3 Advanced Sexual Assault Medical Forensic Course For Physicians James D. Anderst MD, MSCI William Green MD, FACEP Michael Weaver MD, FACEP Agenda This pre-conference course is one of the first of its kind specifically targeted to meet physician s educational needs in caring for the adult sexual assault patient. The course was previously offered in an 8 hour format. This year it has been expanded to 12 hours, as requested by past participants. Participants will gain expertise in conducting a medical forensic exam, knowledge about expert witness and court room testimony, cultural and legal considerations/controversies in care, and the science behind DNA/STR s as they relate to sexual assault patients. This course will provide state of the art practical information for emergency physician s practicing in academic, community or rural settings, with or without the support of specially trained sexual assault nurse examiner (SANEs). A specific evening program will be dedicated to the Forensic Evaluation of the Child. This course has been divided into 2 components in order to optimize physician education/time. Part One, (an excellent highly suggested prerequisite) is a self-paced at home review of a 12 hour interactive DVD created by Dartmouth School of Medicine entitled, the Sexual Assault: Forensic and Clinical Management, Virtual Practicum. This tool has been reviewed by ACEP s education committee which said it is very well done and would provide relevant CME to ACEP members. This DVD can be purchased through the ACEP bookstore for $50 non members (discounted rate for members). This DVD is also available directly from Dartmouth for $50, by sending a request to Joshua.nelson@world2systems.com. After reviewing this DVD, participants may purchase optional CME directly from Dartmouth for an additional $65. Further information about the DVD can be obtained at the ACEP bookstore website or at: Part Two is our onsite 12.5 hour CME course on Monday afternoon May 13th and Tuesday May 14th. Upon successful completion of this live activity; The University of Missouri at Kansas City School of Medicine will issue a 12.5 hour CME certificate stating that you have participated in the live activity titled Advanced Sexual Assault Medical Forensic Course for Physicians. 3

The evolution to a multidisciplinary team approach has proven to lead to more successful outcomes for patient care and in the criminal justice system. 5 Drug and Alcohol Facilitated Sexual Assault Michael Weaver MD, FACEP Module III: 2:45-5:00 PM This module will review the components of Drug and Alcohol Facilitated Sexual Assault (DFSA) and will discuss the clinical presentations, treatment and medical legal aspects of forensic evidence collection in these cases. Discuss the initial assessment of a patient that is a victim of DFSA. Discuss the common drugs used in DFSA. Discuss the forensic testing and procedures in DFSA. Dinner/Evening Program: 5:00 PM-7:00 PM Module IV Dinner Evening Program- The Forensic Evaluation of the Child: What Do I Need to Evaluate Children in My Hospital? James D. Anderst MD, MSCI As forensic providers, we provide evaluations of possible victims of child sexual abuse, physical abuse and all types of neglect. This presentation will discuss appropriate care and resources for caring for these patients. Verbalize the necessity of collaboration between medicine in the evaluation of the alleged child abuse case. Identify medical conditions and accidental injuries commonly mistaken for child physical abuse. Michael Weaver MD, FACEP As our population is increasingly diverse, this module will discuss the culturally appropriate care for the sexual assault patient. Discuss different approaches to patients from different races/ethnicities, religious, gender and sexual orientation. Discuss cultural issues when caring for the elderly and domestic violence victims. William Green MD FACEP, was founding co-chair of the American College of Emergency Physician, Forensic Section in 2007, and was asked to serve as an advisor to the Department of Justice, in their development of the National Protocol for Sexual Assault Medical Forensic Examinations and to the 1st White House Roundtable on Sexual Violence in Michael L. Weaver, MD FACEP, was founding co-chair of the American College of Emergency Physicians, Forensic Section in 2007, and was asked to serve as an advisor to the Department of Justice, in their development of the National Protocol for Sexual Assault Medical Forensic Examinations and to the 1st White House Roundtable on Sexual Violence in Saint Luke's Hospital was the 1st private sexual assault center in the U.S. and Dr. Weaver has been the medical director since 1980 through the establishment of their SANE program to the evolved Forensic Care Program for sexual assault, domestic violence, elderly and child abuse. 7

8 PRE-CONFERENCE OFFERING DATE: May 13, 2013 SANEAPOLOOZA UMKC School Nursing 2464 Charlotte, Kansas City, MO AM-5PM A proctored clinical experienced focused on preparing sexual assault forensic examiners for clinical examinations. This offering will prepare the forensic examiner to complete the pelvic portion of the sexual assault medical forensic exam. The attendee will be completing a hands-on, proctored examinations with multiple diverse Gynecological Teaching Associates. Goal for the skills lab training: This learning activity is designed to assist participants with demonstrating confidence, comfort and competence in the performance of a comprehensive pelvic examination, including photography. Objectives: After completing this training, participants should be able to: Demonstrate knowledge and clinical competence in performing an external genital and vaginal speculum examination of an adult female patient. Demonstrate the ability to incorporate key points from the didactic portion of the Sexual Assault Nurse Examiner training into the pelvic examination, including digital photography. Pre-Requisites: All attendees must have attended an accredited SANE-Adult & Adolescent Course. Transportation: Transportation to the school of nursing will be available for the out of town attendees. 11 FORENSIC INVESTIGATIONS 6 th ANNUAL From Scene to Courtroom AGENDA May 15, 16, 17, 2013 Purpose / Target Audience Join fellow law enforcement personnel, prosecutors, victim advocates, judges, crisis workers, criminalists, crime scene investigators, physicians, nurses, forensic examiners, and EMS personnel for this cutting edge conference. We will focus on a broad array of emerging forensic practices, including sexual assault, interpersonal violence, human trafficking, ballistics, strangulation, child and elder abuse in both an urban and rural environment. This year sconference is designed to provide a multidisciplinary interactive and joint educational opportunity regarding promising practices when working with victims of violence. Topics will include working with multidisciplinary teams and learning to consider all of the forensic implications on the street, in healthcare settings, and in the court room. Since violence is a health care crisis, we will focus on approaching forensic issues in a collaborative, coordinated, compassionate, competent, caring, victim-centered process. This conference will be held at the Kansas City Convention Center in the 2100 Rooms, which is directly adjacent to the Downtown Marriott. A parking map link is at the end of this brochure. 11

Rather, the body and brain react to and record information about a traumatic event in an entirely different way than we have been led to believe. When trauma occurs, the cognitive brain frequently shuts down, leaving the more primitive brainstem to experience and record the event. While brainstems are generally very good at recording experiential and sensory information, they do not do very well at recording the type of information we have been trained to obtain (i.e., the who, what, when, where, why, and how ). Some victims are incapable of providing this type of information, and when they are asked to do so, they often inadvertently provide inaccurate information and details. This, in turn, frequently causes the fact-finder to become suspicious of the information provided and indeed, may lead them to question the legitimacy of the report itself. Discuss how the human brain records and remembers traumatic experiences. We will review the Forensic Experiential Trauma Interview, which has already been proven to be a game changer in the investigation and prosecution of many forms of violence, including child abuse and adult sexual abuse, and is emerging as a promising strategy for increasing successful interventions, investigations, and prosecutions. This presentation will deliver methods for obtaining far more experiential physiological information than ever dreamed possible. Apply the brainstem memory enhancing Forensic Experiential Trauma Interview procedure. 13 4:00 PM-5:30 PM CSI for the 21 st Century- Room 2103 A &B Ryan Rezzelle MFS, CSCSA Johnson County Crime Lab CSI for the 21st Century is a presentation designed to educate the attendee on some of the current happenings in forensic science. Each passing year...well, more accurately speaking, each passing day brings changes in the science available to assist in the identification and apprehension of criminal suspects. Whether applied in the laboratory or in the field, there are tools available that may not be as flashy as they seem when used by our Las Vegas television counterparts, but ground level CSIs around the country unearth (sometimes literally) amazing evidence on a daily basis. The presentation will also look at some of the pending legislation that looks to change the face of forensic science. Objectives: At the conclusion of this educational offering, attendee will be able to: Discuss how to bring the science of the crime lab into the field for use in forensic investigations. Discuss the paradigms of investigation being applied in forensic investigations into volume crimes (ex. burglaries) and how our these perspectives have changed over the past ten years, and what that has meant for getting "gateway criminals" off the street. Verbalize the cutting edge methods used in the evolution of crime scene mapping, specifically as applied to enhancing the crime scene reconstruction abilities of crime lab staff. 5:30 PM Adjourn Thursday, May 16 th, :00 AM-8:00 AM Registration Opens-Kansas City Convention Center-Room :00 AM-9:30 AM Breakout 1: The Anatomy of Suffering: Understanding the Effects of Trauma on Victims of Abuse and the Professionals Who Work With Them. Megan Peterson LCSW This presentation will explain what happens on a biological level when a person experiences a trauma, what the short and long term effects of these biological processes are, and the implications for crime investigation and court testimony, particularly in regards to memory of traumatic events. Objectives: At the conclusion of this educational offering, participants will be able to: Discuss the biological process of a response to a traumatic event and how this affects memory and long term functioning. Communicate effectively with victims and service providers who are still in the midst of a response to a traumatic event. Breakout 2: Women Who Molest Children: Offender Typologies Cpt. Bill Carson This workshop begins with findings that were learned from in-depth interviews with eighteen women who had been convicted of molesting children, but goes well beyond that to include cases from across the nation as well as data from other recently published material on this topic. Captain Carson will discuss past and current offender typologies for female child molesters, including his five distinct typologies. Objectives: At the conclusion of this educational offering, participants will be able to: Discuss typologies for female child molesters. Discuss findings learned from in-depth interviews with 18 women convicted of child molestation. Breakout 3: Prison Elimination Act: What Is It? Why Should I Care?? How Does It Affect Our SANE Programs??? Jennifer Feicht The Prison Elimination Act (PREA)has taken over the corrections field during the past year as compliance to the standards looms ahead in August These standards have been enacted to assist the corrections field in addressing the issue of sexual assault in the corrections setting. The standards address most types of corrections settings in the U.S., including prisons, jails, community corrections facilities, lock ups, detention centers and federal facilities. It covers both adult and juvenile inmates. For too long prison rape has been ignored. It is time to bring attention to this issue. More than 1 million women are currently incarcerated or otherwise under the control of the justice system in the U.S. Of these women, a reported 85-90% has a history of domestic and sexual abuse. Their involvement in the justice system leaves many incarcerated women vulnerable to re-victimization. But this isn t a session just about women men incarcerated in this country 13

14 are being sexual victimized and communities are struggling with how to deal with this epidemic as well. How will this impact your SARRT team? How will this impact your SANE program? Are you prepared? Objectives: At the conclusion of this educational offering, participants will be able to: Verbalize the purpose of the Prison Elimination Act. Discuss how this Act will impact your criminal justice or forensic program in assisting corrections in obtaining compliance to the PREA standards. Breakout 4: What Happened To You? Creating a Trauma Informed Care Culture TARA GUNTHER, PSY.D This training will help participants gain a better understanding of Trauma Informed Care. Experiencing a trauma can change or affect the way an individual perceives the world for the rest of their life. Whether it's a single event, or a more complex trauma like prolonged exposure to abuse, attachment issues, or community violence, an individual's thoughts, feelings, and behaviors can be impacted and filtered through this perspective. Objectives: At the conclusion of this educational offering, participants will be able to: Describe the difference between trauma-informed vs. trauma specific care. Learn how to move from a traditional paradigm to a trauma informed paradigm in your work setting. Increase awareness of trauma in both our patient care and ability to care for staff. Breakout 5: Creative Ideas from the Field: Victim Advocacy in Rural Communities Angie Blumel-Metropolitan Organization to Counter Sexual Assault Linda Tessar-Executive Director Hope Haven This session is designed to help professionals think in new and strategic ways about conducting advocacy at the individual and system level in a rural community. The facilitators will use lecture, group discussion and activities to explore solutions around meeting the unique challenges faced by rural communities when it comes to providing victim advocacy including: Define systems advocacy and discussing how this work directly impacts victims of sexual and domestic violence, Eliminate barriers specific to victims of sexual and domestic violence in rural communities, Explore ideas to enhance existing advocacy strategies. Objectives: At the conclusion of this educational offering, participants will be able to: Define systems advocacy. Identify barriers unique to victims of sexual and domestic violence in rural communities. Develop procedures and practices to enhance existing advocacy strategies in rural communities. Breakout 6: The Forensic Evaluation Of Strangulation: Don t Choke! William Smock MD, FAAEM, FACEP Strangulation is an act of violence frequently used to control and overpower a victim, and has recently been recognized as a particularly lethal form of interpersonal violence. This presentation will explore the physiological effects of strangulation, signs and symptoms, and medical/forensic evaluation of these patients. Written and photo documentation will be discussed, as well as the importance of collaboration between health care and law enforcement to improve patient safety. Objectives: At the conclusion of this educational offering, participants will be able to: Describe the patho-physiology associated with non-fatal strangulation. Discuss the benefits of applying clinical forensic medicine to the non-fatal victim of strangulation. 10:00 AM-11:30 AM Breakout Sessions 9:30-10:00 Break/Vendor Exhibits-Room 2103 C Breakout 1: Human Trafficking: The Journey From Victim to Survivor to Community Leader Teresa Briscoe Human trafficking is one of the fastest growing criminal industries. Unbeknownst to many, it is taking place in the neighborhoods where you live and work. Human trafficking is the recruitment, transportation, harboring, or taking of persons by means of threat, force, coercion, abduction, fraud, or deception for the purpose of exploiting them. You may already be treating these individuals and not be aware that they are human trafficking victims. Join us for a dynamic and informative presentation to learn more about this hidden, yet thriving, crime in the U.S. and internationally and its effects on both victims and society. The speakers will address the legal, psychological, and societal ramifications of human trafficking from the personal perspective. Objectives: At the conclusion of this educational offering, participants will be able to: Discuss the physical and psychological impact of human trafficking from the victim s perspective. Work in collaboration with community services to identify, treat and rescue victims of human trafficking. 14

15 Breakout 2: Groomers and Grabbers: Two Very Different Types of Child Molesters Cpt. Bill Carson This workshop will compare and contrast two very different types of child molesters. There are offenders who use grooming and seduction as their primary method to molest children, and then there are child molesters that tend to use force and violence to accomplish their predatory goals. This offering will explore some of the differences between these offenders and will include video clips from interviews with both types of offenders. Compare and contrast two very different types of child molesters. Explore the differences between child molesters. Breakout 3 : Dissecting the Forensic Autopsy Patrick Besant-Mathews MD. Forensic Pathologist This session will address the differences between medically oriented (traditional, hospital, teaching) autopsy examinations, and those with a practical legal orientation (by Coroner or Medical Examiner) to determine cause and manner of death. Then how to assess a forensic autopsy report for content, wording, userfriendliness, organization, and needs in a courtroom. Misconceptions, vital components, problems with measurements, how best to convey information to those who are unfamiliar with medical terms, organizing multiple gunshot wounds, using diagrams for clarity, and becoming aware of failures to correlate autopsy findings with other important parts of death investigations. At the conclusion of this educational offering, attendees will be able to: Distinguish between the hospital autopsy and the forensic autopsy. Identify and describe the parts of an autopsy report. Identify and describe the three steps in determining the cause (and manner) or death. Breakout 4: Understanding and Investigating Child Pornography Lt. Joe Laramie This training will help participants gain a better understanding of what child pornography is and what it is not. It will discuss the devastating and long-term impact on victims. It will also explain the types of individuals that collect child pornography. This training will also provide attendees a better understanding of how to interview or communicate with those who collect child pornography. Joanne Archambault End Violence Against Women International Retired Captain-San Diego Police Department In a recent study of the factors that influence a prosecutor s decision to charge in a sexual assault case, researchers validated what many experienced investigators already know that charging decisions are primarily, if not exclusively, determined by the victim s perceived character and her behavior at the time of the assault. We could conclude on this basis that law enforcement should just give up thoroughly investigating difficult cases and focus only on those cases where the victim has an untarnished moral character and impeccable behavior at the time of the assault. To the contrary, this study shows the importance of good police investigation and the crucial role that police have in corroborating the victim s allegations by conducting and documenting thorough victim interviews. Recognize inappropriate police responses and potential barriers to effective victim interviews. Identify ways to build trust and cooperation with the victim. Breakout 6: Driver or Passenger? Breakout 5 : Strategic, Victim-Centered Community Responses Redefining Our Success Joanne Archambault End Violence Against Women International Retired Captain-San Diego Police Department Investigating and prosecuting sexual assaults is a privilege and yet a very tough job. Criminal Justice administrators must recognize this fact and create an environment which supports and mentors professionals who work in this field. Because of the complexity of these cases and the dynamics of sexual assault, a multi-disciplinary community response is absolutely necessary to successfully respond to crimes of sexual violence. Sharing responsibility and resources also enhances our success and helps a jury to understand the significant impact these crimes have on the community. Traditional criminal justice measurements used to evaluate the success of law enforcement efforts must be re-examined to assess their ability to truly accomplish this task. For example, some frequently used evaluation measures include arrest rates, unfounded rates, and clearance rates. These statistics are generally used because they can easily be reported by examining law enforcement records alone, without having to coordinate with prosecutorial records. However, although these measures may allow for some sense of the process of the investigation, they do little to measure the actual criminal justice system response. In order to effectively monitor and evaluate the system response, we must work collaboratively with prosecutors to collect and report aggregate information such as the percentage of cases in which an identified suspect is charged with a crime, the type of case adjudication strategies used (plea bargain, jury trial, etc.) and the ultimate prosecutorial and sentencing outcome of the cases. If we wish to communicate to the community that certain crimes are being taken seriously by the criminal justice system, we must be able to accurately describe and monitor the end result of the entire criminal justice response, not just that portion for which law enforcement is responsible. Discuss the development and utilization of local expertise to respond to challenged financial resources in both urban and rural jurisdictions. Identify obstacles that deter law enforcement agencies and prosecutors from creating specialized units. Breakout 6: Evidence Collection in Gunshot Wounds: Avoiding Blunders in the ED. William Smock MD, FAAEM, FACEP The role of the medical/forensic examiner and the forensic evaluation of gunshot wounds is a critical component of criminal investigations as well as patient care. The determination of entrance versus exit, range-of-fire and bullet trajectory are vital facts of the case. They are also components necessary for reconstructing the incident under investigation. The properly trained medical/forensic examiner can perform an evaluation of handgun, rifle and shotgun wounds, accurately document the wounds, and testify to the findings in court. The medical/forensic examiner can also recognize, correctly collect and preserve ballistic-related evidence for submission to the crime laboratory. Weingart JD Assistant District Attorney Johnson County, Kansas Lethality assessment is the attempt to identify the circumstances when a batterer is most dangerous by evaluating the batterer s beliefs and patterns of violence, coercion, and control. The underlying assumption is the higher the number of predictors, the higher the potential for the batterer to commit a homicide or engage in potentially lethal behaviors. This offering will teach attendees how to apply a lethality assessment and how to identify those victims in need of immediate safety interventions. Complete a lethality assessment with victims of inflicted violence. Implement a safety plan to prevent frequent violence and/or death. Breakout 6: The Forensic Investigation of Officer-Involved Shootings William Smock MD, FAAEM, FACEP The forensic evaluation of gunshot wounds is critical to any criminal investigation. When law enforcement officers are shot or shoot a suspect it is critical that the forensic questions of entrance versus exit, range-of-fire and bullet trajectory be answered.
