2 FROM OUR FACILITY MANAGER Great teamwork Dear Residents Phew, we have had a very eventful winter. I wish to thank staff and the management team for hosting the DAA Audit team without me during Glenwood s recent surprise SPOT Surveillance Audit. The auditors turned up when I was participating in an education day in Wellington Murphy s law I guess! The team and staff kept their cool and the audit went off without a hitch well done all! The auditors stated that it was business as usual for our team and thanked us for the welcome and for the open and honest way in which the team approached the audit. So how did we do? We received four accolades called Continued Improvements and have three areas that require work. Continued Improvement Complaints register and process improvements Use of Quality data to improve service delivery Orientation programme improvements Ongoing education, improved skill levels and improved staff satisfaction Areas that require work Transferring short term care plans to long term Documenting final outcomes provide evidence of intervention Evaluation of interventions show residents needs have been met in the notes According to our SIDU Portfolio Manager for Aged Care, achieving four Continued Improvements is an impressive result. This would not have been possible without our collective vision of providing the Best of Care and ensuring that the golden thread of quality is woven through everything we do. Once again well done, team! A stellar year for Glenwood In past newsletters I have discussed our Falls prevention programme, an initiative we ve been working on over the past few years. We have had great results and have significantly reduced falls. I entered our team into the 3DHB Quality Awards for 2014 (Wairarapa DHB, Hutt Valley DHB and Capital & Coast DHB). I am chuffed to be able to inform you that we won the Commitment to Quality Improvement Award for our Falls prevention programme. Our team work hard every day to provide the Best of care to our residents and our commitment to quality underpins everything we do. We are proud to have our commitment to reducing harm from falls and falls prevention publicly acknowledged. Well done, Glenwood team! In addition to this, the Health Quality Safety Commission which helped fund the 3DHB minicollaborative falls prevention workshops that we attended earlier in the year introduced us to Ko Awatea. This is a quality improvement organisation based in Auckland which works closely with the Commission and the Ministry of Health. Ko Awatea is keen to work with some of the organisations which took part in the 3DHB s minicollaborative on falls prevention to see if they can turn some of the Falls prevention programmes into an E-learning module (a training tool for staff). It would be great to share some of what we have learnt on our journey with others in the health care sector. It s also great to see the Aged Care sector being approached to share quality initiatives with other sectors go Aged Care, go Glenwood! So some very positive forward thinking efforts from the team at Glenwood and also some very well deserved recognition for those efforts, great work and well done all. 2 Glenwood News, Summer 2014
3 The Girls Trip from the West Coast to Queenstown Glenwood News, Summer
4 On a personal note I was very grateful to have a two week break in November and would like to thank the management team for covering for me. I did a South Island Girls Road Trip with my 76 year old mother. Mum surprised me she was a real trooper! We went in my little Suzuki and stayed in backpackers and cabins with our sleeping bags. From Blenheim to Bluff via the West Coast and back up the East Coast. I had a good rest and great fun with mum; nice memories to treasure. I even had a ride over the Fox Glacier in a helicopter for $99 a bargain! Rosie missed me and is sticking to me like glue! Staff changes We have farewelled Jo Miller, our Community Manager, after one year of service with us here at Glenwood and welcome Shalini Vij to this post. Shalini also continues in the role of Education Coordinator for the hospital and community services. I wish you all the best till our autumn newsletter and like most of you look forward to the approaching summer, I love daylight saving more time in the day to do the things we love outside of work. Warm regards Danielle Farmer, Facility Manager COMMUNITY CARE Life is full of changes Hi Friends Are you surprised to see me here under the Community Care banner? I know not all of you will be but I bet a few of you are. Yup, I changed my role from hospital RN to Community Manager in October of this year. In the 18 years of my nursing career, I have worked in hospitals, nursing colleges and aged care facilities but now, for the first time, I have moved my step into community nursing. It s exciting and challenging. It s a part time role for three days a week but you will see me in Glenwood because I am continuing in my role as Education Coordinator on the other two days. Working as Community Manager is a big change for me. Now, I move around the town bringing care services to people s doorsteps and I am enjoying it. This summer is bringing lots of changes not only in my professional life but also in my personal life. Like seasons your life changes. Another good change in my life is that this month my husband Smit and my son Anshuman came to New Zealand from India. We are now all together after being apart for approximately four years. 5 CLINICAL NURSE MANAGER A year of achievement This quarter has gone so fast I can t believe we have nearly reached the end of Before we know it we will be slipping into 2015! It seems that there are not enough hours in the day anymore, and a year is almost equal to a week now. Yes, as the saying goes if you want to do something do it quick and do it now. We have had so many achievements this year, it is almost unbelievable. Most recently, we were the winners of the Commitment to Quality Improvement Award at the 3DHBs Quality Awards at the end of November [see Danielle s report]. Safe administration of medicines I commented in an earlier newsletter on our initiatives around ensuring that our nurses administer medicines safely and effectively to our residents. One of the initiatives was for all nurses to enrol in a Health Education Trust module Safe administering of medicines. This module aims to improve practice around medication safety by allowing them to reflect on their own practice. Two of our enrolled nurses (Peggy Ruback and Jacqui Staats ) and one registered nurse (Lesley Crook) have completed this module. Congratulations to them as I know it was not easy. When we began this module they had mixed feelings about it. Feedback I received on completion was that they are glad they have done it. They feel that it was very valuable and they have learnt a lot. Professional Development Recognition Programme Congratulations to Shalini, who has successfully met the requirements for Proficient RN, PDRP. I quote from the received from Karen Shaw, Nurse Coordinator, Professional Development for Wairarapa & Hutt Valley DHBs: Shalini s achievement is a reflection of her commitment to quality patient care and high standards of practice. The PDRP enables us to formally acknowledge the significant contribution Shalini has made towards nursing practice in your area. Thank you for supporting your staff to participate in the PDRP. Your commitment to strengthening skill mix in your organisation will benefit both the patients in your care and your organisation. Peggy Ruback and Jacqui Staats received their Proficient EN level last year. Shalini, Peggy and Jacqui Staats will be formally acknowledged and will receive a certificate from the Nursing Director next year on International Nurses Day. Well done to all of you! QPS Report Benchmarking This is a quarterly quality performance report done for us by a quality performance system management company. They benchmark us with other aged care facilities of our standard in NZ and Australia. I thought to start putting our results out there so you can all be aware of how we are doing. Our QPS Report for July, August and September 2014 shows that our average daily occupancy rate has significantly increased this quarter by 14.48%. The overall occupancy rate was 94.87%. We had a significant reduction in manual handling injuries. These have decreased by 81.5% and staff accidents have declined by 81%. This is due to ensuring staff are familiar with safe manual handling practices and ensuring training is given during the orientation process, and as soon as an issue arises. There is a significant improvement in pressure injury and skin tear rates, wound, UTI and RTI infections. Well done team. It is useful to note that the training offered around these issues are put in practice. However, our falls rate has increased this quarter due to two residents that are high falls risk. We have had a look at the causes for this increase and aimed to find preventative measures. This appeared to be effective and hopefully we will see some positive results this quarter. Wacky Wednesdays Our Wacky Wednesdays are proving to be most popular with staff and residents. 6 STAFF TRAINING A great year for achieving excellence Hi Friends And here I am again. Roses are blooming in my garden this summer and spreading their fragrance around. So do our staff bloom with education and spread the fragrance of their quality care. This quarter lots of exciting things have happened with staff education. Danielle Farmer (FM) and I in consultation with John Hudson (Fire Agent) have reviewed our Emergency Flip Chart, Fire Evacuation Scheme, Emergency Manual, Zone Maps and Fire Bag contents. Following this, we developed an Education Plan for wardens and all staff, along with a PowerPoint presentation. Inservice training at Glenwood The in-service training session on Fire Evacuation was conducted for all staff in mid-october, and a two day in-service session was organised on annual compulsory training topics. Twenty-one staff members (hospital & community) attended both study days. Participants received their certificates at a general staff meeting mid November. Congratulations! Eleanor Millane (HCA) received the National Certificate in Health, Disability and Aged Support (Core Competencies) Level 3 from NZQA. Yvonne Mackey (HCA) received the National Certificate in Health, Disability and Aged Support (Core Competencies) & Community Support Services (Residential) Level 3 from NZQA. Richelle Perales (HCA) received the Certificate of ACE Dementia Programme by Health Ed Trust. Yvonne Mackey (HCA) and Joe Vincent Dawang (HCA) received a First Aid certificate from Red Cross NZ was a great year for educational achievement and development. 7 ADMINISTRATION In the lead up to Christmas Dear Residents and Families At the beginning of this month my daughter was very excited on the first day of December she was finally allowed to open a little door on her Advent calendar. It was great to see how excited she was and it reminded me at my own childhood days. The holiday season is a very special time of the year but it s definitely not the same now as it was when we were children, is it? I like to pass on some of my German traditions to the next generation and this year I m lucky to have my Mum here with us. She is spending Christmas and New Year in New Zealand. Last night, we finished off filling in the self-made Advent calendar, and there is Christmas crafting and cookie baking on the list for the next couple of weeks too. There are lots of old traditions. Last year I told you about the famous German Christmas markets and their origins; this year I m writing about the history of the Advent calender... have fun reading. History of the Advent calendar The origin of the Advent calendar can be traced back to the 19th century. The first known handmade calendar dates back to Religious families hung little pictures (Bible motifs) on the wall one for each day in December. Another tradition was to paint chalk strokes on the door, one per day until Christmas Eve. Other early styles were the Advent clock or the Advent candle a candle for each of the 24 days until Christmas, like today s Advent wreath. The first printed Advent calendar was made in 1908 by Swabian parishioner, Gerhard Lang (born 1881 in Maulbronn, Germany, died in 1974). When he was a child, his mother made him an Advent calendar with 24 Wibbele (little candies) which were stuck onto cardboard. Glenwood News, Summer
8 Later, in 1908 when working at the printing office of Reichold & Lang Gerhard produced little colored pictures which could be fixed onto card on each day of December. ON THE NOTICEBOARD This was the first printed Advent calendar, although it did not have windows to open. It was called the Christmas-Calendar. Gerhard later produced the first Advent Calendars with little doors to open. The Advent calendar started a triumphal way around the globe, but Gerhard had to close his company in the 1930s. Until that time he had produced about 30 different designs. World War II terminated the success of this German tradition. The cardboard was rationed and it was forbidden to produce calendars with pictures. The first printed version after the war was printed by Richard Sellmer in Advent calendars filled with chocolate became available in 1958, and are still going strong you can find them in our local supermarkets and stores. These days the motifs are often not religious Superman, Barbie & Frozen motives are common. How traditions change! May your holiday season be blessed with peace, love and joy.
