1 2. IPD All the admissions done in the hospital will be entered through IPD. There are two ways of making entry for a new IPD patient. First, method is to go by new IPD registration screen and the second method is to go directly from IPD detail option. If we want to just make entry of admission and have a registration printout then we go through new IPD registration method otherwise if we don t want registration printout and we want to enter service detail of the patient as well at the same time we go through IPD detail option. New IPD Registration Go to IPD option in main menu and click on new ipd registration or ctr + N for any new registration. The following window will appear on the screen. Enter the patient details in the following fields. IPD No. : The software will automatically show the new IPD no. press enter for next field. Note 6 : Press enter or Tab key to move to the next field everywhere in the software. If you want to go back to previous field press shift + Tab.You can use mouse also to move anywhere. Use Alt key to activate any button or field with a character of the field underlined. For example, use Alt + O for Ok button. Date : Administrator plus (AP)will automatically show the current date. Patient No. : AP will give new patient no. if the patient has visited earlier, enter his patient no.. Prefix : Choose prefix. Patient Name : Enter patient name here. Sex : Choose patient sex (male or female). 3.1

2 Age (mons, days) : Enter patient age here. If the patient is below 1 year of age enter age in months and similarly enter age in days if the patient is below one month. Father/Gaurdian : Enter father/guardian name here. Husband Name : Enter husband name here if applicable. Occupation : Enter occupation here. Address : There are three lines for address fill complete address of the patient along with state and pincode. Martial Status : Choose martial status. Religion : Select religion. Nationality : Enter nationality. Tel (Res) : Enter the telephone numbers here. Use second field for mobile number. Dept. Under : Select the department. MLC NO. : If the patient has medico-legal case. Enter MLC no. here. Refred by : Fill the referring doctor code here. Surgeon : Enter the surgeon name if applicable Case Type : Select the case type if applicable Panel : Enter the penal name here if applicable Panel ID No. : Panel ID number is to be entered in this field Consultant Incharge : Enter the code of consultant incharge of this patient here. Room No. : Select the room available and enter. Source : Choose the patient source from master. After we have entered all the desired information press Ok button at the bottom to save this registration. If we want to print the registration information or you can say the front page of the patient file containing all his/her information, click on Print button. 3.2

4 IPD Details: Whenever we have to enter the details of an IPD patient we go through IPD detail option. To admit new patient, click on IPD and enter or we can use the Ctrl +I key. It will ask you New registration number YES or NO. YES for new registration number and NO for updating already admitted patients. New Admission: Every patient will be given a new registration number. While the patient is admitted in the hospital, all the services provided to him will be entered in this allotted registration no. The following window will appear on the screen. On the top of this window registration number is displayed along with Go and Cancel buttons. The registration number will be automatically provided by the Administrator Plus for every new registration. Click on Go button or press enter to make entries in this registration number. Click on cancel button if you want to exit from this screen. 3.4

5 Old Registrations: If we want to enter the services of any admitted patient, select No from new registration and the following screen will appear. Enter the patient s registration no. or click on Select button to choose from the list provided. You can search either by patient registration number, name, or any other field as we search in masters. 3.5

6 IPD ENTRY - Once we enter the registration number, the following screen will appear having the following tabs. GENERAL INFORMATION ADMISSION INFORMATION ROOMS SERVICE AVAILED PAYMENT DETAILS DISCHARGE DETAILS DISCHARGE SUMMARY PACKAGE TAKEN GENERAL INFORMATION: This tab is for entering the general information of the patient. Enter Date & Time of patient admission. You can choose the date by opening the calender as shown below. To open the calender click on down button from Date box.this calender option is available everywhere in the software, whereever we have to select date. 3.6

7 After entering the date and time press Tab or Enter key. The cursor will move to patient code field. A new patient number is automatically alloted to the patient. If we want to select any previous patient, enter his patient number here or enter any number and select from the list provided. The following screen will show the list of previous patients.enter the patient name and the cursor will automatically move to that entry. You can also search on any field. For new patient enter the patient code field and the following window appears on the screen. Patient Code : Patient code is automatically generated Denotes patient Code. Prefix : Enter prefix to be used before name. Mr. / Mrs. / Ms. / Master / Baby Name : In this field you can write the patient name. Age (yrs) (months) (Days) : In this field you can write the patient age. Sex : In this field you can write the patient sex, Male or Female. Father/ Husband Name : In this field you can write the patient father name or husband name. Address : In this field you can write the patient address. 3.7

8 City : In this field write the patient city. Phones : In this field write patient phone no. Religion : In this field write the religion, to which he/she belongs. Nationality : Write patient nationality here. Occupation : In this field write the patient occupation like service or businessmen, housewife, student, engineer etc. Click ok to save. When you click on ok it will come back to general information window. Referred By Panel Panel ID No. : In this field we will write consultant code who have referred the patient. We can also select from the list of consultants. : Enter the panel code if the patient belongs to any panel or TPA. : Enter panel ID number here if applicable. To move to another tab use F2 key (mentioned in this window at the bottom on the left side) or Alt +A or click on admission information tab. F2 key is used for move from one detail tab to next detail tab. ADMISSION INFORMATION: In Admission information tab, we enter all the details of admission. The following screen will be displayed. Admission Source Room no. : In this field we enter the exact date and time of patient admission. : Enter the source of patient whether he is coming from Casuality, OPD or Referred. : In this field enter the room no., where the patient is to be admitted. You can also select from the room category wise list. 3.8

9 Select the category of room like double room and press Ok. This will display all the rooms available in the selected category as shown below. Select the room number and press Ok button. Room Consultant Surgeon Case type : When you enter the room no. AP will automatically display the room charges in this field. : write the consultant incharge code for this patient. : Enter surgeon code if required for this patient. : In this field you can write the type of case. 3.9

10 Dept. under : In this field write the department code for this patient. F8 : F8 tab is compulsory if we have modified the patient room later on in this screen. Press F2 to move to next tab. ROOMS: The room alloted to the patient will be displayed in this window as shown below. 3.10

11 This window shows the current room status of the patient. Room no. shows the room in which the patient is admitted, Date shows the date and time of patient admission in this room, Vacant date will show when this room is vacated, Days will show the number of days patient stayed in the given room, Rate will display the room charges per day and the Charges will show the total charges of that room during his stay. This window is normally used when a patient is transferred from one room to another or if we want to edit room charges or no. of days manually. Half Day : If the patient is admitted for short stay or the day of admission or discharge is not to be charged fully click on half day box.the software will charge half of the room charges for that day. Room Transfer : This button is used whenever the patient is transferred from one room to another.click on Room transfer option, then the following window will open. In this window enter the new room code where the patient is being transferred and in Date field enter the date and time of transfer then press Ok. 3.11

12 Undo Transfer : If we had made any mistake in transferring the patient, then we have to Undo the transfer. Click on Undo transfer button. Refresh Rates : This button is used whenever we want to refresh the rates after any changes. Press F2 to move to next tab. SERVICE AVAILED: This is the window where we will enter all the services provided to the patient. 3.12

13 Click on the text box of service field and enter the service code of the service availed by the patient. If you don t remember the service code or enter wrong code here, the software will show you the department list as shown below. Select the department of the service and press Enter. The software will display all the services of the selected department as shown below. To Select a particular entry just click on it, or press enter on that row. Its color will change and a star will appear in the last column. Now you can do the same with other entries too. When done press F5. You can also use F6 for selecting a particular group of entries what you have to do is write the name of that field, operator and then the value. Automatically the group will be selected. You can also use Select All for selecting all the entries. We will come back to service availed screen and all the services selected will be shown here. 3.13

14 DATE Service Rate QTY Amount : This field denotes date of service availed. Whenever we enter the service the system will itself take the current server date here. However if we are making entries of a previous day, we can change this date. : In this field we enter all services provided to the patient as described earlier. : This field denotes rate per unit of a particular services. By default rate is picked up from service master. While picking the rate it will also considered class of room if specified in service master.the rates if dependent on any parameter like panel, percentage of department etc. will be shown as per the define parameter.if rates are not defined in service master or rate in service master is 0, the software will ask to enter the rate of this service. : This field denotes no. of times service availed e.g. 5 medicines is given to patient we can make qty equal 5. : This field shows the total amount of service. Service Name : This field show the service name of the code entered in service field. This name is to check and verify the service code. In some cases we can edit or add remarks in service name if permitted by system tunning. The name entered here will be printed on bill. Discount In Room Cons. Code Cons. Name Package Ref. no. Daily SNO User Name DOC Type LAB NO : If we want to give any discount to patient on this service, then we enter the percentage of discount allowed in this field. : Some services are linked with the room. whenever we enter any such service its room will be displayed in this field. : If this service is performed by a consultant and we want its details tobe transferred to consultant account, then we have to enter the consultant code here. For ex. if an Ultrasound is performed, then we will enter the consultant code here who have done this Ultrasound and its detail will be shown in doctor s record. : This will show the name of the consultant whose code is entered in Cons. code field : Like in rooms, if the service is linked with package the package name will be displayed here. : This field is to enter any reference number or bill number for this service. Like medicine bill no. : This field is used to enter daily serial no. of any department. : This field will display the user name who have entered this service. : This will show the Doctor type. : This will display the Lab no. if the service belongs to Laboratory. At the bottom of the screen the total amount of the services is shown. This total does not include room charges. Below the total they are some help keys mentioned. Now we will discuss the utility of these keys. F3 F4 : F3 key is used for going to next service. When you press F3 key automatically you can see the cursor will directly move to next service field. This key is very useful for fast entry as we don t have to use mouse. : F4 key is used for all services. If the billing clerk doesn t know the service department then he can choose the service from complete list of services. Press F4 on service field after pressing any code and you will see the complete list. 3.14

15 F6 : F6 key is used for Delete the service. If you want to delete any service, select the service and press F6 key. A message will appear to confirm. Are you sure you want to delete the entry if you are sure to delete then press Yes and if you don t want delete the entry then press No. F7 : F7 key is used to find any service from the entered services of the patient. Press the F7 key and a Searching Window will open. Enter the name of service or its initials in the blank box and press Ok. The cursor will automatically move on the matching service. F11 F12 : F11 key is used to sort the entered services as per the date. : F12 key is used to sort the services alphabatically as per the service name. PAYMENT DETAILS: The next step is Payment Details. All the payments received including advance from this patient will be entered here. 3.15

16 Click on Add new payment button for entering any new payment or advance. The following screen will appear. Regd. No. Receipt No. : This field will show the registration number of the patient itself. : This field automatically show the new receipt no. Payment Date : This field automatically show the current date and time.you can also change it, if not correct. Amount Type : In this field you can fill the amount which is given by patient. : This field shows the payment mode. C is for cash, B is for bank or cheque, D is for draft and R is for credit card. Enter the type or you can select form the list. If the payment mode is B or cheque, it will ask the following details. Chq. Date Chq No. Bank : By default it shows the current date. : Enter the cheque no. here. : Enter bank name and branch here. After you have entered the details click on Print or Print Preview to print the receipt or press Ok to move to previous window. This window will now show the payment details we have just entered. 3.16

17 SAVING THE ENTRY AND EXIT: Once we have entered all admission and payment detail, click on Ok button on the top right hand side. There are four buttons here. Ok Cancel : when we want to save and exit from this window, click on Ok button. : If we want to cancel the recent modification, click on Cancel button and the entries will not be saved. Computer will reconfirm about the cancilation. Save and Continue : When we want to continue working on this window and save the entries made till now click on Save and Continue button. Delete : This button if used will delete all the entries of the patient. This access is only allowed through manager passward and if set in tunning only. This option is used only for testing entries. ENTRIES OF ADMITTED PATIENT: Once a patient is admitted his, the services or treatment provided to him have to be entered in his bill on regular basis. It can be done either through transaction option (add bill items), if entered department wise or the entries can be made through main IPD screen as well. Similarly, any payment received during his stay in the hospital also has tobe recorded. The payment can also be entered either through transaction option(payment) or through payment details tab of main IPD entry screen. All entries has to be updated regularly so that the hospital as well as the patient attendance knows the current billing status any time. Any room transfer also has to be recorded timely for exact updated bill. To make entries of any admitted patient we can also go through main window where on the right hand side the software displays all admitted patients. Press the Refresh button and software will show you latest updated list. Simply double click on the patient name and his record will open. The records can also be sorted on any field like Registration no., name, admission time, consultant name and room no. Simply double click on the field heading and the list will be sorted on that field. Again double click on the same field and the list will be sorted on that field in descending order. For ex. if you double click on name, the list will be sorted ascendingly(a to Z) on name field, and if you again double click on name the list will be sorted descendingly (Z to A) on name. At the bottom of this window it will show you the number of admitted patients. 3.17

18 PATIENT DISCHARGE: At the time of discharge of a patient select the patient from main window and go to discharge detail tab. DISCHARGE DETAILS: It is used only when a patient is discharged. Select the patient and click on discharge details tab. The following window will appear on the screen. Click on discharge box and the software will display current date and time in date column alongwith a new bill no. in bill number column. Current date will also be displayed on Bill Date column. The duration of stay of the patient will be displayed in No. of Days column. The user name discharging the patient will be displayed in Dis. By column. Verify the date and no. of days before finally discharging the patient. Any corrections or modifications required can be done at this stage only. Click on Half day box if half day is applicable. Press Done&Proceed button to proceed further. Once you press this button all discharge details will be freezed and Administrator Plus will show you the following details. Total Room charges Room facilities Other services : This field shows the total room charges. : This field shows the total of room facilities charges. i.e the services charges of services attached with the rooms like RMO charges, Nursing charges etc. : In this field the total charge of services availed other than room charges and room facilities charges is displayed. 3.18

19 Total payable Concession(%) Concessions Amt. Dpt. wise Concessions Credit Note Net payable Paid Balance Refund Amount : This is the total payable amount i.e. the some of room charges, room facilities and other services. : Enter the concesssiona in percentage if any allowed to this patient here. : In this field enter the concession in amount (if allowed). : This field will show the total concession amount when entered from department wise concession option. (describe later on). : This field will show the credit note amount if any issue to the patient(medicine return etc.). : This is the final net payable amount i.e. total payable - concessions - credit note. : This field shows the total amount paid till now. : Net payable - paid amount is the balance amount : If the paid amount exceeds net payable amount the balance amount has to be refunded to the patient. After refund entry (describe later on) the refunded amount will be shown in this field. On the bottom of the left side you will see few buttons. Add New Payment : This button is for adding the final balance payment at discharge. It has same fields and work similar to the payment detail option discussed earlier. 3.19

20 Concession Details : Here we enter the concession if any allowed along with its details like the concession authority or the person who has allowed the concession. Add details in concession detail column and amount in amount column and press Ok. Refund : We will click this button whenever we have to refund the excess money.the software will show the Patient no., Refund No., Refund Date and the amount to be refunded. Enter the description and Refund payment type(default C for cash) and press Save & Print button to print the Refund Voucher. Press Ok to move to previous screen. 3.20

21 Credit Note : If there is any medicine return or any other credit note to be issued to the patient click on Credit Note button. It will show the Reg. No., Credit Note no. and current date on the screen as shown below. Enter the Amount, Description and Department code and press Ok. Adjust against previous Payment : If the patient has made any payment before getting admitted for reservation or any other purpose, that payment will be adjusted through this button. The window will show Reg. No., Adjustmnet Voucher no. and Date by default. Enter the previous receipt no.(the receipt no. of the previous payment made) or select from the list. It will show you the Amount. Enter the De scription and press Ok. 3.21

22 Department wise concessions : If the concession given to the patient varies with the department. i.e. different conces sion for different departements then we give concession from this window. This will show all the departments whose services are availed by the patient along with their respective Billed Amt.. Enter the Concession in percentage or in amount against dif ferent departments. If you want to give uniform discount from all departments, add the percentage discount in the box Add to all Departments concession and press Add button. The percentage return here wiil be edit to all departments, you can further update any single department concession as well. After entering the values press Ok. Show Refunds : This window will show the refund entries if any given to this patient. 3.22

23 Show Credit Notes : This window will show the refund entries if any given to this patient. Show Adjustments : This window will show the refund entries if any given to this patient. In this window at the bottom on the right sidethe following options are available. Consultant share: click this box for consultant share amount. Print Bill : Click this box if you want to print the detailed bill. 3.23

24 Print Summarized Bill : Click this box if you want to print the summarized bill (department total only). Printing Format Draft or Windows : Click on Draft format if you want to print the bill on dot matrix printer and click on Windows if you want to print in windows format either on laser, inkjet or dot matrix printer. Press Ok button at the top to print the bill and exit from this screen. Discharge Summary: This tab is used to make Discharge Summary of the patient. This summary will be helpful as it can be given to the patient while discharge and at the same time the hospital will have on its record the clinical details during discharge. Weight Operation Discussion Discharge Comments : Enter the patient weight at the time of discharge here. : Enter the date of operation/surgery here. : Enter Discussion details here. : This is for complete details including medicine advised, follow up visits and for entering the clinical findings during the patients stay in the hospital. Patient s investigation reports and treatment given can also be mentioned here. : Enter any comments here. Patient Status : Enter the final status of the patient at the time of discharge in this field. You can choose from the options given in the masters like recovered, referred, LAMA etc. 3.24

25 Package Taken : Here we enter the packages availed by the patient during his stay. To add a new package, click on Add New Package button at the bottom, the following screen will appear. Enter the package code or select from the list. The software will give you the package duration (Date From and Date To) as defined in package for ex. if the package is for three days, the software will show the current date as date from and the third day as date to. You can also change the dates if the package is availed from a previous date. Enter the consultant incharge name in consultant column and press Ok. More than one packages can also be entered for the same patient. 3.25

26 OPD: All the out patient entries will be made through the OPD option (third option from the Main Menu). Whenever a new patient enters the hospital he has to be registered with the hospital. There are many ways of making entry for a new OPD patient. First, method is to go by new OPD registration screen, the second method is to go directly from OPD detail option. If we want to just make entry of consultation OPD and have a OPD card printout then we go through new OPD registration method otherwise if we don t want OPD card printout or the patient has come to avail any other service apart from consultation we go through General OPD option and enter service detail. NEW OPD REGISTRATION: This is the screen for OPD registration. This option is used when we want to print an OPD card for consultation OPD. OPD no. Date Patient No. Prefix & Name Sex,Age Father/Guardian Occupation Address & Tel Status Refred by Panel : The software will itself give a new OPD No. to the new patient This is OPD registration No. : Current date and time will be displayed here. : A new patient no. will be alloted to the patient. Enter the previous no. if the patient has allready been alloted any number earlier. : Select the prefix and patient name here. : Enter patient age and sex. : Enter father/husband/guardian name here. : Enter the patient occupation here. : Enter complete patient address & Telephone number here. : Select OPD status, G for general OPD, E for emergency OPD, and F for free OPD. : Enter refering consultant code here. : Enter panel code here if applicable. Consultant Incharge: Enter cosultant to be consulted here. 3.26

27 Press Ok after completing the entries, click on preview or print if you want to see preview or correctly print the OPD card. EDIT PREVIOUS REGISTRATION: If we have made any mistake or for any other reason, we want to edit the OPD registration click on this option and select from the list of previous registration as shown below. Now you can edit any value or code as required and press Ok to continue. 3.27

28 OPD ENTRY : Whenever we have to make any entry of OPD, or to say any OPD bill we go to this option. This will first ask whether the patient is already registered, (OPD card patient) or the patient is a new patient (general patient). OPD Card Patient: The OPD card patient is the patient who has come to the hospital basically for consultation and we first make an OPD card through new registration option which is just describe. In OPD card patient the patient details like name, age, sex etc. are already recorded and a patient no. as well as and OPD registration no. is alloted to him. When you click this option the following window will appear. The current date will be displayed automatically. Select the OPD card number from the list or from his OPD card and enter inhis field and Click on go button to move further. General Patient: All the patient s in OPD whose OPD card is not made or required comes under general patient. Here we make complete entry of the patient including his demographic details on this screen itself. click on general patient and the software will show the current date, press enter on date and the cursor will move on to patient code. A new patient code will be alloted to the patient. Now press enter or Go and the software will show the window for patient details entry. 3.28

29 Prefix: Choose prefix. Name: Enter the name here. Age Sex : Enter the patient age here. : Choose patient sex (male or female). If we want to enter extra detail of the patient like address, phone no., refer by, panel etc., click on More Info button or simply Alt+ M. The software will display complete patient details window. Months, days : If the patient is below 1 year of age enter age in months and similarly enter age in days if the patient is below one month. Address Phone Reff. By Panel : There are three lines for address fill complete address of the patient along with state and pincode. : Enter the telephone numbers here. Use second field for mobile number. : Fill the reffering doctor code here. : Enter the penal name here if applicable Panel ID No. : Panel ID number is to be entered in this field. 3.29

30 After entering the patient details, when we press enter on last field, the cursor will move to service field as shown below. Enter the service code or select from the list provided. The software will show you the service name, its charges and the consultant name if the service is linked with the consultant. Service: Enter the service code and if you don t know the service code, press enter on blank field or if you have typed any wrong code a list of departments will be displayed. This screen will show you all the departments of the hospital. Type the department name initials like PATH for PA- THOLOGY CHARGES and the cursor will move to that department. Once the department is highlighted, press enter or Ok. A new screen will display all the services of the selected department. 3.30

31 Select the current order (code/name). Enter the service name initials like A for all services starting from A and ALB for all services starting from ALB like ALBUMIN. To Select a particular entry just click on it, or press enter on that row. Its color will change and a star will appear in the last column. Now you can select the other services in the same manner. When done press F5 or Ok. The selected entries will be displayed in the grid form on main screen. Name : This field display the service name. Rate Cons. Code : The service charges will be displayed here. If we have not defined any fixed charges to a service, we can enter the charges here. : If this service is performed by a consultant and we want its details tobe transferred to consultant account, then we have to enter the consultant code here. For ex. if an Ultrasound is performed, then we will enter the consultant code here who have done this Ultrasound and its detail will be shown in doctor s record. Cons. Name : This will show the name of the consultant whose code is entered in Cons. code field %) Conc.: If we want to give any concession or discount to the patient on a particular service or services, enter the percent age of concession in this field. LAB NO: This will display the Lab no. if the service belongs to Laboratory. Qty. : Some services like medicines or consumable are billed in multiple quantities, enter the service quantity here if more than one. At the bottom of the screen the total amount of the services is shown alongwith the Net Payable amount. Below the window. Total : This field will show the total bill amount. Net Payable: Here you can see the net payable amount i.e., total - concession. 3.31

32 Amount paid : By default amount paid field will show the net payable amount, but if the paid amount is less than net payable amount, press F7 and enter the paid amount and its details. (describe lator on) The actual paid amount will be displayed here thereafter. Balance: Balance amount if any will be displayed here. OPD Bill No. : This field will automatically show new OPD bill number when we save this bill. Receipt Number : A new receipt number will be displayed here on saving the bill. On the right hand side there are three options for printing. Select receipt if you want to print receipt of this patient, select bill if you want to print detailed bill or select summarized bill for printing summarized bill (only department total amount). In the bottom of this window we will see these options:- Save : Click on save button or Alt+Stosave the bill and move to next entry. You can also press F11 key to save the bill. Save & Draft Print: For draft printing of receipt /bill click on this button. Cancel : Cancel the bill without saving it click on cancel button. Save & Preview: To preview the bill/receipt click on this button. Save & Windows Print: For direct window printing click on this button. Settings: This window will help to tune the software as per your specific requirements. Here we enter specific settings like Page, length, bill header & footer, receipt header& footer and many more such things. We call this system tunning. we will discuss about system tunning in our transaction section later in this manual. 3.32

33 Below these buttons we will see some keys, they are help keys. Now we will discuss the utility of these keys. F2: - F2 key is used for OT entry. This button is used to make all entries of OT at one time into the bill. The following screen will be displayed on when we press F2 button. All the services related to OT like Surgery, Anethesia, Medicines, OT Charges and any Other Information will be entered in this module. At the bottom of this screen the software shows the Grand Total amount. Detail of this module will be described later on in the manual. F3 F4 : F3 key is used for going to next service. When you press F3 key automatically you can see the cursor will directly move to next service field. This key is very useful for fast entry as we don t have to use mouse. : F4 key is used for concession; if you want give concession to the patient on the whole bill then use F4 key and follow ing will appear. Enter the concession authority ( the person who has authorised the concession) or select from the list. Now you can enter the concession either in percentage or in amount. Now enter concession description if required. 3.33

34 F5:-F5 key is used for patient information, if you want any information of patient then use this key. F6:- F6 key is used for Delete the service. If you want to delete any service, select the service and press F6 key. A message will appear to confirm. Are you sure you want to delete the entry if you are sure to delete then press Yes and if you don t want delete the entry then press No. F7:-F7 key is used for payment mode.
