPM) Dr.Murhulakshmi Reddy Maternity Benefit Scheme & JSY The State Government gives Rs.6,000/- to the BPL families for deliveries in Govt. health institutions (Rs. 3,000/- before delivery and Rs.3,000/- after delivery). 2
3 (iv) (v) (vi) (vii) Bangle Ceremony All expectant mothers are brought together at PHC once in a month and given nutrients/antenatal checkups. (DHS/SPM) Mobile Medical Unit Tamilnadu Government has provided Mobile Medical Units for every Health Block. (DHS/SPM) Upgraded PHCs (24 x 7 hour deliveries) The State Government has upgraded all the PHCs for 24 x 7 hour deliveries. The PHCs have Ultrasound, Baby Warmer and other accessories, equipments. (DHS/SPM) School Health Students have been provided Health Cards by the schools. The team of doctors from the Health Department make visits to the schools for health check-ups. A particular day is fixed for every district for checkup of referral cases in respect of school students. The children suffering from Congenital Heart Disease and Pneumatic Heart Disease are given curative treatment at the specialist hospitals free of cost by the State Government. (DHS/SPM) (viii) Detail of Posts Each Health institution has a big Display Board indicating the number of posts sanctioned, vacancies and the names/photographs of the incumbents. (DHS/SPM) (ix) (x) Performance of Health Institutions Health Institutions have Performance Boards indicating the achievements in respect of the important programmes and comparison with the performance of the previous years. The health institution also prepare a monthly printed booklet giving details of :- (a) administrative map and other vital statistics (b) facilities available in the PHC (c) important work done by them and (d) comparison with the previous year. (DHS/SPM/SRO) Online Reporting System The State Government in collaboration with NIC has prepared an Online Reporting System for all the PHCs. The PHCs have computers and the concerned staff have been imparted training. Some of the programmes in respect of which Online Reporting is there, are :- 3
4 (a) Health Camps at Sub Centre Level (Varumun Kappom Thittam) (b) Dr.Muthulakshmi Reddy Maternity Benefit Scheme & JSY (c) Pregnancy and Infant Cohort Monitoring (d) Integrated Management of Neonatal and Childhood Illness (e) Effective Disease Surveillance Information System (f) PHC Deliveries (g) PHC OP Morbidity (h) PHC Performance (i) RCH Outreach Camp (j) Patient Welfare Society (k) Immunisation related activities (l) IDSP (m) Civil Registration System (n) Communicable Diseases I am circulating details separately to all the concerned Officers. We have already assigned the work of preparing the Online Reporting formats to Punjab Engineering College. Ms.Vasundhara, Monitoring & Evaluation Officer-NRHM is coordinating with them. Most of the Programme Officers have prepared the reporting formats. They are once again advised to go through the proformas/formats of Govt of India & Tamilnadu Govt and see if any improvement is required in our formats. Once our formats are finalized, the service provider, Punjab Engineering College, would be in operation. (Programme Officers/MEO) (xi) Institutional Deliveries Tamilnadu Government has taken a great initiative for ensuring institutional deliveries. (a) (b) (c) (d) Maternity Benefit Scheme under which Rs.6,000/- is given to BPL families. Upgrading all the PHCs for 24 x 7 hour delivery and deputing regular doctors for that purpose. All institutional deliveries in Govt. Institutions are free of cost. Punjab Health System Corporation charges about Rs.300/- in its hospitals for normal delivery. We should request the Finance Department to bear these charges. The mothers who come for delivery in the Govt. Institutions are provided diet free of cost for three days. (MD PHSC) 4
5 (xii) Notification of Maternal Death Death of every mother during delivery is enquired into not only by the Health Department Officers but also separately by the District Collectors/SDMs. The purpose of enquiry by the District Administration is to see if the death could have been prevented and whether there was any deficiency in extending the medical service to the mother. (DHS/SPM) ARYAVAIDYA CHIKITSALAYAM & RESEARCH INSTITUTE COIMBATORE The team visited the Aryavaidya Chikitsalayam & Research Institute and interacted with the management. They are willing to help us in :- (a) activating our State Pharmacy (b) training our Therapists at their college campus (c) setting up treatment centres in the State.
