She had back surgery to alleviate severe pain from a herniated disk, then learned she had type 2 diabetes. But she s feeling great these days and enjoying new activities, like skiing, biking and weight training. Rarely do I even feel like I had back surgery, she said. And I ve been able to reduce my diabetes medication and manage my blood sugar levels very well. Looft, a 36-year-old purchasing professional, gives her team of caring primary and specialty physicians at St. Francis Regional Medical Center a lot of the credit for her good health and confidence about the future. On the St. Francis campus, patients can see family medicine doctors and specialists, have same-day diagnostic testing, and receive physical therapy and pain management services, such as steroid injections. Looft s spinal saga Looft s severe back pain lasted for about six months, starting after a spring vacation. That summer, after chiropractic care and physical therapy didn t help, her primary care doctor at Allina Health Shakopee Clinic ordered an MRI, then referred her to Kevin Mullaney, MD. She was on narcotic pain medication and muscle relaxers. Looft would time her medication to get through her workday, then try often unsuccessfully to sleep at night. I was barely able to function. I was really like a walking zombie, she recalled. I m a fast walker, and one day it took me 30 minutes to get from the door of the building to my office. After one steroid injection didn t help and a second injection gave Looft relief for just one week, Mullaney recommended surgery. I ve worked at 15 to 18 different hospitals and have never seen a team work this well in unison. Kevin Mullaney, MD I was quite nervous, but Dr. Mullaney helped because he talks in basic terms, Looft said. He explained the surgery so I understood it. I felt I could ask him anything. He gave great before and after instructions and made me realize how much your back does and how to take care of it with things like good posture. The results of the outpatient procedure greatly exceeded her expectations. Looft was off pain medication within a few days after surgery. She recovered at home for six weeks but started walking right away, following Mullaney s instructions. She had no restrictions beyond not lifting heavy weight for a while, and she did not need postsurgery rehab. It was amazing, Looft said. I ve had many surgeries, and this was by far the easiest and best. If Dr. Mullaney wanted to do surgery on me again, I wouldn t hesitate one second. In fact, she recommended the surgeon to her best friend, who also had a good experience. Ward Godsall, MD, endocrinology, Allina Health Shakopee Clinic Top spinal care in an integrated network Mullaney is a top regional spine surgeon who was one of Mpls.St.Paul Magazine s Top Doctors in 2013 and He comes from a family of engineers but said he always wanted to work with people rather than mechanical parts. He has been a spine surgeon for 10 years, focusing on minimally invasive neck and low back procedures, now including implantation of cervical artificial disks. Many of his procedures are minimally invasive microdiskectomies, like Looft s surgery, in which a piece of a herniated lumbar disk is removed through a small incision to make more room for a nerve that is being compressed and causing back and leg pain. Sciatica that causes back and leg pain is very common, Mullaney said. Patients have tremendous anxiety about spinal surgery, but with lumbar diskectomy, there s no chance of paralysis and 90 to 95 percent of the time, it alleviates the pain. My job is to educate, ease patients fears and give them choices. A majority of the time, we don t have to do surgery. People with back pain can start with a primary care physician or physical therapist, Mullaney said, but see an orthopedic surgeon right away if disk herniation starts causing leg weakness, pelvic numbness or incontinence. Mullaney calls each of his patients the night before surgery to see if they have any last-minute questions or concerns. I love to work at St. Francis and would do surgery here every day if I could, Mullaney said. Physical therapy is right next to our clinic, and the communication among the integrated network of physicians is great. The patient experience really is superior; I ve done probably 1,000 surgeries at St. Francis with absolutely no complaints because of the quality of the care team. I ve worked at 15 to 18 different hospitals and have never seen a team work this well in unison. Diabetes dilemma After sailing through her back surgery, Looft had a checkup with Robert Nicholson, MD, her family medicine doctor at Allina Health Shakopee Clinic. She described him as a good listener who has a fun, lighthearted side. 2 partners in health
3 Rarely do I even feel like I had back surgery, Looft said. I complained that I was extremely tired. He checked a few things, including my blood sugar, and found it to be high that day and when it was rechecked, Looft said. Dr. Nicholson recommended that I see an endocrinologist. Nicholson said he works with a number of patients with diabetes and often handles the care management himself, but he appreciates the specialty resources available at St. Francis such as J. Ward Godsall, MD, the endocrinologist at his own clinic, who Looft chose. I love Dr. Godsall, Looft said. The first time he saw me, I told him I wanted to change my medication dosage and why that I wanted to manage my diabetes. He had a skeptical look on his face. Then he got my test results back and gave me a nice compliment, saying my A1c level would probably be the best one he d see all day. One campus offers the care you need Nicholson said it s uncommon for him or his care team partner, Jackie Tewes, certified nurse practitioner, to have to look beyond their clinic and the St. Francis campus for specialty resources. They talk daily with doctors who are just down the hallway. Nicholson said he can consult a kidney specialist about hard-to-manage high blood pressure, ask a cardiologist to look over test results or show an X-ray to an orthopedic surgeon. This extends our knowledge and gives our patients access to specialists through me and Jackie, Nicholson said. From our experience, there is good teamwork and communication throughout the St. Francis campus. As for Looft, she s a prime example of why things work so well and harmoniously at St. Francis for both patients and doctors. Primary care physicians, nurse practitioners and physician assistants provide comprehensive, informed care and serve as an engine for referrals to the specialists, who are close at hand for consultations and specialized treatment when needed. McMahan succeeds Mike Baumgartner, who left St. Francis to become president of Baptist Health Madisonville in Kentucky. McMahan comes to St. Francis from the HealthEast Care System, where he served in hospital and clinical leadership roles since His most recent position was the vice president of Acute Care Operations at St. John s Hospital, St. Joseph s Hospital and Woodwinds Health Campus for HealthEast. Under McMahan s leadership at HealthEast, the hospital group developed and implemented a system leadership approach to surgical services, radiology, pharmacy, security, guest services and plant operations. He also created and implemented improvements that led to higher quality at a reduced cost and championed initiatives resulting in improved patient experience scores and employee engagement throughout his areas of responsibility. Grilled meats have a reduced fat and calorie content, as the fat drips off while cooking. Or consider a BLT; even with a swipe of mayo, it s usually a lower calorie choice. When in doubt, ask for a half-order, share a main dish with a companion or pack half your meal to go immediately when delivered. The trick, and benefit, is all in how and what you order.
